KVM: selftests: Add coverage of EPT-disabled to vmx_dirty_log_test

Extend vmx_dirty_log_test to include accesses made by L2 when EPT is
disabled.

This commit adds explicit coverage of a bug caught by syzkaller, where
the TDP MMU would clear D-bits instead of write-protecting SPTEs being
used to map an L2, which only happens when L1 does not enable EPT,
causing writes made by L2 to not be reflected in the dirty log when PML
is enabled:

KVM: selftests: Assert in prepare_eptp() that nEPT is supported

Now that a VM isn't needed to check for nEPT support, assert that KVM
supports nEPT in prepare_eptp() instead of skipping the test, and push
the TEST_REQUIRE() check out to individual tests. The require+assert are
somewhat redundant and will incur some amount of ongoing maintenance
burden, but placing the "require" logic in the test makes it easier to
find/understand a test's requirements and in this case, provides a very
strong hint that the test cares about nEPT.

KVM: selftests: Fix filename reporting in guest asserts

Fix filename reporting in guest asserts by ensuring the GUEST_ASSERT
macro records __FILE__ and substituting REPORT_GUEST_ASSERT for many
repetitive calls to TEST_FAIL.

Previously filename was reported by using __FILE__ directly in the
selftest, wrongly assuming it would always be the same as where the
assertion failed.

KVM: selftests: Convert vmx_dirty_log_test away from VCPU_ID

Convert vmx_dirty_log_test to use vm_create_with_one_vcpu() and pass
around a 'struct kvm_vcpu' object instead of using a global VCPU_ID.
Note, this is a "functional" change in the sense that the test now
creates a vCPU with vcpu_id==0 instead of vcpu_id==1. The non-zero
VCPU_ID was 100% arbitrary and added little to no validation coverage.
If testing non-zero vCPU IDs is desirable for generic tests, that can be
done in the future by tweaking the VM creation helpers.

Almost all tests do this anyway and the ones that don't don't
appear to care. Only vmx_set_nested_state_test assumes that
a feature (VMX) is disabled until later setting the supported
