xen/evtchn: avoid WARN() when unbinding an event channel

When unbinding a user event channel, the related handler might be
called a last time in case the kernel was built with
CONFIG_DEBUG_SHIRQ. This might cause a WARN() in the handler.

Avoid that by adding an "unbinding" flag to struct user_event which
will short circuit the handler.

xen: evtchn: Allow shared registration of IRQ handers

Currently the handling of events is supported either in the kernel or
userspace, but not both.

In order to support fast delivery of interrupts from the guest to the
backend, we need to handle the Queue notify part of Virtio protocol in
kernel and the rest in userspace.

Update the interrupt handler registration flag to IRQF_SHARED for event
channels, which would allow multiple entities to bind their interrupt
handler for the same event channel port.

Also increment the reference count of irq_info when multiple entities
try to bind event channel to irqchip, so the unbinding happens only
after all the users are gone.

xen/evtchn: Introduce new IOCTL to bind static evtchn

Xen 4.17 supports the creation of static evtchns. To allow user space
application to bind static evtchns introduce new ioctl
"IOCTL_EVTCHN_BIND_STATIC". Existing IOCTL doing more than binding
that’s why we need to introduce the new IOCTL to only bind the static
event channels.

Static evtchns to be available for use during the lifetime of the
guest. When the application exits, __unbind_from_irq() ends up being
called from release() file operations because of that static evtchns
are getting closed. To avoid closing the static event channel, add the
new bool variable "is_static" in "struct irq_info" to mark the event
channel static when creating the event channel to avoid closing the
static evtchn.

Also, take this opportunity to remove the open-coded version of the
evtchn close in drivers/xen/evtchn.c file and use xen_evtchn_close().

xen/events: Remove disfunct affinity spreading

This function can only ever work when the event channels:

- are already established
- interrupts assigned to them
- the affinity has been set by user space already

because any newly set up event channel is forced to be bound to CPU0 and
the affinity mask of the interrupt is forced to contain cpumask_of(0).

As the CPU0 enforcement was in place _before_ this was implemented it's
entirely unclear how that can ever have worked at all.

Remove it as preparation for doing it proper.

xen/events: fix binding user event channels to cpus

When binding an interdomain event channel to a vcpu via
IOCTL_EVTCHN_BIND_INTERDOMAIN not only the event channel needs to be
bound, but the affinity of the associated IRQi must be changed, too.
Otherwise the IRQ and the event channel won't be moved to another vcpu
in case the original vcpu they were bound to is going offline.

*: convert stream-like files from nonseekable_open -> stream_open

Using scripts/coccinelle/api/stream_open.cocci added in 10dce8af3422
("fs: stream_open - opener for stream-like files so that read and write
can run simultaneously without deadlock"), search and convert to
stream_open all in-kernel nonseekable_open users for which read and
write actually do not depend on ppos and where there is no other methods
in file_operations which assume @offset access.

I've verified each generated change manually - that it is correct to convert -
and each other nonseekable_open instance left - that it is either not correct
to convert there, or that it is not converted due to current stream_open.cocci
limitations. The script also does not convert files that should be valid to
convert, but that currently have .llseek = noop_llseek or generic_file_llseek
for unknown reason despite file being opened with nonseekable_open (e.g.
drivers/input/mousedev.c)

Among cases converted 14 were potentially vulnerable to read vs write deadlock
(see details in 10dce8af3422):

The kvmalloc() function has a 2-factor argument form, kvmalloc_array(). This
patch replaces cases of:

kvmalloc(a * b, gfp)

with:
kvmalloc_array(a * b, gfp)

as well as handling cases of:

kvmalloc(a * b * c, gfp)

with:

kvmalloc(array3_size(a, b, c), gfp)

as it's slightly less ugly than:

kvmalloc_array(array_size(a, b), c, gfp)

This does, however, attempt to ignore constant size factors like:

kvmalloc(4 * 1024, gfp)

though any constants defined via macros get caught up in the conversion.

Any factors with a sizeof() of "unsigned char", "char", and "u8" were
dropped, since they're redundant.

The next patch from Al will sort out the final differences, and we
should be all done.

xen-evtchn: Bind dyn evtchn:qemu-dm interrupt to next online VCPU

A HVM domian booting generates around 200K (evtchn:qemu-dm xen-dyn)
interrupts,in a short period of time. All these evtchn:qemu-dm are bound
to VCPU 0, until irqbalance sees these IRQ and moves it to a different VCPU.
In one configuration, irqbalance runs every 10 seconds, which means
irqbalance doesn't get to see these burst of interrupts and doesn't
re-balance interrupts most of the time, making all evtchn:qemu-dm to be
processed by VCPU0. This cause VCPU0 to spend most of time processing
hardirq and very little time on softirq. Moreover, if dom0 kernel PREEMPTION
is disabled, VCPU0 never runs watchdog (process context), triggering a
softlockup detection code to panic.

Binding evtchn:qemu-dm to next online VCPU, will spread hardirq
processing evenly across different CPU. Later, irqbalance will try to balance
evtchn:qemu-dm, if required.

treewide: use kv[mz]alloc* rather than opencoded variants

There are many code paths opencoding kvmalloc. Let's use the helper
instead. The main difference to kvmalloc is that those users are
usually not considering all the aspects of the memory allocator. E.g.
allocation requests <= 32kB (with 4kB pages) are basically never failing
and invoke OOM killer to satisfy the allocation. This sounds too
disruptive for something that has a reasonable fallback - the vmalloc.
On the other hand those requests might fallback to vmalloc even when the
memory allocator would succeed after several more reclaim/compaction
attempts previously. There is no guarantee something like that happens
though.

This patch converts many of those places to kv[mz]alloc* helpers because
they are more conservative.

xen/evtchn: add IOCTL_EVTCHN_RESTRICT

IOCTL_EVTCHN_RESTRICT limits the file descriptor to being able to bind
to interdomain event channels from a specific domain. Event channels
that are already bound continue to work for sending and receiving
notifications.

This is useful as part of deprivileging a user space PV backend or
device model (QEMU). e.g., Once the device model as bound to the
ioreq server event channels it can restrict the file handle so an
exploited DM cannot use it to create or bind to arbitrary event
channels.

xen/evtchn: fix ring resize when binding new events

The copying of ring data was wrong for two cases: For a full ring
nothing got copied at all (as in that case the canonicalized producer
and consumer indexes are identical). And in case one or both of the
canonicalized (after the resize) indexes would point into the second
half of the buffer, the copied data ended up in the wrong (free) part
of the new buffer. In both cases uninitialized data would get passed
back to the caller.

Fix this by simply copying the old ring contents twice: Once to the
low half of the new buffer, and a second time to the high half.

This addresses the inability to boot a HVM guest with 64 or more
vCPUs. This regression was caused by 8620015499101090 (xen/evtchn:
dynamically grow pending event channel ring).

xen/evtchn: dynamically grow pending event channel ring

If more than 1024 event channels are bound to a evtchn device then it
possible (even with well behaved applications) for the ring to
overflow and events to be lost (reported as an -EFBIG error).

Dynamically increase the size of the ring so there is always enough
space for all bound events. Well behaved applicables that only unmask
events after draining them from the ring can thus no longer lose
events.

However, an application could unmask an event before draining it,
allowing multiple entries per port to accumulate in the ring, and a
overflow could still occur. So the overflow detection and reporting
is retained.

The ring size is initially only 64 entries so the common use case of
an application only binding a few events will use less memory than
before. The ring size may grow to 512 KiB (enough for all 2^17
possible channels). This order 7 kmalloc() may fail due to memory
fragmentation, so we fall back to trying vmalloc().

scalability of the evtchn device. Instead of the global array lookup,
use a per-use (per-fd) tree of event channels bound by that user and
protect the tree with a per-user lock.

This is also a prerequiste for extended the number of supported event
channels, by removing the fixed size, per-event channel array.

xen/evtchn: avoid a deadlock when unbinding an event channel

Unbinding an event channel (either with the ioctl or when the evtchn
device is closed) may deadlock because disable_irq() is called with
port_user_lock held which is also locked by the interrupt handler.

Think of the IOCTL_EVTCHN_UNBIND is being serviced, the routine has
just taken the lock, and an interrupt happens. The evtchn_interrupt
is invoked, tries to take the lock and spins forever.

A quick glance at the code shows that the spinlock is a local IRQ
variant. Unfortunately that does not help as "disable_irq() waits for
the interrupt handler on all CPUs to stop running. If the irq occurs
on another VCPU, it tries to take port_user_lock and can't because
the unbind ioctl is holding it." (from David). Hence we cannot
depend on the said spinlock to protect us. We could make it a system
wide IRQ disable spinlock but there is a better way.

We can piggyback on the fact that the existence of the spinlock is
to make get_port_user() checks be up-to-date. And we can alter those
checks to not depend on the spin lock (as it's protected by u->bind_mutex
in the ioctl) and can remove the unnecessary locking (this is
IOCTL_EVTCHN_UNBIND) path.

In the interrupt handler we cannot use the mutex, but we do not
need it.

"The unbind disables the irq before making the port user stale, so when
you clear it you are guaranteed that the interrupt handler that might
use that port cannot be running." (from David).

Hence this patch removes the spinlock usage on the teardown path
and piggybacks on disable_irq happening before we muck with the
get_port_user() data. This ensures that the interrupt handler will
never run on stale data.

xen/event: Add reference counting to event channels

Event channels exposed to userspace by the evtchn module may be used by
other modules in an asynchronous manner, which requires that reference
counting be used to prevent the event channel from being closed before
the signals are delivered.

The reference count on new event channels defaults to -1 which indicates
the event channel is not referenced outside the kernel; evtchn_get fails
if called on such an event channel. The event channels made visible to
userspace by evtchn have a normal reference count.

llseek: automatically add .llseek fop

All file_operations should get a .llseek operation so we can make
nonseekable_open the default for future file operations without a
.llseek pointer.

The three cases that we can automatically detect are no_llseek, seq_lseek
and default_llseek. For cases where we can we can automatically prove that
the file offset is always ignored, we use noop_llseek, which maintains
the current behavior of not returning an error from a seek.

New drivers should normally not use noop_llseek but instead use no_llseek
and call nonseekable_open at open time. Existing drivers can be converted
to do the same when the maintainer knows for certain that no user code
relies on calling seek on the device file.

The generated code is often incorrectly indented and right now contains
comments that clarify for each added line why a specific variant was
chosen. In the version that gets submitted upstream, the comments will
be gone and I will manually fix the indentation, because there does not
seem to be a way to do that using coccinelle.

Some amount of new code is currently sitting in linux-next that should get
the same modifications, which I will do at the end of the merge window.

unbind_from_irqhandler can end up doing /proc operations, which can't
happen under a spinlock. So before removing the IRQ handler,

won't start running), then remove the handler without the lock.

include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.

2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).

* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.
