igc: Remove temporary workaround

PHY_CONTROL register works as defined in the IEEE 802.3 specification
(IEEE 802.3-2008 22.2.4.1). Tidy up the temporary workaround.

User impact: PHY can now be powered down when the ethernet link is down.

Testing hints: ip link set down <device> (or just disconnect the
ethernet cable).

Oldest tested NVM version is: 1045:740.

igc: field get conversion

Refactor the igc driver to use FIELD_GET() for mask and shift reads,
which reduces lines of code and adds clarity of intent.

This code was generated by the following coccinelle/spatch script and
then manually repaired in a later patch.

intel: add bit macro includes where needed

This series is introducing the use of FIELD_GET and FIELD_PREP which
requires bitfield.h to be included. Fix all the includes in this one
change, and rearrange includes into alphabetical order to ease
readability and future maintenance.

virtchnl.h and it's usage was modified to have it's own includes as it
should. This required including bits.h for virtchnl.h.

Complete to commit 8e153faf5827 ("igc: Remove unused phy type")
i225 parts have only one PHY. There is no point to use phy_type enum.
Clean up the code accordingly, and get rid of the unused enum lines.

Replace usleep_range() method with udelay() method to allow atomic contexts
in low-level MDIO access functions.

igc: igc_write_phy_reg_gpy: drop premature return

Similar to "igc_read_phy_reg_gpy: drop premature return" patch.
igc_write_phy_reg_gpy checks the return value from igc_write_phy_reg_mdic
and if it's not 0, returns immediately. By doing this, it leaves the HW
semaphore in the acquired state.

Drop this premature return statement, the function returns after
releasing the semaphore immediately anyway.

i225 devices have only one PHY vendor. There is no point checking
_I_PHY_ID during the link establishment and auto-negotiation process.
