ALSA: ctxfi: Add SB046x PCI ID

Adds the PCI ID for X-Fi cards sold under the Platnum and XtremeMusic names

Before: snd_ctxfi 0000:05:05.0: chip 20K1 model Unknown (1102:0021) is found
After: snd_ctxfi 0000:05:05.0: chip 20K1 model SB046x (1102:0021) is found

[ This is only about defining the model name string, and the rest is
handled just like before, as a default unknown device.
Edward confirmed that the stuff has been working fine -- tiwai ]

ALSA: ctxfi: fix comment syntax in file headers

The opening comment mark '/**' is used for highlighting the beginning of
kernel-doc comments.
There are files in sound/pci/ctxfi which follow this syntax in their file
headers, i.e. start with '/**' like comments, which causes unexpected
warnings from kernel-doc.

E.g., running scripts/kernel-doc -none on sound/pci/ctxfi/ctresource.c
causes this warning:
"warning: wrong kernel-doc identifier on line:
* Copyright (C) 2008, Creative Technology Ltd. All Rights Reserved."

Similarly for other files too.

Provide a simple fix by replacing the kernel-doc like comment syntax with
general format, i.e. "/*", to prevent kernel-doc from parsing it.

ALSA: ctxfi: added reference of snd_card

added a pointer of snd_card in some of the structures to get a
reference of the card from other functions.
these references of snd_card will be initialised in the next patch
of this series and as of now these snd_card will be used to print the
the device information when we convert the pr_* macros to dev_* in a
later patch of this series.

ALSA: ctxfi: sparse warning

fixed sparse warning of incorrect type (different address spaces) in
cthw20k1.c and cthw20k2.c which was being actually caused as mem_base
was of the type unsigned long.

Again as mem_base was previously unsigned long , so it required many
typecasts in the code to convert interger to pointer.

Now after giving the correct type of mem_base as void __iomem *
we can also remove those typecasts maintaining the same functionality
and logic of the code.

ALSA: ctxfi: Add support for Creative Titanium HD

Initialise model-specific DAC and ADC parts.
Add controls for output and mic source selection.
Rename some mixer controls according to ControlNames.txt.
Remove Playback switches for Line-in and IEC958-in - these
were controlling the input mute/unmute which affected
capture too. Use the capture switches to control the
input mute/unmute instead - it's less confusing.
Initialise the WM8775 to invert the left-right clock
to swap the left and right channels of the mic and aux
input.

ALSA: ctxfi - Allow unknown PCI SSIDs

Allow unknown PCI SSIDs for emu20k1 and emu20k2 as "unknown" model.
Also, add a black-list check in case any device has to be listed
as "unsupported". It has a negative value in the pci quirk entry.

ALSA: ctxfi - Use native timer interrupt on emu20k1

emu20k1 has a native timer interrupt based on the audio clock, which
is more accurate than the system timer (from the synchronization POV).
This patch adds the code to handle this with multiple streams.

The system timer is still used on emu20k2, and can be used also for
emu20k1 easily by changing USE_SYSTEM_TIMER to 1 in cttimer.c.
