use 0 instead of NULL in headers where NULL is not defined

some macros in sys/mtio.h and syslog.h used NULL without defining it

add cleaned-up sys/mtio.h

this is mostly junk, but a few programs with tape-drive support
unconditionally include it, and it might be useful.
