USB: Follow-up to SPDX GPL-2.0+ identifiers addition - remove now useless comments

All these files have been updated in the commit given in the Fixes: tag
below.

When the SPDX-License-Identifier: has been added, the corresponding text at
the beginning of the files has not been deleted.
All these texts are about GPL-2.0+, with different variation in the
wording.

Remove these now useless lines to save some LoC.

rndis_host: enable the bogus MAC fixup for ZTE devices from cdc_ether

Certain ZTE modems, namely: MF823. MF831, MF910, built-in modem from
MF286R, expose both CDC-ECM and RNDIS network interfaces.
They have a trait of ignoring the locally-administered MAC address
configured on the interface both in CDC-ECM and RNDIS part,
and this leads to dropping of incoming traffic by the host.
However, the workaround was only present in CDC-ECM, and MF286R
explicitly requires it in RNDIS mode.

Re-use the workaround in rndis_host as well, to fix operation of MF286R
module, some versions of which expose only the RNDIS interface. Do so by
introducing new flag, RNDIS_DRIVER_DATA_DST_MAC_FIXUP, and testing for it
in rndis_rx_fixup. This is required, as RNDIS uses frame batching, and all
of the packets inside the batch need the fixup. This might introduce a
performance penalty, because test is done for every returned Ethernet
frame.

Apply the workaround to both "flavors" of RNDIS interfaces, as older ZTE
modems, like MF823 found in the wild, report the USB_CLASS_COMM class
interfaces, while MF286R reports USB_CLASS_WIRELESS_CONTROLLER.

USB: add SPDX identifiers to all remaining files in drivers/usb/

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

usb/net: rndis: break out <linux/rndis.h> defines

As a first step to consolidate the RNDIS implementations, break out
a common file with all the #defines and move it to <linux/rndis.h>.

This also deletes the immediate duplicated defines in the
<linux/rndis.h> file that yields a lot of compilation warnings.

usb/net: rndis: inline the cpu_to_le32() macro

The header file <linux/usb/rndis_host.h> used a number of #defines
that included the cpu_to_le32() macro to assure the result will be
in LE endianness. Inlining this into the code instead of using it
in the code definitions yields consolidation opportunities later
on as you will see in the following patches. The individual
drivers also used local defines - all are switched over to the
pattern of doing the conversion at the call sites instead.

rndis_host: Poll status before control channel where necessary

Some RNDIS devices don't respond on the control channel until polled
on the status channel. In particular, this was reported to be the
case for the 2Wire HomePortal 1000SW and for some Windows Mobile
devices.

This is roughly based on a patch by John Carr <john.carr@unrouted.co.uk>
which is currently applied by Mandriva.

apparent standards:

* macro name of __LINUX_USB_headerfile_H
* inclusion protection placed after leading comment block
* macro name added as a comment on the final #endif
* any obvious trivial whitespace cleanup associated with the above

Move usbnet.h and rndis_host.h to include/linux/usb

Move headers usbnet.h and rndis_host.h to include/linux/usb and fix includes
for drivers/net/usb modules. Headers are moved because rndis_wlan will be
outside drivers/net/usb in drivers/net/wireless and yet need these headers.
