global: Finish CONFIG -> CFG migration

At this point, the remaining places where we have a symbol that is
defined as CONFIG_... are in fairly odd locations. While as much dead
code has been removed as possible, some of these locations are simply
less obvious at first. In other cases, this code is used, but was
defined in such a way as to have been missed by earlier checks. Perform
a rename of all such remaining symbols to be CFG_... rather than
CONFIG_...

fsl/usb: Workaround for USB erratum-A005275

Workaround makes FS as default mode on all affected socs.

Add support to check erratum-A005275 validity for an soc. This info is
required to determine whether a given soc is affected by this erratum.
Add quirk for this erratum "has_fsl_erratum_a005275" . This quirk is used
to enable workaround for the errata

Force FS mode as default by:
- making EPS as FS
- setting PFSC bit to disable HS chirping

This workaround can be disabled by mentioning "no_erratum_a005275" in
hwconfig string

SPDX: Convert all of our single license tags to Linux Kernel style

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.

In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.

This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

drivers: usb: xhci-fsl: Implement Erratum A-010151 for FSL USB3 controller

Currently the controller by default enables the Receive Detect feature in P3
mode in USB 3.0 PHY. However, USB 3.0 PHY does not reliably support receive
detection in P3 mode.
Enabling the USB3 controller to configure USB in P2 mode whenever the Receive
Detect feature is required.

Add a delay of 1 microsecond before issuing soft reset to the
controller to let ongoing ULPI transaction complete.
This prevents corruption of ULPI Function Control Register which
eventually prevents phy clock from entering to low power mode

Adds support for clock sourcing from sysclk(100MHz) for usb
on T104xRDB and T1040QDS. This requires changing reference divisor
and multiplication factor to derive usb clock from sysclk.

powerpc/usb: Workaround for erratum-A006261

USB spec says that the minimum disconnect threshold should be
over 525 mV. However, internal USB PHY threshold value is below
this specified value. Due to this some devices disconnect at
run-time. Hence, phy settings are tweaked to increased disconnect
threshold to be above 525mV by using this workaround.
