powerpc: Support execute-only on all powerpc Introduce PAGE_EXECONLY_X macro which provides exec-only rights. The _X may be seen as redundant with the EXECONLY but it helps keep consistency, all macros having the EXEC right have _X. And put it next to PAGE_NONE as PAGE_EXECONLY_X is somehow PAGE_NONE + EXEC just like all other SOMETHING_X are just SOMETHING + EXEC. On book3s/64 PAGE_EXECONLY becomes PAGE_READONLY_X. On book3s/64, as PAGE_EXECONLY is only valid for Radix add VM_READ flag in vm_get_page_prot() for non-Radix. And update access_error() so that a non exec fault on a VM_EXEC only mapping is always invalid, even when the underlying layer don't always generate a fault for that. For 8xx, set PAGE_EXECONLY_X as _PAGE_NA | _PAGE_EXEC. For others, only set it as just _PAGE_EXEC With that change, 8xx, e500 and 44x fully honor execute-only protection. On 40x that is a partial implementation of execute-only. The implementation won't be complete because once a TLB has been loaded via the Instruction TLB miss handler, it will be possible to read the page. But at least it can't be read unless it is executed first. On 603 MMU, TLB missed are handled by SW and there are separate DTLB and ITLB. Execute-only is therefore now supported by not loading DTLB when read access is not permitted. On hash (604) MMU it is more tricky because hash table is common to load/store and execute. Nevertheless it is still possible to check whether _PAGE_READ is set before loading hash table for a load/store access. At least it can't be read unless it is executed first. powerpc/32s: Introduce _PAGE_READ and remove _PAGE_USER On 603 MMU, TLB missed are handled by SW and there are separated DTLB and ITLB. It is therefore possible to implement execute-only protection by not loading DTLB when read access is not permitted. To do that, _PAGE_READ flag is needed but there is no bit available for it in PTE. On the other hand the only real use of _PAGE_USER is to implement PAGE_NONE by clearing _PAGE_USER. As _PAGE_NONE can also be implemented by clearing _PAGE_READ, remove _PAGE_USER and add _PAGE_READ. Then use the virtual address to know whether user rights or kernel rights are to be used. With that change, 603 MMU now honors execute-only protection. For hash (604) MMU it is more tricky because hash table is common to load/store and execute. Nevertheless it is still possible to check whether _PAGE_READ is set before loading hash table for a load/store access. At least it can't be read unless it is executed first. For the time being _PAGE_WRITE is equivalent to _PAGE_RW but that will change when _PAGE_READ gets added in following patches. mm: Rename arch pte_mkwrite()'s to pte_mkwrite_novma() The x86 Shadow stack feature includes a new type of memory called shadow stack. This shadow stack memory has some unusual properties, which requires some core mm changes to function properly. One of these unusual properties is that shadow stack memory is writable, but only in limited ways. These limits are applied via a specific PTE bit combination. Nevertheless, the memory is writable, and core mm code will need to apply the writable permissions in the typical paths that call pte_mkwrite(). The goal is to make pte_mkwrite() take a VMA, so that the x86 implementation of it can know whether to create regular writable or shadow stack mappings. But there are a couple of challenges to this. Modifying the signatures of each arch pte_mkwrite() implementation would be error prone because some are generated with macros and would need to be re-implemented. Also, some pte_mkwrite() callers operate on kernel memory without a VMA. So this can be done in a three step process. First pte_mkwrite() can be renamed to pte_mkwrite_novma() in each arch, with a generic pte_mkwrite() added that just calls pte_mkwrite_novma(). Next callers without a VMA can be moved to pte_mkwrite_novma(). And lastly, pte_mkwrite() and all callers can be changed to take/pass a VMA. Start the process by renaming pte_mkwrite() to pte_mkwrite_novma() and adding the pte_mkwrite() wrapper in linux/pgtable.h. Apply the same pattern for pmd_mkwrite(). Since not all archs have a pmd_mkwrite_novma(), create a new arch config HAS_HUGE_PAGE that can be used to tell if pmd_mkwrite() should be defined. Otherwise in the !HAS_HUGE_PAGE cases the compiler would not be able to find pmd_mkwrite_novma(). No functional change. The same problem will be hit on all 32-bit architectures that have a physical address space larger than 4GB and happen to not enable sparsemem and include asm/sparsemem.h from asm/pgtable.h. After the initial discussion, I suggested just always defining MAX_POSSIBLE_PHYSMEM_BITS whenever CONFIG_PHYS_ADDR_T_64BIT is set, or provoking a build error otherwise. This addresses all configurations that can currently have this runtime bug, but leaves all other configurations unchanged. I looked up the possible number of bits in source code and datasheets, here is what I found: - on ARC, CONFIG_ARC_HAS_PAE40 controls whether 32 or 40 bits are used - on ARM, CONFIG_LPAE enables 40 bit addressing, without it we never support more than 32 bits, even though supersections in theory allow up to 40 bits as well. - on MIPS, some MIPS32r1 or later chips support 36 bits, and MIPS32r5 XPA supports up to 60 bits in theory, but 40 bits are more than anyone will ever ship - On PowerPC, there are three different implementations of 36 bit addressing, but 32-bit is used without CONFIG_PTE_64BIT - On RISC-V, the normal page table format can support 34 bit addressing. There is no highmem support on RISC-V, so anything above 2GB is unused, but it might be useful to eventually support CONFIG_ZRAM for high pages. Use a dedicated segment for modules. There is not much space above kernel, and we don't waste vmalloc space to do alignment. Therefore, we take the segment before PAGE_OFFSET for modules. mm: consolidate pte_index() and pte_offset_*() definitions All architectures define pte_index() as (address >> PAGE_SHIFT) & (PTRS_PER_PTE - 1) and all architectures define pte_offset_kernel() as an entry in the array of PTEs indexed by the pte_index(). For the most architectures the pte_offset_kernel() implementation relies on the availability of pmd_page_vaddr() that converts a PMD entry value to the virtual address of the page containing PTEs array. Let's move x86 definitions of the PTE accessors to the generic place in <linux/pgtable.h> and then simply drop the respective definitions from the other architectures. The architectures that didn't provide pmd_page_vaddr() are updated to have that defined. The generic implementation of pte_offset_kernel() can be overridden by an architecture and alpha makes use of this because it has special ordering requirements for its version of pte_offset_kernel(). powerpc/mm: Don't kmap_atomic() in pte_offset_map() on PPC32 On PPC32, pte_offset_map() does a kmap_atomic() in order to support page tables allocated in high memory, just like ARM and x86/32. But since at least 2008 and commit 8054a3428fbe ("powerpc: Remove dead CONFIG_HIGHPTE"), page tables are never allocated in high memory. When the page is in low mem, kmap_atomic() just returns the page address but still disable preemption and pagefault. And it is not an inlined function, so we suffer function call for no reason. Make pte_offset_map() the same as pte_offset_kernel() and make pte_unmap() void, in the same way as PPC64 which doesn't have HIGHMEM. powerpc: use the generic dma coherent remap allocator This switches to using common code for the DMA allocations, including potential use of the CMA allocator if configured. Switching to the generic code enables DMA allocations from atomic context, which is required by the DMA API documentation, and also adds various other minor features drivers start relying upon. It also makes sure we have on tested code base for all architectures that require uncached pte bits for coherent DMA allocations. Another advantage is that consistent memory allocations now share the general vmalloc pool instead of needing an explicit careout from it. powerpc/mm/32s: Use BATs for STRICT_KERNEL_RWX Today, STRICT_KERNEL_RWX is based on the use of regular pages to map kernel pages. On Book3s 32, it has three consequences: - Using pages instead of BAT for mapping kernel linear memory severely impacts performance. - Exec protection is not effective because no-execute cannot be set at page level (except on 603 which doesn't have hash tables) - Write protection is not effective because PP bits do not provide RO mode for kernel-only pages (except on 603 which handles it in software via PAGE_DIRTY) On the 603+, we have: - Independent IBAT and DBAT allowing limitation of exec parts. - NX bit can be set in segment registers to forbit execution on memory mapped by pages. - RO mode on DBATs even for kernel-only blocks. On the 601, there is nothing much we can do other than warn the user about it, because: - BATs are common to instructions and data. - BAT do not provide RO mode for kernel-only blocks. - segment registers don't have the NX bit. In order to use IBAT for exec protection, this patch: - Aligns _etext to BAT block sizes (128kb) - Set NX bit in kernel segment register (Except on vmalloc area when CONFIG_MODULES is selected) - Maps kernel text with IBATs. powerpc/mm: add pte helpers to query and change pte flags In order to avoid using generic _PAGE_XXX flags in powerpc core functions, define helpers for all needed flags: - pte_mkuser() and pte_mkprivileged() to set/unset and/or unset/set _PAGE_USER and/or _PAGE_PRIVILEGED - pte_hashpte() to check if _PAGE_HASHPTE is set. - pte_ci() check if cache is inhibited (already existing on book3s/64) - pte_exprotect() to protect against execution - pte_exec() and pte_mkexec() to query and set page execution - pte_mkpte() to set _PAGE_PTE flag. - pte_hw_valid() to check _PAGE_PRESENT since pte_present does something different on book3s/64. On book3s/32 there is no exec protection, so pte_mkexec() and pte_exprotect() are nops and pte_exec() returns always true. powerpc/mm/radix: Change pte relax sequence to handle nest MMU hang When relaxing access (read -> read_write update), pte needs to be marked invalid to handle a nest MMU bug. We also need to do a tlb flush after the pte is marked invalid before updating the pte with new access bits. We also move tlb flush to platform specific __ptep_set_access_flags. This will help us to gerid of unnecessary tlb flush on BOOK3S 64 later. We don't do that in this patch. This also helps in avoiding multiple tlbies with coprocessor attached. powerpc/mm: Fix crashes with 16G huge pages To support memory keys, we moved the hash pte slot information to the second half of the page table. This was ok with PTE entries at level 4 (PTE page) and level 3 (PMD). We already allocate larger page table pages at those levels to accomodate extra details. For level 4 we already have the extra space which was used to track 4k hash page table entry details and at level 3 the extra space was allocated to track the THP details. With hugetlbfs PTE, we used this extra space at the PMD level to store the slot details. But we also support hugetlbfs PTE at PUD level for 16GB pages and PUD level page didn't allocate extra space. This resulted in memory corruption. Fix this by allocating extra space at PUD level when HUGETLB is enabled. powerpc/mm: Add proper pte access check helper for other platforms pte_access_premitted get called in get_user_pages_fast path. If we have marked the pte PROT_NONE, we should not allow a read access on the address. With the current implementation we are not checking the READ and only check for WRITE. This is needed on archs like ppc64 that implement PROT_NONE using _PAGE_USER access instead of _PAGE_PRESENT. Also add pte_user check just to make sure we are not accessing kernel mapping. Even though there is code duplication, keeping the low level pte accessors different for different platforms helps in code readability. License cleanup: add SPDX GPL-2.0 license identifier to files with no license Many source files in the tree are missing licensing information, which makes it harder for compliance tools to determine the correct license. By default all files without license information are under the default license of the kernel, which is GPL version 2. Update the files which contain no license information with the 'GPL-2.0' SPDX license identifier. The SPDX identifier is a legally binding shorthand, which can be used instead of the full boiler plate text. This patch is based on work done by Thomas Gleixner and Kate Stewart and Philippe Ombredanne. How this work was done: Patches were generated and checked against linux-4.14-rc6 for a subset of the use cases: - file had no licensing information it it. - file was a */uapi/* one with no licensing information in it, - file was a */uapi/* one with existing licensing information, Further patches will be generated in subsequent months to fix up cases where non-standard license headers were used, and references to license had to be inferred by heuristics based on keywords. The analysis to determine which SPDX License Identifier to be applied to a file was done in a spreadsheet of side by side results from of the output of two independent scanners (ScanCode & Windriver) producing SPDX tag:value files created by Philippe Ombredanne. Philippe prepared the base worksheet, and did an initial spot review of a few 1000 files. The 4.13 kernel was the starting point of the analysis with 60,537 files assessed. Kate Stewart did a file by file comparison of the scanner results in the spreadsheet to determine which SPDX license identifier(s) to be applied to the file. She confirmed any determination that was not immediately clear with lawyers working with the Linux Foundation. Criteria used to select files for SPDX license identifier tagging was: - Files considered eligible had to be source code files. - Make and config files were included as candidates if they contained >5 lines of source - File already had some variant of a license header in it (even if <5 lines). All documentation files were explicitly excluded. The following heuristics were used to determine which SPDX license identifiers to apply. - when both scanners couldn't find any license traces, file was considered to have no license information in it, and the top level COPYING file license applied. For non */uapi/* files that summary was: SPDX license identifier # files ---------------------------------------------------|------- GPL-2.0 11139 and resulted in the first patch in this series. If that file was a */uapi/* path one, it was "GPL-2.0 WITH Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was: SPDX license identifier # files ---------------------------------------------------|------- GPL-2.0 WITH Linux-syscall-note 930 and resulted in the second patch in this series. - if a file had some form of licensing information in it, and was one of the */uapi/* ones, it was denoted with the Linux-syscall-note if any GPL family license was found in the file or had no licensing in it (per prior point). Results summary: SPDX license identifier # files ---------------------------------------------------|------ GPL-2.0 WITH Linux-syscall-note 270 GPL-2.0+ WITH Linux-syscall-note 169 ((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause) 21 ((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause) 17 LGPL-2.1+ WITH Linux-syscall-note 15 GPL-1.0+ WITH Linux-syscall-note 14 ((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause) 5 LGPL-2.0+ WITH Linux-syscall-note 4 LGPL-2.1 WITH Linux-syscall-note 3 ((GPL-2.0 WITH Linux-syscall-note) OR MIT) 3 ((GPL-2.0 WITH Linux-syscall-note) AND MIT) 1 and that resulted in the third patch in this series. - when the two scanners agreed on the detected license(s), that became the concluded license(s). - when there was disagreement between the two scanners (one detected a license but the other didn't, or they both detected different licenses) a manual inspection of the file occurred. - In most cases a manual inspection of the information in the file resulted in a clear resolution of the license that should apply (and which scanner probably needed to revisit its heuristics). - When it was not immediately clear, the license identifier was confirmed with lawyers working with the Linux Foundation. - If there was any question as to the appropriate license identifier, the file was flagged for further research and to be revisited later in time. In total, over 70 hours of logged manual review was done on the spreadsheet to determine the SPDX license identifiers to apply to the source files by Kate, Philippe, Thomas and, in some cases, confirmation by lawyers working with the Linux Foundation. Kate also obtained a third independent scan of the 4.13 code base from FOSSology, and compared selected files where the other two scanners disagreed against that SPDX file, to see if there was new insights. The Windriver scanner is based on an older version of FOSSology in part, so they are related. Thomas did random spot checks in about 500 files from the spreadsheets for the uapi headers and agreed with SPDX license identifier in the files he inspected. For the non-uapi files Thomas did random spot checks in about 15000 files. In initial set of patches against 4.14-rc6, 3 files were found to have copy/paste license identifier errors, and have been fixed to reflect the correct identifier. Additionally Philippe spent 10 hours this week doing a detailed manual inspection and review of the 12,461 patched files from the initial patch version early this week with: - a full scancode scan run, collecting the matched texts, detected license ids and scores - reviewing anything where there was a license detected (about 500+ files) to ensure that the applied SPDX license was correct - reviewing anything where there was no detection but the patch license was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied SPDX license was correct This produced a worksheet with 20 files needing minor correction. This worksheet was then exported into 3 different .csv files for the different types of files to be modified. These .csv files were then reviewed by Greg. Thomas wrote a script to parse the csv files and add the proper SPDX tag to the file, in the format that the file expected. This script was further refined by Greg based on the output to detect more types of files automatically and to distinguish between header and source .c files (which need different comment types.) Finally Greg ran the script using the .csv files to generate the patches. powerpc/hugetlb: fix page rights verification in gup_hugepte() gup_hugepte() checks if pages are present and readable, and when 'write' is set, also checks if the pages are writable. Initially this was done by checking if _PAGE_PRESENT and _PAGE_READ were set. In addition, _PAGE_WRITE was verified for write accesses. The problem is that we have to handle the three following cases: 1/ The target defines __PAGE_READ and __PAGE_WRITE 2/ The target defines __PAGE_RW 3/ The target defines __PAGE_RO In case 1/, this is obvious In case 2/, __PAGE_READ is defined as 0 and __PAGE_WRITE as __PAGE_RW so it works as well. But in case 3, __PAGE_RW is defined as 0, which means __PAGE_WRITE is 0 and then the test returns true (page writable) in all cases. A first correction was attempted in commit 6b8cb66a6a7cc ("powerpc: Fix usage of _PAGE_RO in hugepage"), but that fix is wrong: instead of checking that the page is writable when write is requested, it checks that the page is NOT writable when write is NOT requested. This patch adds a new pte_read() helper to check whether a page is readable or not. This avoids handling all possible cases in gup_hugepte(). Then gup_hugepte() is modified to use pte_present(), pte_read() and pte_write() instead of the raw flags. powerpc: port 64 bits pgtable_cache to 32 bits Today powerpc64 uses a set of pgtable_caches while powerpc32 uses standard pages when using 4k pages and a single pgtable_cache if using other size pages. In preparation of implementing huge pages on the 8xx, this patch replaces the specific powerpc32 handling by the 64 bits approach.
