xprtrdma: Boost client's max slot table size to match Linux server

I've heard rumors of an NFS/RDMA server implementation that has a
default credit limit of 1024. The client's default setting remains
at 128.

xprtrdma: Replace all usage of "frmr" with "frwr"

Clean up: Over time, the industry has adopted the term "frwr"
instead of "frmr". The term "frwr" is now more widely recognized.

For the past couple of years I've attempted to add new code using
"frwr" , but there still remains plenty of older code that still
uses "frmr". Replace all usage of "frmr" to avoid confusion.

While we're churning code, rename variables unhelpfully called "f"
to "frwr", to improve code clarity.

xprtrdma: Support larger inline thresholds

The Version One default inline threshold is still 1KB. But allow
testing with thresholds up to 64KB.

This maximum is somewhat arbitrary. There's no fundamental
architectural limit I'm aware of, but it's good to keep the size of
Receive buffers reasonable. Now that Send can use a s/g list, a
Send buffer is only as large as each RPC requires. Receive buffers
are always the size of the inline threshold, however.

xprtrdma: Bound the inline threshold values

Currently the sysctls that allow setting the inline threshold allow
any value to be set.

Small values only make the transport run slower. The default 1KB
setting is as low as is reasonable. And the logic that decides how
to divide a Send buffer between RPC-over-RDMA header and RPC message
assumes (but does not check) that the lower bound is not crazy (say,
57 bytes).

Send and receive buffers share a page with some control information.
Values larger than about 3KB can't be supported, currently.

xprtrdma: Increase default credit limit

In preparation for similar increases on NFS/RDMA servers, bump the
advertised credit limit for RPC/RDMA to 128. This allocates some
extra resources, but the client will continue to allow only the
number of RPCs in flight that the server requests via its advertised
credit limit.

xprtrdma: Remove RPCRDMA_PERSISTENT_REGISTRATION macro

Clean up.

RPCRDMA_PERSISTENT_REGISTRATION was a compile-time switch between
RPCRDMA_REGISTER mode and RPCRDMA_ALLPHYSICAL mode. Since
RPCRDMA_REGISTER has been removed, there's no need for the extra
conditional compilation.

Add defensive timeouts to wait_for_completion() calls in RDMA
address resolution, and make them interruptible. Fix the timeout
units to milliseconds (formerly jiffies) and move to private header.
