man pages: add missing commas between subordinate and main clauses

jmc@ dislikes a comma before "then" in a conditional, so leave those
untouched.

Cleanup documentation for SCSI device driver ioctl()s.

- drivers ported from FreeBSD did mention rc.conf instead of hostname.if as
the place to put mediaopt
- some .Tn UNIX -> .Ox replacements
- etc, etc...

Ok, so there could be a cost/benefit debate with this commit, but since I have
the patch we might as well commit it...

More man page fixes. Spelling, grammar, some typos. Lots of double-word
occurrences squashed as well.

Move ncr.4 out of man4.i386/ncr.4 and update it from NetBSD.
Remove one of the tekram cards from man4.i386/aha.4 that is really
ncr-based and add cd.4, fpa.4, sd.4, st.4, uk.4 from NetBSD
(some originally from FreeBSD).
