New POSIX xlocale implementation written from scratch.
Complete in the sense that all POSIX *locale(3) and *_l(3) functions
are included, but in OpenBSD, we of course only really care about
LC_CTYPE and we only support ASCII and UTF-8.

With important help from kettenis@, guenther@, and jca@.
Repeated testing in ports bulk builds by naddy@.
Additional testing by jca@, sebastia@, dcoppa@, and others.
OK kettenis@ dcoppa@, and guenther@ on an earlier version.
Riding guenther@'s libc/librthread major bump.

Stop exposing <sys/localedef.h> and various symbols internal to the libc
locale implementation: _{Current,Default}*Locale, __[mn]locale_changed,
__mb_len_max_runtime

Wrap <stdlib.h> so that calls go direct and the symbols not in the
C standard are all weak.
Apply __{BEGIN,END}_HIDDEN_DECLS to gdtoa{,imp}.h, hiding the
arch-specific __strtorx, __ULtox_D2A, __strtorQ, __ULtoQ_D2A symbols.

major abi changes:

* introduce the mbstate_t typedef.
impacts gnu libiconv, which has already been taken care of.

* Prepare for mb stuff to really exist, replace macro MB_CUR_MAX with
an external variable __mb_cur_max (impacts libX11 and various ports).

* use mbstate in all the mb <-> wchar functions with state.
* add a stub iswctype function allowing some ports to compile.

bash and gdiff are missing wcscoll, and need to be told there's no i18n
until this is fixed.
