powerpc: Refactor __kernel_map_pages() __kernel_map_pages() is almost identical for PPC32 and RADIX. Refactor it. On PPC32 it is not needed for KFENCE, but to keep it simple just make it similar to PPC64. Move the prototype of hash__kernel_map_pages() into mmu_decl.h to allow IS_ENABLED() to work on 32-bit. mm/powerpc: define pXd_large() with pXd_leaf() Patch series "mm/treewide: Replace pXd_large() with pXd_leaf()", v3. These two APIs are mostly always the same. It's confusing to have both of them. Merge them into one. Here I used pXd_leaf() only because pXd_leaf() is a global API which is always defined, while pXd_large() is not. We have yet one more API that is similar which is pXd_huge(), but that's even trickier, so let's do it step by step. Some special cares are taken for ppc and x86, they're done as separate cleanups first. This patch (of 10): The two definitions are the same. The only difference is that pXd_large() is only defined with THP selected, and only on book3s 64bits. Instead of implementing it twice, make pXd_large() a macro to pXd_leaf(). Define it unconditionally just like pXd_leaf(). This helps to prepare merging the two APIs. powerpc/book3s/hash: Drop _PAGE_PRIVILEGED from PAGE_NONE There used to be a dependency on _PAGE_PRIVILEGED with pte_savedwrite. But that got dropped by commit 6a56ccbcf6c6 ("mm/autonuma: use can_change_(pte|pmd)_writable() to replace savedwrite") With the change in this patch numa fault pte (pte_protnone()) gets mapped as regular user pte with RWX cleared (no-access) whereas earlier it used to be mapped _PAGE_PRIVILEGED. Hash fault handling code gets some WARN_ON added in this patch because those functions are not expected to get called with _PAGE_READ cleared. commit 18061c17c8ec ("powerpc/mm: Update PROTFAULT handling in the page fault path") explains the details. powerpc: Support execute-only on all powerpc Introduce PAGE_EXECONLY_X macro which provides exec-only rights. The _X may be seen as redundant with the EXECONLY but it helps keep consistency, all macros having the EXEC right have _X. And put it next to PAGE_NONE as PAGE_EXECONLY_X is somehow PAGE_NONE + EXEC just like all other SOMETHING_X are just SOMETHING + EXEC. On book3s/64 PAGE_EXECONLY becomes PAGE_READONLY_X. On book3s/64, as PAGE_EXECONLY is only valid for Radix add VM_READ flag in vm_get_page_prot() for non-Radix. And update access_error() so that a non exec fault on a VM_EXEC only mapping is always invalid, even when the underlying layer don't always generate a fault for that. For 8xx, set PAGE_EXECONLY_X as _PAGE_NA | _PAGE_EXEC. For others, only set it as just _PAGE_EXEC With that change, 8xx, e500 and 44x fully honor execute-only protection. On 40x that is a partial implementation of execute-only. The implementation won't be complete because once a TLB has been loaded via the Instruction TLB miss handler, it will be possible to read the page. But at least it can't be read unless it is executed first. On 603 MMU, TLB missed are handled by SW and there are separate DTLB and ITLB. Execute-only is therefore now supported by not loading DTLB when read access is not permitted. On hash (604) MMU it is more tricky because hash table is common to load/store and execute. Nevertheless it is still possible to check whether _PAGE_READ is set before loading hash table for a load/store access. At least it can't be read unless it is executed first. mm: Rename arch pte_mkwrite()'s to pte_mkwrite_novma() The x86 Shadow stack feature includes a new type of memory called shadow stack. This shadow stack memory has some unusual properties, which requires some core mm changes to function properly. One of these unusual properties is that shadow stack memory is writable, but only in limited ways. These limits are applied via a specific PTE bit combination. Nevertheless, the memory is writable, and core mm code will need to apply the writable permissions in the typical paths that call pte_mkwrite(). The goal is to make pte_mkwrite() take a VMA, so that the x86 implementation of it can know whether to create regular writable or shadow stack mappings. But there are a couple of challenges to this. Modifying the signatures of each arch pte_mkwrite() implementation would be error prone because some are generated with macros and would need to be re-implemented. Also, some pte_mkwrite() callers operate on kernel memory without a VMA. So this can be done in a three step process. First pte_mkwrite() can be renamed to pte_mkwrite_novma() in each arch, with a generic pte_mkwrite() added that just calls pte_mkwrite_novma(). Next callers without a VMA can be moved to pte_mkwrite_novma(). And lastly, pte_mkwrite() and all callers can be changed to take/pass a VMA. Start the process by renaming pte_mkwrite() to pte_mkwrite_novma() and adding the pte_mkwrite() wrapper in linux/pgtable.h. Apply the same pattern for pmd_mkwrite(). Since not all archs have a pmd_mkwrite_novma(), create a new arch config HAS_HUGE_PAGE that can be used to tell if pmd_mkwrite() should be defined. Otherwise in the !HAS_HUGE_PAGE cases the compiler would not be able to find pmd_mkwrite_novma(). No functional change. powerpc/book3s64/mm: enable transparent pud hugepage This is enabled only with radix translation and 1G hugepage size. This will be used with devdax device memory with a namespace alignment of 1G. Anon transparent hugepage is not supported even though we do have helpers checking pud_trans_huge(). We should never find that return true. The only expected pte bit combination is _PAGE_PTE | _PAGE_DEVMAP. Some of the helpers are never expected to get called on hash translation and hence is marked to call BUG() in such a case. powerpc/64s: Enable KFENCE on book3s64 KFENCE support was added for ppc32 in commit 90cbac0e995d ("powerpc: Enable KFENCE for PPC32"). Enable KFENCE on ppc64 architecture with hash and radix MMUs. It uses the same mechanism as debug pagealloc to protect/unprotect pages. All KFENCE kunit tests pass on both MMUs. KFENCE memory is initially allocated using memblock but is later marked as SLAB allocated. This necessitates the change to __pud_free to ensure that the KFENCE pages are freed appropriately. Based on previous work by Christophe Leroy and Jordan Niethe. powerpc/mm/64s: Drop p4d_leaf() Because 64-bit Book3S uses pgtable-nop4d.h, the P4D is folded into the PGD. So P4D entries are actually PGD entries, or vice versa. The other way to think of it is that the P4D is a single entry page table below the PGD. Zero bits of the address are needed to index into the P4D, therefore a P4D entry maps the same size address space as a PGD entry. As explained in the previous commit, there are no huge page sizes supported directly at the PGD level on 64-bit Book3S, so there are also no huge page sizes supported at the P4D level. Therefore p4d_is_leaf() can never be true, so drop the definition and fallback to the default implementation that always returns false. powerpc/64s: add pte_needs_flush and huge_pmd_needs_flush Allow PTE changes to avoid flushing the TLB when access permissions are being relaxed, the dirty bit is being set, and the accessed bit is being changed. Relaxing access permissions and setting dirty and accessed bits do not require a flush because the MMU will re-load the PTE and notice the updates (it may also cause a spurious fault). Clearing the accessed bit does not require a flush because of the imprecise PTE accessed bit accounting that is already performed, as documented in ptep_clear_flush_young(). This reduces TLB flushing for some mprotect(2) calls. powerpc/mm: Support execute-only memory on the Radix MMU Add support for execute-only memory (XOM) for the Radix MMU by using an execute-only mapping, as opposed to the RX mapping used by powerpc's other MMUs. The Hash MMU already supports XOM through the execute-only pkey, which is a separate mechanism shared with x86. A PROT_EXEC-only mapping will map to RX, and then the pkey will be applied on top of it. mmap() and mprotect() consumers in userspace should observe the same behaviour on Hash and Radix despite the differences in implementation. Replacing the vma_is_accessible() check in access_error() with a read check should be functionally equivalent for non-Radix MMUs, since it follows write and execute checks. For Radix, the change enables detecting faults on execute-only mappings where vma_is_accessible() would return true. powerpc: Book3S 64-bit outline-only KASAN support Implement a limited form of KASAN for Book3S 64-bit machines running under the Radix MMU, supporting only outline mode. - Enable the compiler instrumentation to check addresses and maintain the shadow region. (This is the guts of KASAN which we can easily reuse.) - Require kasan-vmalloc support to handle modules and anything else in vmalloc space. - KASAN needs to be able to validate all pointer accesses, but we can't instrument all kernel addresses - only linear map and vmalloc. On boot, set up a single page of read-only shadow that marks all iomap and vmemmap accesses as valid. - Document KASAN in powerpc docs. Background ---------- KASAN support on Book3S is a bit tricky to get right: - It would be good to support inline instrumentation so as to be able to catch stack issues that cannot be caught with outline mode. - Inline instrumentation requires a fixed offset. - Book3S runs code with translations off ("real mode") during boot, including a lot of generic device-tree parsing code which is used to determine MMU features. [ppc64 mm note: The kernel installs a linear mapping at effective address c000...-c008.... This is a one-to-one mapping with physical memory from 0000... onward. Because of how memory accesses work on powerpc 64-bit Book3S, a kernel pointer in the linear map accesses the same memory both with translations on (accessing as an 'effective address'), and with translations off (accessing as a 'real address'). This works in both guests and the hypervisor. For more details, see s5.7 of Book III of version 3 of the ISA, in particular the Storage Control Overview, s5.7.3, and s5.7.5 - noting that this KASAN implementation currently only supports Radix.] - Some code - most notably a lot of KVM code - also runs with translations off after boot. - Therefore any offset has to point to memory that is valid with translations on or off. One approach is just to give up on inline instrumentation. This way boot-time checks can be delayed until after the MMU is set is up, and we can just not instrument any code that runs with translations off after booting. Take this approach for now and require outline instrumentation. Previous attempts allowed inline instrumentation. However, they came with some unfortunate restrictions: only physically contiguous memory could be used and it had to be specified at compile time. Maybe we can do better in the future. We cannot use 0x40, as that collides with _RPAGE_RSV1 -- contained in _PAGE_HPTEFLAGS. The next candidate would be _RPAGE_SW3 (0x200) -- which is used for _PAGE_SOFT_DIRTY for !swp ptes. So let's just use _PAGE_SOFT_DIRTY for _PAGE_SWP_SOFT_DIRTY (to make it easier to grasp) and use 0x20 now for _PAGE_SWP_EXCLUSIVE. powerpc/pgtable: remove _PAGE_BIT_SWAP_TYPE for book3s The swap type is simply stored in bits 0x1f of the swap pte. Let's simplify by just getting rid of _PAGE_BIT_SWAP_TYPE. It's not like that we can simply change it: _PAGE_SWP_SOFT_DIRTY would suddenly fall into _RPAGE_RSV1, which isn't possible and would make the BUILD_BUG_ON(_PAGE_HPTEFLAGS & _PAGE_SWP_SOFT_DIRTY) angry. While at it, make it clearer which bit we're actually using for _PAGE_SWP_SOFT_DIRTY by just using the proper define and introduce and use SWP_TYPE_MASK. powerpc/s64: Clarify that radix lacks DEBUG_PAGEALLOC The page_alloc.c code will call into __kernel_map_pages() when DEBUG_PAGEALLOC is configured and enabled. As the implementation assumes hash, this should crash spectacularly if not for a bit of luck in __kernel_map_pages(). In this function linear_map_hash_count is always zero, the for loop exits without doing any damage. There are no other platforms that determine if they support debug_pagealloc at runtime. Instead of adding code to mm/page_alloc.c to do that, this change turns the map/unmap into a noop when in radix mode and prints a warning once. powerpc/64: Fix the definition of the fixmap area At the time being, the fixmap area is defined at the top of the address space or just below KASAN. This definition is not valid for PPC64. For PPC64, use the top of the I/O space. Because of circular dependencies, it is not possible to include asm/fixmap.h in asm/book3s/64/pgtable.h , so define a fixed size AREA at the top of the I/O space for fixmap and ensure during build that the size is big enough. powerpc/mm/64s: Add _PAGE_KERNEL_ROX In the past we had a fallback definition for _PAGE_KERNEL_ROX, but we removed that in commit d82fd29c5a8c ("powerpc/mm: Distribute platform specific PAGE and PMD flags and definitions") and added definitions for each MMU family. However we missed adding a definition for 64s, which was not really a bug because it's currently not used. But we'd like to use PAGE_KERNEL_ROX in a future patch so add a definition now. powerpc/mm: Move setting PTE specific flags to pfn_pmd() powerpc used to set the PTE specific flags in set_pte_at(). That is different from other architectures. To be consistent with other architectures powerpc updated pfn_pte() to set _PAGE_PTE in commit 379c926d6334 ("powerpc/mm: move setting pte specific flags to pfn_pte") That commit didn't do the same for pfn_pmd() because we expect pmd_mkhuge() to do that. But as per Linus that is a bad rule: The rule that you must use "pmd_mkhuge()" seems _completely_ wrong. The only valid use to ever make a pmd out of a pfn is to make a huge-page. Hence update pfn_pmd() to set _PAGE_PTE. powerpc/mm/book3s: Split radix and hash MAX_PHYSMEM limit MAX_PHYSMEM #define is used along with sparsemem to determine the SECTION_SHIFT value. Powerpc also uses the same value to limit the max memory enabled on the system. With 4K PAGE_SIZE and hash translation mode, we want to limit the max memory enabled to 64TB due to page table size restrictions. However, with radix translation, we don't have these restrictions. Hence split the radix and hash MA_PHYSMEM limit and use different limit for each of them. powerpc/64s: Remove PROT_SAO support ISA v3.1 does not support the SAO storage control attribute required to implement PROT_SAO. PROT_SAO was used by specialised system software (Lx86) that has been discontinued for about 7 years, and is not thought to be used elsewhere, so removal should not cause problems. We rather remove it than keep support for older processors, because live migrating guest partitions to newer processors may not be possible if SAO is in use (or worse allowed with silent races). - PROT_SAO stays in the uapi header so code using it would still build. - arch_validate_prot() is removed, the generic version rejects PROT_SAO so applications would get a failure at mmap() time. powerpc/book3s64/pkeys: Move pkey related bits in the linux page table To keep things simple, all the pkey related bits are kept together in linux page table for 64K config with hash translation. With hash-4k kernel requires 4 bits to store slots details. This is done by overloading some of the RPN bits for storing the slot details. mm: consolidate pte_index() and pte_offset_*() definitions All architectures define pte_index() as (address >> PAGE_SHIFT) & (PTRS_PER_PTE - 1) and all architectures define pte_offset_kernel() as an entry in the array of PTEs indexed by the pte_index(). For the most architectures the pte_offset_kernel() implementation relies on the availability of pmd_page_vaddr() that converts a PMD entry value to the virtual address of the page containing PTEs array. Let's move x86 definitions of the PTE accessors to the generic place in <linux/pgtable.h> and then simply drop the respective definitions from the other architectures. The architectures that didn't provide pmd_page_vaddr() are updated to have that defined. The generic implementation of pte_offset_kernel() can be overridden by an architecture and alpha makes use of this because it has special ordering requirements for its version of pte_offset_kernel(). Before that it drops an existing pmd_mknotpresent() definition from powerpc platform which was never required as it defines it's pmdp_invalidate() through subscribing __HAVE_ARCH_PMDP_INVALIDATE. This does not create any functional change. powerpc/64s/radix: Don't prefetch DAR in update_mmu_cache The idea behind this prefetch was to kick off a page table walk before returning from the fault, getting some pipelining advantage. But this never showed up any noticable performance advantage, and in fact with KUAP the prefetches are actually blocked and cause some kind of micro-architectural fault. Removing this improves page fault microbenchmark performance by about 9%. powerpc/mm/book3s64: Fix MADV_DONTNEED and parallel page fault race MADV_DONTNEED holds mmap_sem in read mode and that implies a parallel page fault is possible and the kernel can end up with a level 1 PTE entry (THP entry) converted to a level 0 PTE entry without flushing the THP TLB entry. Most architectures including POWER have issues with kernel instantiating a level 0 PTE entry while holding level 1 TLB entries. The code sequence I am looking at is down_read(mmap_sem) down_read(mmap_sem) zap_pmd_range() zap_huge_pmd() pmd lock held pmd_cleared table details added to mmu_gather pmd_unlock() insert a level 0 PTE entry() tlb_finish_mmu(). Fix this by forcing a tlb flush before releasing pmd lock if this is not a fullmm invalidate. We can safely skip this invalidate for task exit case (fullmm invalidate) because in that case we are sure there can be no parallel fault handlers. powerpc/mm/hash64: use _PAGE_PTE when checking for pte_present This makes the pte_present check stricter by checking for additional _PAGE_PTE bit. A level 1 pte pointer (THP pte) can be switched to a pointer to level 0 pte page table page by following two operations. 1) THP split. 2) madvise(MADV_DONTNEED) in parallel to page fault. A lockless page table walk need to make sure we can handle such changes gracefully. powerpc/hash64/devmap: Use H_PAGE_THP_HUGE when setting up huge devmap PTE entries H_PAGE_THP_HUGE is used to differentiate between a THP hugepage and hugetlb hugepage entries. The difference is WRT how we handle hash fault on these address. THP address enables MPSS in segments. We want to manage devmap hugepage entries similar to THP pt entries. Hence use H_PAGE_THP_HUGE for devmap huge PTE entries. With current code while handling hash PTE fault, we do set is_thp = true when finding devmap PTE huge PTE entries. For this it needs to know when it has reached a 'leaf' entry in the page tables. This information is provided by the p?d_leaf() functions/macros. For powerpc p?d_is_leaf() functions already exist. Export them using the new p?d_leaf() name. For PPC64, we add the 4K PFN hack sanity check to __ioremap_caller() in order to avoid using __ioremap_at(). Other checks in __ioremap_at() are irrelevant for __ioremap_caller(). mm: introduce ARCH_HAS_PTE_DEVMAP ARCH_HAS_ZONE_DEVICE is somewhat meaningless in itself, and combined with the long-out-of-date comment can lead to the impression than an architecture may just enable it (since __add_pages() now "comprehends device memory" for itself) and expect things to work. In practice, however, ZONE_DEVICE users have little chance of functioning correctly without __HAVE_ARCH_PTE_DEVMAP, so let's clean that up the same way as ARCH_HAS_PTE_SPECIAL and make it the proper dependency so the real situation is clearer. powerpc/book3s: Use config independent helpers for page table walk Even when we have HugeTLB and THP disabled, kernel linear map can still be mapped with hugepages. This is only an issue with radix translation because hash MMU doesn't map kernel linear range in linux page table and other kernel map areas are not mapped using hugepage. Add config independent helpers and put WARN_ON() when we don't expect things to be mapped via hugepages. I don't have a benchmark for this change, but huge vmap will be used by a later core kernel change to enable huge vmalloc memory mappings. This improves cached `git diff` performance by about 5% on a 2-node POWER9 with 32MB size dentry cache hash. powerpc/64s: Fix THP PMD collapse serialisation Commit 1b2443a547f9 ("powerpc/book3s64: Avoid multiple endian conversion in pte helpers") changed the actual bitwise tests in pte_access_permitted by using pte_write() and pte_present() helpers rather than raw bitwise testing _PAGE_WRITE and _PAGE_PRESENT bits. The pte_present() change now returns true for PTEs which are !_PAGE_PRESENT and _PAGE_INVALID, which is the combination used by pmdp_invalidate() to synchronize access from lock-free lookups. pte_access_permitted() is used by pmd_access_permitted(), so allowing GUP lock free access to proceed with such PTEs breaks this synchronisation. This bug has been observed on a host using the hash page table MMU, with random crashes and corruption in guests, usually together with bad PMD messages in the host. Fix this by adding an explicit check in pmd_access_permitted(), and documenting the condition explicitly. The pte_write() change should be okay, and would prevent GUP from falling back to the slow path when encountering savedwrite PTEs, which matches what x86 (that does not implement savedwrite) does. powerpc/mm/hash64: Map all the kernel regions in the same 0xc range This patch maps vmalloc, IO and vmemap regions in the 0xc address range instead of the current 0xd and 0xf range. This brings the mapping closer to radix translation mode. With hash 64K page size each of this region is 512TB whereas with 4K config we are limited by the max page table range of 64TB and hence there regions are of 16TB size. powerpc/64s: Fix possible corruption on big endian due to pgd/pud_present() In v4.20 we changed our pgd/pud_present() to check for _PAGE_PRESENT rather than just checking that the value is non-zero, e.g.: static inline int pgd_present(pgd_t pgd) { - return !pgd_none(pgd); + return (pgd_raw(pgd) & cpu_to_be64(_PAGE_PRESENT)); } Unfortunately this is broken on big endian, as the result of the bitwise & is truncated to int, which is always zero because _PAGE_PRESENT is 0x8000000000000000ul. This means pgd_present() and pud_present() are always false at compile time, and the compiler elides the subsequent code. Remarkably with that bug present we are still able to boot and run with few noticeable effects. powerpc/mm: add pte helpers to query and change pte flags In order to avoid using generic _PAGE_XXX flags in powerpc core functions, define helpers for all needed flags: - pte_mkuser() and pte_mkprivileged() to set/unset and/or unset/set _PAGE_USER and/or _PAGE_PRIVILEGED - pte_hashpte() to check if _PAGE_HASHPTE is set. - pte_ci() check if cache is inhibited (already existing on book3s/64) - pte_exprotect() to protect against execution - pte_exec() and pte_mkexec() to query and set page execution - pte_mkpte() to set _PAGE_PTE flag. - pte_hw_valid() to check _PAGE_PRESENT since pte_present does something different on book3s/64. On book3s/32 there is no exec protection, so pte_mkexec() and pte_exprotect() are nops and pte_exec() returns always true. powerpc/mm/book3s: Update pmd_present to look at _PAGE_PRESENT bit With this patch we use 0x8000000000000000UL (_PAGE_PRESENT) to indicate a valid pgd/pud/pmd entry. We also switch the p**_present() to look at this bit. With pmd_present, we have a special case. We need to make sure we consider a pmd marked invalid during THP split as present. Right now we clear the _PAGE_PRESENT bit during a pmdp_invalidate. Inorder to consider this special case we add a new pte bit _PAGE_INVALID (mapped to _RPAGE_SW0). This bit is only used with _PAGE_PRESENT cleared. Hence we are not really losing a pte bit for this special case. pmd_present is also updated to look at _PAGE_INVALID. xarray: Replace exceptional entries Introduce xarray value entries and tagged pointers to replace radix tree exceptional entries. This is a slight change in encoding to allow the use of an extra bit (we can now store BITS_PER_LONG - 1 bits in a value entry). It is also a change in emphasis; exceptional entries are intimidating and different. As the comment explains, you can choose to store values or pointers in the xarray and they are both first-class citizens. KVM: PPC: Avoid marking DMA-mapped pages dirty in real mode At the moment the real mode handler of H_PUT_TCE calls iommu_tce_xchg_rm() which in turn reads the old TCE and if it was a valid entry, marks the physical page dirty if it was mapped for writing. Since it is in real mode, realmode_pfn_to_page() is used instead of pfn_to_page() to get the page struct. However SetPageDirty() itself reads the compound page head and returns a virtual address for the head page struct and setting dirty bit for that kills the system. This adds additional dirty bit tracking into the MM/IOMMU API for use in the real mode. Note that this does not change how VFIO and KVM (in virtual mode) set this bit. The KVM (real mode) changes include: - use the lowest bit of the cached host phys address to carry the dirty bit; - mark pages dirty when they are unpinned which happens when the preregistered memory is released which always happens in virtual mode; - add mm_iommu_ua_mark_dirty_rm() helper to set delayed dirty bit; - change iommu_tce_xchg_rm() to take the kvm struct for the mm to use in the new mm_iommu_ua_mark_dirty_rm() helper; - move iommu_tce_xchg_rm() to book3s_64_vio_hv.c (which is the only caller anyway) to reduce the real mode KVM and IOMMU knowledge across different subsystems. This removes realmode_pfn_to_page() as it is not used anymore. While we at it, remove some EXPORT_SYMBOL_GPL() as that code is for the real mode only and modules cannot call it anyway. powerpc/mm/books3s: Add new pte bit to mark pte temporarily invalid. When splitting a huge pmd pte, we need to mark the pmd entry invalid. We can do that by clearing _PAGE_PRESENT bit. But then that will be taken as a swap pte. In order to differentiate between the two use a software pte bit when invalidating. For regular pte, due to bd5050e38aec ("powerpc/mm/radix: Change pte relax sequence to handle nest MMU hang") we need to mark the pte entry invalid when relaxing access permission. Instead of marking pte_none which can result in different page table walk routines possibly skipping this pte entry, invalidate it but still keep it marked present. mm: introduce ARCH_HAS_PTE_SPECIAL Currently the PTE special supports is turned on in per architecture header files. Most of the time, it is defined in arch/*/include/asm/pgtable.h depending or not on some other per architecture static definition. This patch introduce a new configuration variable to manage this directly in the Kconfig files. It would later replace __HAVE_ARCH_PTE_SPECIAL. Here notes for some architecture where the definition of __HAVE_ARCH_PTE_SPECIAL is not obvious: arm __HAVE_ARCH_PTE_SPECIAL which is currently defined in arch/arm/include/asm/pgtable-3level.h which is included by arch/arm/include/asm/pgtable.h when CONFIG_ARM_LPAE is set. So select ARCH_HAS_PTE_SPECIAL if ARM_LPAE. powerpc __HAVE_ARCH_PTE_SPECIAL is defined in 2 files: - arch/powerpc/include/asm/book3s/64/pgtable.h - arch/powerpc/include/asm/pte-common.h The first one is included if (PPC_BOOK3S & PPC64) while the second is included in all the other cases. So select ARCH_HAS_PTE_SPECIAL all the time. sparc: __HAVE_ARCH_PTE_SPECIAL is defined if defined(__sparc__) && defined(__arch64__) which are defined through the compiler in sparc/Makefile if !SPARC32 which I assume to be if SPARC64. powerpc/mm: Fix crashes with 16G huge pages To support memory keys, we moved the hash pte slot information to the second half of the page table. This was ok with PTE entries at level 4 (PTE page) and level 3 (PMD). We already allocate larger page table pages at those levels to accomodate extra details. For level 4 we already have the extra space which was used to track 4k hash page table entry details and at level 3 the extra space was allocated to track the THP details. With hugetlbfs PTE, we used this extra space at the PMD level to store the slot details. But we also support hugetlbfs PTE at PUD level for 16GB pages and PUD level page didn't allocate extra space. This resulted in memory corruption. Fix this by allocating extra space at PUD level when HUGETLB is enabled. powerpc: map vma key-protection bits to pte key bits. Map the key protection bits of the vma to the pkey bits in the PTE. The PTE bits used for pkey are 3,4,5,6 and 57. The first four bits are the same four bits that were freed up initially in this patch series. remember? :-) Without those four bits this patch wouldn't be possible. BUT, on 4k kernel, bit 3, and 4 could not be freed up. remember? Hence we have to be satisfied with 5, 6 and 7. powerpc/mm: extend _PAGE_PRIVILEGED to all CPUs commit ac29c64089b74 ("powerpc/mm: Replace _PAGE_USER with _PAGE_PRIVILEGED") introduced _PAGE_PRIVILEGED for BOOK3S/64 This patch generalises _PAGE_PRIVILEGED for all CPUs, allowing to have either _PAGE_PRIVILEGED or _PAGE_USER or both. PPC_8xx has a _PAGE_SHARED flag which is set for and only for all non user pages. Lets rename it _PAGE_PRIVILEGED to remove confusion as it has nothing to do with Linux shared pages. powerpc/mm/book3s/64: Add proper pte access check helper pte_access_premitted get called in get_user_pages_fast path. If we have marked the pte PROT_NONE, we should not allow a read access on the address. With the current implementation we are not checking the READ and only check for WRITE. This is needed on archs like ppc64 that implement PROT_NONE using RWX access instead of _PAGE_PRESENT. Also add pte_user check just to make sure we are not accessing kernel mapping. License cleanup: add SPDX GPL-2.0 license identifier to files with no license Many source files in the tree are missing licensing information, which makes it harder for compliance tools to determine the correct license. By default all files without license information are under the default license of the kernel, which is GPL version 2. Update the files which contain no license information with the 'GPL-2.0' SPDX license identifier. The SPDX identifier is a legally binding shorthand, which can be used instead of the full boiler plate text. This patch is based on work done by Thomas Gleixner and Kate Stewart and Philippe Ombredanne. How this work was done: Patches were generated and checked against linux-4.14-rc6 for a subset of the use cases: - file had no licensing information it it. - file was a */uapi/* one with no licensing information in it, - file was a */uapi/* one with existing licensing information, Further patches will be generated in subsequent months to fix up cases where non-standard license headers were used, and references to license had to be inferred by heuristics based on keywords. The analysis to determine which SPDX License Identifier to be applied to a file was done in a spreadsheet of side by side results from of the output of two independent scanners (ScanCode & Windriver) producing SPDX tag:value files created by Philippe Ombredanne. Philippe prepared the base worksheet, and did an initial spot review of a few 1000 files. The 4.13 kernel was the starting point of the analysis with 60,537 files assessed. Kate Stewart did a file by file comparison of the scanner results in the spreadsheet to determine which SPDX license identifier(s) to be applied to the file. She confirmed any determination that was not immediately clear with lawyers working with the Linux Foundation. Criteria used to select files for SPDX license identifier tagging was: - Files considered eligible had to be source code files. - Make and config files were included as candidates if they contained >5 lines of source - File already had some variant of a license header in it (even if <5 lines). All documentation files were explicitly excluded. The following heuristics were used to determine which SPDX license identifiers to apply. - when both scanners couldn't find any license traces, file was considered to have no license information in it, and the top level COPYING file license applied. For non */uapi/* files that summary was: SPDX license identifier # files ---------------------------------------------------|------- GPL-2.0 11139 and resulted in the first patch in this series. If that file was a */uapi/* path one, it was "GPL-2.0 WITH Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was: SPDX license identifier # files ---------------------------------------------------|------- GPL-2.0 WITH Linux-syscall-note 930 and resulted in the second patch in this series. - if a file had some form of licensing information in it, and was one of the */uapi/* ones, it was denoted with the Linux-syscall-note if any GPL family license was found in the file or had no licensing in it (per prior point). Results summary: SPDX license identifier # files ---------------------------------------------------|------ GPL-2.0 WITH Linux-syscall-note 270 GPL-2.0+ WITH Linux-syscall-note 169 ((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause) 21 ((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause) 17 LGPL-2.1+ WITH Linux-syscall-note 15 GPL-1.0+ WITH Linux-syscall-note 14 ((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause) 5 LGPL-2.0+ WITH Linux-syscall-note 4 LGPL-2.1 WITH Linux-syscall-note 3 ((GPL-2.0 WITH Linux-syscall-note) OR MIT) 3 ((GPL-2.0 WITH Linux-syscall-note) AND MIT) 1 and that resulted in the third patch in this series. - when the two scanners agreed on the detected license(s), that became the concluded license(s). - when there was disagreement between the two scanners (one detected a license but the other didn't, or they both detected different licenses) a manual inspection of the file occurred. - In most cases a manual inspection of the information in the file resulted in a clear resolution of the license that should apply (and which scanner probably needed to revisit its heuristics). - When it was not immediately clear, the license identifier was confirmed with lawyers working with the Linux Foundation. - If there was any question as to the appropriate license identifier, the file was flagged for further research and to be revisited later in time. In total, over 70 hours of logged manual review was done on the spreadsheet to determine the SPDX license identifiers to apply to the source files by Kate, Philippe, Thomas and, in some cases, confirmation by lawyers working with the Linux Foundation. Kate also obtained a third independent scan of the 4.13 code base from FOSSology, and compared selected files where the other two scanners disagreed against that SPDX file, to see if there was new insights. The Windriver scanner is based on an older version of FOSSology in part, so they are related. Thomas did random spot checks in about 500 files from the spreadsheets for the uapi headers and agreed with SPDX license identifier in the files he inspected. For the non-uapi files Thomas did random spot checks in about 15000 files. In initial set of patches against 4.14-rc6, 3 files were found to have copy/paste license identifier errors, and have been fixed to reflect the correct identifier. Additionally Philippe spent 10 hours this week doing a detailed manual inspection and review of the 12,461 patched files from the initial patch version early this week with: - a full scancode scan run, collecting the matched texts, detected license ids and scores - reviewing anything where there was a license detected (about 500+ files) to ensure that the applied SPDX license was correct - reviewing anything where there was no detection but the patch license was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied SPDX license was correct This produced a worksheet with 20 files needing minor correction. This worksheet was then exported into 3 different .csv files for the different types of files to be modified. These .csv files were then reviewed by Greg. Thomas wrote a script to parse the csv files and add the proper SPDX tag to the file, in the format that the file expected. This script was further refined by Greg based on the output to detect more types of files automatically and to distinguish between header and source .c files (which need different comment types.) Finally Greg ran the script using the .csv files to generate the patches. powerpc/mm: Don't send IPI to all cpus on THP updates Now that we made sure that lockless walk of linux page table is mostly limitted to current task(current->mm->pgdir) we can update the THP update sequence to only send IPI to CPUs on which this task has run. This helps in reducing the IPI overload on systems with large number of CPUs. WRT kvm even though kvm is walking page table with vpc->arch.pgdir, it is done only on secondary CPUs and in that case we have primary CPU added to task's mm cpumask. Sending an IPI to primary will force the secondary to do a vm exit and hence this mm cpumask usage is safe here. WRT CAPI, we still end up walking linux page table with capi context MM. For now the pte lookup serialization sends an IPI to all CPUs in CPI is in use. We can further improve this by adding the CAPI interrupt handling CPU to task mm cpumask. powerpc/hugetlb: fix page rights verification in gup_hugepte() gup_hugepte() checks if pages are present and readable, and when 'write' is set, also checks if the pages are writable. Initially this was done by checking if _PAGE_PRESENT and _PAGE_READ were set. In addition, _PAGE_WRITE was verified for write accesses. The problem is that we have to handle the three following cases: 1/ The target defines __PAGE_READ and __PAGE_WRITE 2/ The target defines __PAGE_RW 3/ The target defines __PAGE_RO In case 1/, this is obvious In case 2/, __PAGE_READ is defined as 0 and __PAGE_WRITE as __PAGE_RW so it works as well. But in case 3, __PAGE_RW is defined as 0, which means __PAGE_WRITE is 0 and then the test returns true (page writable) in all cases. A first correction was attempted in commit 6b8cb66a6a7cc ("powerpc: Fix usage of _PAGE_RO in hugepage"), but that fix is wrong: instead of checking that the page is writable when write is requested, it checks that the page is NOT writable when write is NOT requested. This patch adds a new pte_read() helper to check whether a page is readable or not. This avoids handling all possible cases in gup_hugepte(). Then gup_hugepte() is modified to use pte_present(), pte_read() and pte_write() instead of the raw flags. powerpc/mm/book3s64: Make KERN_IO_START a variable Currently KERN_IO_START is defined as: #define KERN_IO_START (KERN_VIRT_START + (KERN_VIRT_SIZE >> 1)) Although it looks like a constant, both the components are actually variables, to allow us to have a different value between Radix and Hash with a single kernel. However that still requires both Radix and Hash to place the kernel IO region at the same location relative to the start and end of the kernel virtual region (namely 1/2 way through it), and we'd like to change that. So split KERN_IO_START out into its own variable, and initialise it for Radix and Hash. In the medium term we should be able to reconsolidate this, by doing a more involved rearrangement of the location of the regions. powerpc/mm: Fix pmd/pte_devmap() on non-leaf entries The Radix MMU translation tree as defined in ISA v3.0 contains two different types of entry, directories and leaves. Leaves are identified by _PAGE_PTE being set. The formats of the two entries are different, with the directory entries containing no spare bits for use by software. In particular the bit we use for _PAGE_DEVMAP is not reserved for software, and is part of the NLB (Next Level Base) field, essentially the address of the next level in the tree. Note that the Linux pte_t is not == _PAGE_PTE. A huge page pmd entry (or devmap!) is also a leaf and so has _PAGE_PTE set, even though we use a pmd_t for it in Linux. The fix is to ensure that the pmd/pte_devmap() confirm they are looking at a leaf entry (_PAGE_PTE) as well as checking _PAGE_DEVMAP. powerpc/mm: Fix swapper_pg_dir size on 64-bit hash w/64K pages Recently in commit f6eedbba7a26 ("powerpc/mm/hash: Increase VA range to 128TB"), we increased H_PGD_INDEX_SIZE to 15 when we're building with 64K pages. This makes it larger than RADIX_PGD_INDEX_SIZE (13), which means the logic to calculate MAX_PGD_INDEX_SIZE in book3s/64/pgtable.h is wrong. The end result is that the PGD (Page Global Directory, ie top level page table) of the kernel (aka. swapper_pg_dir), is too small. This generally doesn't lead to a crash, as we don't use the full range in normal operation. powerpc/mm: Lower the max real address to 53 bits Max value supported by hardware is 51 bits address. Radix page table define a slot of 57 bits for future expansion. We restrict the value supported in linux kernel 53 bits, so that we can use the bits between 57-53 for storing hash linux page table bits. This is done in the next patch. This will free up the software page table bits to be used for features that are needed for both hash and radix. The current hash linux page table format doesn't have any free software bits. Moving hash linux page table specific bits to top of RPN field free up the software bits for other purpose. power/mm: update pte_write and pte_wrprotect to handle savedwrite We use pte_write() to check whethwer the pte entry is writable. This is mostly used to later mark the pte read only if it is writable. The other use of pte_write() is to check whether the pte_entry is writable so that hardware page table entry can be marked accordingly. This is used in kvm where we look at qemu page table entry and update hardware hash page table for the guest with correct write enable bit. With the above, for the first usage we should also check the savedwrite bit so that we can correctly clear the savedwite bit. For the later, we add a new variant __pte_write(). With this we can revert write_protect_page part of 595cd8f256d2 ("mm/ksm: handle protnone saved writes when making page write protect"). But I left it as it is as an example code for savedwrite check. On fork we need to make sure we remove the saved write bit. Instead of adding the protnone check in the caller update ptep_set_wrprotect variants to clear savedwrite bit. powerpc: port 64 bits pgtable_cache to 32 bits Today powerpc64 uses a set of pgtable_caches while powerpc32 uses standard pages when using 4k pages and a single pgtable_cache if using other size pages. In preparation of implementing huge pages on the 8xx, this patch replaces the specific powerpc32 handling by the 64 bits approach. powerpc/mm: Introduce _PAGE_LARGE software pte bits This patch adds a new software defined pte bit. We use the reserved fields of ISA 3.0 pte definition since we will only be using this on DD1 code paths. We can possibly look at removing this code later. The software bit will be used to differentiate between 64K/4K and 2M ptes. This helps in finding the page size mapping by a pte so that we can do efficient tlb flush. We don't support 1G hugetlb pages yet. So we add a DEBUG WARN_ON to catch wrong usage. powerpc: Initialise pci_io_base as early as possible Commit d6a9996e84ac ("powerpc/mm: vmalloc abstraction in preparation for radix") turned kernel memory and IO addresses from #defined constants to variables initialised at runtime. On PA6T (pasemi) systems the setup_arch() machine call initialises the onboard PCI-e root-ports, and uses pci_io_base to do this, which is now before its value has been set, resulting in a panic early in boot before console IO is initialised. Move the pci_io_base initialisation to the same place as vmalloc ranges are set (hash__early_init_mmu()/radix__early_init_mmu()) - this is the earliest possible place we can initialise it. arch: fix has_transparent_hugepage() I've just discovered that the useful-sounding has_transparent_hugepage() is actually an architecture-dependent minefield: on some arches it only builds if CONFIG_TRANSPARENT_HUGEPAGE=y, on others it's also there when not, but on some of those (arm and arm64) it then gives the wrong answer; and on mips alone it's marked __init, which would crash if called later (but so far it has not been called later). Straighten this out: make it available to all configs, with a sensible default in asm-generic/pgtable.h, removing its definitions from those arches (arc, arm, arm64, sparc, tile) which are served by the default, adding #define has_transparent_hugepage has_transparent_hugepage to those (mips, powerpc, s390, x86) which need to override the default at runtime, and removing the __init from mips (but maybe that kind of code should be avoided after init: set a static variable the first time it's called). powerpc/mm: Make page table size a variable Radix and hash MMU models support different page table sizes. Make the #defines a variable so that existing code can work with variable sizes. Slice related code is only used by hash, so use hash constants there. We will replicate some of the boundary conditions with resepct to TASK_SIZE using radix values too. Right now we do boundary condition check using hash constants. Swapper pgdir size is initialized in asm code. We select the max pgd size to keep it simple. For now we select hash pgdir. When adding radix we will switch that to radix pgdir which is 64K. BUILD_BUG_ON check which is removed is already done in hugepage_init() using MAYBE_BUILD_BUG_ON(). powerpc/mm: Remove RPN_SHIFT and RPN_SIZE PTE_RPN_SHIFT is actually page size dependent. Even though PowerISA 3.0 expects only the lower 12 bits to be zero, we will always find the pages to be PAGE_SHIFT aligned. In case of hash config, this also allows us to use the additional 3 bits to track pte specific information. We need to make sure we use these bits only for hash specific pte flags. For both 4K and 64K config, pte now can hold 57 bits address. Inorder to keep things simpler, drop PTE_RPN_SHIFT and PTE_RPN_SIZE and specify the 57 bit detail explicitly. powerpc/mm: Replace _PAGE_USER with _PAGE_PRIVILEGED _PAGE_PRIVILEGED means the page can be accessed only by the kernel. This is done to keep pte bits similar to PowerISA 3.0 Radix PTE format. User pages are now marked by clearing _PAGE_PRIVILEGED bit. Previously we allowed the kernel to have a privileged page in the lower address range (USER_REGION). With this patch such access is denied. We also prevent a kernel access to a non-privileged page in higher address range (ie, REGION_ID != 0). Both the above access scenarios should never happen. powerpc/mm: Switch book3s 64 with 64K page size to 4 level page table This is needed so that we can support both hash and radix page table using single kernel. Radix kernel uses a 4 level table. We now use physical address in upper page table tree levels. Even though they are aligned to their size, for the masked bits we use the bit positions as per PowerISA 3.0. powerpc/mm/book3s-64: Free up 7 high-order bits in the Linux PTE This frees up bits 57-63 in the Linux PTE on 64-bit Book 3S machines. In the 4k page case, this is done just by reducing the size of the RPN field to 39 bits, giving 51-bit real addresses. In the 64k page case, we had 10 unused bits in the middle of the PTE, so this moves the RPN field down 10 bits to make use of those unused bits. This means the RPN field is now 3 bits larger at 37 bits, giving 53-bit real addresses in the normal case, or 49-bit real addresses for the special 4k PFN case. We are doing this in order to be able to move some other PTE bits into the positions where PowerISA V3.0 processors will expect to find them in radix-tree mode. powerpc/mm: Fix Multi hit ERAT cause by recent THP update With ppc64 we use the deposited pgtable_t to store the hash pte slot information. We should not withdraw the deposited pgtable_t without marking the pmd none. This ensure that low level hash fault handling will skip this huge pte and we will handle them at upper levels. Recent change to pmd splitting changed the above in order to handle the race between pmd split and exit_mmap. The race is explained below. Consider following race: CPU0 CPU1 shrink_page_list() add_to_swap() split_huge_page_to_list() __split_huge_pmd_locked() pmdp_huge_clear_flush_notify() // pmd_none() == true exit_mmap() unmap_vmas() zap_pmd_range() // no action on pmd since pmd_none() == true pmd_populate() As result the THP will not be freed. The leak is detected by check_mm(): BUG: Bad rss-counter state mm:ffff880058d2e580 idx:1 val:512 The above required us to not mark pmd none during a pmd split. The fix for ppc is to clear the huge pte of _PAGE_USER, so that low level fault handling code skip this pte. At higher level we do take ptl lock. That should serialze us against the pmd split. Once the lock is acquired we do check the pmd again using pmd_same. That should always return false for us and hence we should retry the access. We do the pmd_same check in all case after taking plt with THP (do_huge_pmd_wp_page, do_huge_pmd_numa_page and huge_pmd_set_accessed) Also make sure we wait for irq disable section in other cpus to finish before flipping a huge pte entry with a regular pmd entry. Code paths like find_linux_pte_or_hugepte depend on irq disable to get a stable pte_t pointer. A parallel thp split need to make sure we don't convert a pmd pte to a regular pmd entry without waiting for the irq disable section to finish. powerpc/mm: fix _PAGE_SWP_SOFT_DIRTY breaking swapoff Swapoff after swapping hangs on the G5, when CONFIG_CHECKPOINT_RESTORE=y but CONFIG_MEM_SOFT_DIRTY is not set. That's because the non-zero _PAGE_SWP_SOFT_DIRTY bit, added by CONFIG_HAVE_ARCH_SOFT_DIRTY=y, is not discounted when CONFIG_MEM_SOFT_DIRTY is not set: so swap ptes cannot be recognized. (I suspect that the peculiar dependence of HAVE_ARCH_SOFT_DIRTY on CHECKPOINT_RESTORE in arch/powerpc/Kconfig comes from an incomplete attempt to solve this problem.) It's true that the relationship between CONFIG_HAVE_ARCH_SOFT_DIRTY and and CONFIG_MEM_SOFT_DIRTY is too confusing, and it's true that swapoff should be made more robust; but nevertheless, fix up the powerpc ifdefs as x86_64 and s390 (which met the same problem) have them, defining the bits as 0 if CONFIG_MEM_SOFT_DIRTY is not set. powerpc/mm: Fix _PAGE_PTE breaking swapoff Core kernel expects swp_entry_t to consist of only swap type and swap offset. We should not leak pte bits into swp_entry_t. This breaks swapoff which use the swap type and offset to build a swp_entry_t and later compare that to the swp_entry_t obtained from linux page table pte. Leaking pte bits into swp_entry_t breaks that comparison and results in us looping in try_to_unuse. The stack trace can be anywhere below try_to_unuse() in mm/swapfile.c, since swapoff is circling around and around that function, reading from each used swap block into a page, then trying to find where that page belongs, looking at every non-file pte of every mm that ever swapped. powerpc/mm: Add page soft dirty tracking User space checkpoint and restart tool (CRIU) needs the page's change to be soft tracked. This allows to do a pre checkpoint and then dump only touched pages. This is done by using a newly assigned PTE bit (_PAGE_SOFT_DIRTY) when the page is backed in memory, and a new _PAGE_SWP_SOFT_DIRTY bit when the page is swapped out. To introduce a new PTE _PAGE_SOFT_DIRTY bit value common to hash 4k and hash 64k pte, the bits already defined in hash-*4k.h should be shifted left by one. The _PAGE_SWP_SOFT_DIRTY bit is dynamically put after the swap type in the swap pte. A check is added to ensure that the bit is not overwritten by _PAGE_HPTEFLAGS. powerpc/mm: Add a _PAGE_PTE bit For a pte entry we will have _PAGE_PTE set. Our pte page address have a minimum alignment requirement of HUGEPD_SHIFT_MASK + 1. We use the lower 7 bits to indicate hugepd. ie. For pmd and pgd we can find: 1) _PAGE_PTE set pte -> indicate PTE 2) bits [2..6] non zero -> indicate hugepd. They also encode the size. We skip bit 1 (_PAGE_PRESENT). 3) othewise pointer to next table.
