SPDX: Convert all of our single license tags to Linux Kernel style

When U-Boot started using SPDX tags we were among the early adopters and
there weren't a lot of other examples to borrow from. So we picked the
area of the file that usually had a full license text and replaced it
with an appropriate SPDX-License-Identifier: entry. Since then, the
Linux Kernel has adopted SPDX tags and they place it as the very first
line in a file (except where shebangs are used, then it's second line)
and with slightly different comment styles than us.

In part due to community overlap, in part due to better tag visibility
and in part for other minor reasons, switch over to that style.

This commit changes all instances where we have a single declared
license in the tag as both the before and after are identical in tag
contents. There's also a few places where I found we did not have a tag
and have introduced one.

# 6862b50f 17-May-2017 Simon Glass <sjg@chromium.org>

x86: Make u-boot-x86.h a private header

Rather than including this arch-specific header file in common.h, include
it from within x86's u-boot.h header.

Also drop the comment about something to be fixed. It is not clear what
needs fixing.

x86: Remove legacy board init code

Since we use CONFIG_SYS_GENERIC_BOARD on x86, we don't need this anymore.

This enables generic board support so that x86 boards can define
CONFIG_SYS_GENERIC_BOARD.

x86: Add some missing includes

I suspect these includes were usually available because something else
included them earlier or because they were brought in transitively.

net: punt bd->bi_ip_addr

This field gets read in one place (by "bdinfo"), and we can replace
that with getenv("ipaddr"). After all, the bi_ip_addr field is kept
up-to-date implicitly with the value of the ipaddr env var.

image: push default arch values to arch headers

This pushes the ugly duplicated arch ifdef lists we maintain in various
