ALSA: msnd: Use proper mmap method

The old ISA MSND driver basically maps the iomem as the DMA buffer.
For this type of buffer, we have already the standard mmap helper,
snd_pcm_lib_mmap_iomem(). Correct the PCM info, set the DMA address
and use the standard helper for handling the mmap on all architectures
properly.

ALSA: msnd: constify snd_pcm_ops structures

snd_pcm_ops are not supposed to change at runtime. All functions
working with snd_pcm_ops provided by <sound/pcm.h> work with
const snd_pcm_ops. So mark the non-const structs as const.

ALSA: msnd: Remove always NULL parameter

snd_msnd_pcm() takes a pointer to a pointer of a PCM where if this parameter
is provided the newly allocated PCM is stored. All callers pass NULL though,
so remove the parameter. This makes the code a bit cleaner and shorter.
