x86/xen: attempt to inflate the memory balloon on PVH When running as PVH or HVM Linux will use holes in the memory map as scratch space to map grants, foreign domain pages and possibly miscellaneous other stuff. However the usage of such memory map holes for Xen purposes can be problematic. The request of holesby Xen happen quite early in the kernel boot process (grant table setup already uses scratch map space), and it's possible that by then not all devices have reclaimed their MMIO space. It's not unlikely for chunks of Xen scratch map space to end up using PCI bridge MMIO window memory, which (as expected) causes quite a lot of issues in the system. At least for PVH dom0 we have the possibility of using regions marked as UNUSABLE in the e820 memory map. Either if the region is UNUSABLE in the native memory map, or it has been converted into UNUSABLE in order to hide RAM regions from dom0, the second stage translation page-tables can populate those areas without issues. PV already has this kind of logic, where the balloon driver is inflated at boot. Re-use the current logic in order to also inflate it when running as PVH. onvert UNUSABLE regions up to the ratio specified in EXTRA_MEM_RATIO to RAM, while reserving them using xen_add_extra_mem() (which is also moved so it's no longer tied to CONFIG_PV). xen/balloon: Bring alloc(free)_xenballooned_pages helpers back This patch rolls back some of the changes introduced by commit 121f2faca2c0a "xen/balloon: rename alloc/free_xenballooned_pages" in order to make possible to still allocate xenballooned pages if CONFIG_XEN_UNPOPULATED_ALLOC is enabled. On Arm the unpopulated pages will be allocated on top of extended regions provided by Xen via device-tree (the subsequent patches will add required bits to support unpopulated-alloc feature on Arm). The problem is that extended regions feature has been introduced into Xen quite recently (during 4.16 release cycle). So this effectively means that Linux must only use unpopulated-alloc on Arm if it is running on "new Xen" which advertises these regions. But, it will only be known after parsing the "hypervisor" node at boot time, so before doing that we cannot assume anything. In order to keep working if CONFIG_XEN_UNPOPULATED_ALLOC is enabled and the extended regions are not advertised (Linux is running on "old Xen", etc) we need the fallback to alloc_xenballooned_pages(). This way we wouldn't reduce the amount of memory usable (wasting RAM pages) for any of the external mappings anymore (and eliminate XSA-300) with "new Xen", but would be still functional ballooning out RAM pages with "old Xen". Also rename alloc(free)_xenballooned_pages to xen_alloc(free)_ballooned_pages and make xen_alloc(free)_unpopulated_pages static inline in xen.h if CONFIG_XEN_UNPOPULATED_ALLOC is disabled. xen/balloon: add late_initcall_sync() for initial ballooning done When running as PVH or HVM guest with actual memory < max memory the hypervisor is using "populate on demand" in order to allow the guest to balloon down from its maximum memory size. For this to work correctly the guest must not touch more memory pages than its target memory size as otherwise the PoD cache will be exhausted and the guest is crashed as a result of that. In extreme cases ballooning down might not be finished today before the init process is started, which can consume lots of memory. In order to avoid random boot crashes in such cases, add a late init call to wait for ballooning down having finished for PVH/HVM guests. Warn on console if initial ballooning fails, panic() after stalling for more than 3 minutes per default. Add a module parameter for changing this timeout. xen: add helpers to allocate unpopulated memory To be used in order to create foreign mappings. This is based on the ZONE_DEVICE facility which is used by persistent memory devices in order to create struct pages and kernel virtual mappings for the IOMEM areas of such devices. Note that on kernels without support for ZONE_DEVICE Xen will fallback to use ballooned pages in order to create foreign mappings. The newly added helpers use the same parameters as the existing {alloc/free}_xenballooned_pages functions, which allows for in-place replacement of the callers. Once a memory region has been added to be used as scratch mapping space it will no longer be released, and pages returned are kept in a linked list. This allows to have a buffer of pages and prevents resorting to frequent additions and removals of regions. If enabled (because ZONE_DEVICE is supported) the usage of the new functionality untangles Xen balloon and RAM hotplug from the usage of unpopulated physical memory ranges to map foreign pages, which is the correct thing to do in order to avoid mappings of foreign pages depend on memory hotplug. Note the driver is currently not enabled on Arm platforms because it would interfere with the identity mapping required on some platforms. mm: remove unneeded includes of <asm/pgalloc.h> Patch series "mm: cleanup usage of <asm/pgalloc.h>" Most architectures have very similar versions of pXd_alloc_one() and pXd_free_one() for intermediate levels of page table. These patches add generic versions of these functions in <asm-generic/pgalloc.h> and enable use of the generic functions where appropriate. In addition, functions declared and defined in <asm/pgalloc.h> headers are used mostly by core mm and early mm initialization in arch and there is no actual reason to have the <asm/pgalloc.h> included all over the place. The first patch in this series removes unneeded includes of <asm/pgalloc.h> In the end it didn't work out as neatly as I hoped and moving pXd_alloc_track() definitions to <asm-generic/pgalloc.h> would require unnecessary changes to arches that have custom page table allocations, so I've decided to move lib/ioremap.c to mm/ and make pgalloc-track.h local to mm/. This patch (of 8): In most cases <asm/pgalloc.h> header is required only for allocations of page table memory. Most of the .c files that include that header do not use symbols declared in <asm/pgalloc.h> and do not require that header. As for the other header files that used to include <asm/pgalloc.h>, it is possible to move that include into the .c file that actually uses symbols from <asm/pgalloc.h> and drop the include from the header file. The process was somewhat automated using sed -i -E '/[<"]asm\/pgalloc\.h/d' \ $(grep -L -w -f /tmp/xx \ $(git grep -E -l '[<"]asm/pgalloc\.h')) where /tmp/xx contains all the symbols defined in arch/*/include/asm/pgalloc.h. Most of these definitions are actually identical and typically it boils down to, e.g. static inline unsigned long pmd_index(unsigned long address) { return (address >> PMD_SHIFT) & (PTRS_PER_PMD - 1); } static inline pmd_t *pmd_offset(pud_t *pud, unsigned long address) { return (pmd_t *)pud_page_vaddr(*pud) + pmd_index(address); } These definitions can be shared among 90% of the arches provided XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined. For architectures that really need a custom version there is always possibility to override the generic version with the usual ifdefs magic. These patches introduce include/linux/pgtable.h that replaces include/asm-generic/pgtable.h and add the definitions of the page table accessors to the new header. This patch (of 12): The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the functions involving page table manipulations, e.g. pte_alloc() and pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h> in the files that include <linux/mm.h>. The include statements in such cases are remove with a simple loop: for f in $(git grep -l "include <linux/mm.h>") ; do sed -i -e '/include <asm\/pgtable.h>/ d' $f done Signed-off-by: Mike Rapoport <rppt@linux.ibm.com> Signed-off-by: Andrew Morton <akpm@linux-foundation.org> Cc: Arnd Bergmann <arnd@arndb.de> Cc: Borislav Petkov <bp@alien8.de> Cc: Brian Cain <bcain@codeaurora.org> Cc: Catalin Marinas <catalin.marinas@arm.com> Cc: Chris Zankel <chris@zankel.net> Cc: "David S. proc/sysctl: add shared variables for range check In the sysctl code the proc_dointvec_minmax() function is often used to validate the user supplied value between an allowed range. This function uses the extra1 and extra2 members from struct ctl_table as minimum and maximum allowed value. On sysctl handler declaration, in every source file there are some readonly variables containing just an integer which address is assigned to the extra1 and extra2 members, so the sysctl range is enforced. The special values 0, 1 and INT_MAX are very often used as range boundary, leading duplication of variables like zero=0, one=1, int_max=INT_MAX in different source files: $ git grep -E '\.extra[12].*&(zero|one|int_max)' |wc -l 248 Add a const int array containing the most commonly used values, some macros to refer more easily to the correct array member, and use them instead of creating a local one for every object file. xen: let alloc_xenballooned_pages() fail if not enough memory free Instead of trying to allocate pages with GFP_USER in add_ballooned_pages() check the available free memory via si_mem_available(). GFP_USER is far less limiting memory exhaustion than the test via si_mem_available(). This will avoid dom0 running out of memory due to excessive foreign page mappings especially on ARM and on x86 in PVH mode, as those don't have a pre-ballooned area which can be used for foreign mappings. As the normal ballooning suffers from the same problem don't balloon down more than si_mem_available() pages in one iteration. At the same time limit the default maximum number of retries. xen/balloon: Fix mapping PG_offline pages to user space The XEN balloon driver - in contrast to other balloon drivers - allows to map some inflated pages to user space. Such pages are allocated via alloc_xenballooned_pages() and freed via free_xenballooned_pages(). The pfn space of these allocated pages is used to map other things by the hypervisor using hypercalls. Pages marked with PG_offline must never be mapped to user space (as this page type uses the mapcount field of struct pages). So what we can do is, clear/set PG_offline when allocating/freeing an inflated pages. This way, most inflated pages can be excluded by dumping tools and the "reused for other purpose" balloon pages are correctly not marked as PG_offline. Revert "xen/balloon: Mark unallocated host memory as UNUSABLE" This reverts commit b3cf8528bb21febb650a7ecbf080d0647be40b9f. That commit unintentionally broke Xen balloon memory hotplug with "hotplug_unpopulated" set to 1. As long as "System RAM" resource got assigned under a new "Unusable memory" resource in IO/Mem tree any attempt to online this memory would fail due to general kernel restrictions on having "System RAM" resources as 1st level only. The original issue that commit has tried to workaround fa564ad96366 ("x86/PCI: Enable a 64bit BAR on AMD Family 15h (Models 00-1f, 30-3f, 60-7f)") also got amended by the following 03a551734 ("x86/PCI: Move and shrink AMD 64-bit window to avoid conflict") which made the original fix to Xen ballooning unnecessary. mm/memory_hotplug: make add_memory() take the device_hotplug_lock add_memory() currently does not take the device_hotplug_lock, however is aleady called under the lock from arch/powerpc/platforms/pseries/hotplug-memory.c drivers/acpi/acpi_memhotplug.c to synchronize against CPU hot-remove and similar. In general, we should hold the device_hotplug_lock when adding memory to synchronize against online/offline request (e.g. from user space) - which already resulted in lock inversions due to device_lock() and mem_hotplug_lock - see 30467e0b3be ("mm, hotplug: fix concurrent memory hot-add deadlock"). add_memory()/add_memory_resource() will create memory block devices, so this really feels like the right thing to do. Holding the device_hotplug_lock makes sure that a memory block device can really only be accessed (e.g. via .online/.state) from user space, once the memory has been fully added to the system. The lock is not held yet in drivers/xen/balloon.c arch/powerpc/platforms/powernv/memtrace.c drivers/s390/char/sclp_cmd.c drivers/hv/hv_balloon.c So, let's either use the locked variants or take the lock. Don't export add_memory_resource(), as it once was exported to be used by XEN, which is never built as a module. If somebody requires it, we also have to export a locked variant (as device_hotplug_lock is never exported). Don't try to reach the configured target size by onlining new memory in this case, as the current memory size will be smaller in almost all cases due to e.g. BIOS reserved pages. Onlining new memory will lead to more problems e.g. undesired conflicts with NVMe devices meant to be operated as block devices. memory-hotplug: add automatic onlining policy for the newly added memory Currently, all newly added memory blocks remain in 'offline' state unless someone onlines them, some linux distributions carry special udev rules like: SUBSYSTEM=="memory", ACTION=="add", ATTR{state}=="offline", ATTR{state}="online" to make this happen automatically. This is not a great solution for virtual machines where memory hotplug is being used to address high memory pressure situations as such onlining is slow and a userspace process doing this (udev) has a chance of being killed by the OOM killer as it will probably require to allocate some memory. Introduce default policy for the newly added memory blocks in /sys/devices/system/memory/auto_online_blocks file with two possible values: "offline" which preserves the current behavior and "online" which causes all newly added memory blocks to go online as soon as they're added. The default is "offline". xen: Use correctly the Xen memory terminologies Based on include/xen/mm.h [1], Linux is mistakenly using MFN when GFN is meant, I suspect this is because the first support for Xen was for PV. This resulted in some misimplementation of helpers on ARM and confused developers about the expected behavior. For instance, with pfn_to_mfn, we expect to get an MFN based on the name. Although, if we look at the implementation on x86, it's returning a GFN. For clarity and avoid new confusion, replace any reference to mfn with gfn in any helpers used by PV drivers. The x86 code will still keep some reference of pfn_to_mfn which may be used by all kind of guests No changes as been made in the hypercall field, even though they may be invalid, in order to keep the same as the defintion in xen repo. Note that page_to_mfn has been renamed to xen_page_to_gfn to avoid a name to close to the KVM function gfn_to_page. Take also the opportunity to simplify simple construction such as pfn_to_mfn(page_to_pfn(page)) into xen_page_to_gfn. More complex clean up will come in follow-up patches. xen/balloon: Don't continue ballooning when BP_ECANCELED is encountered Commit 3dcf63677d4e ("xen/balloon: cancel ballooning if adding new memory failed") makes reserve_additional_memory() return BP_ECANCELED when an error is encountered. This error, however, is ignored by the caller (balloon_process()) since it is overwritten by subsequent call to update_schedule(). This results in continuous attempts to add more memory, all of which are likely to fail again. We should stop trying to schedule next iteration of ballooning when the current one has failed. xen/balloon: set ballooned out pages as invalid in p2m Since cd9151e26d31048b2b5e00fd02e110e07d2200c9 (xen/balloon: set a mapping for ballooned out pages), a ballooned out page had its entry in the p2m set to the MFN of one of the scratch pages. This means that the p2m will contain many entries pointing to the same MFN. During a domain save, these many-to-one entries are not identified as such and the scratch page is saved multiple times. On restore the ballooned pages are populated with new frames and the domain may use up its allocation before all pages can be restored. Since the original fix only needed to keep a mapping for the ballooned page it is safe to set ballooned out pages as INVALID_P2M_ENTRY in the p2m (as they were before). Thus preventing them from being saved and re-populated on restore. xen/balloon: flush persistent kmaps in correct position Xen balloon driver will update ballooned out pages' P2M entries to point to scratch page for PV guests. In 24f69373e2 ("xen/balloon: don't alloc page while non-preemptible", kmap_flush_unused was moved after updating P2M table. In that case for 32 bit PV guest we might end up with P2M X -----> S (S is mfn of balloon scratch page) M2P Y -----> X (Y is mfn in persistent kmap entry) kmap_flush_unused() iterates through all the PTEs in the kmap address space, using pte_to_page() to obtain the page. If the p2m and the m2p are inconsistent the incorrect page is returned. This will clear page->address on the wrong page which may cause subsequent oopses if that page is currently kmap'ed. Move the flush back between get_page and __set_phys_to_machine to fix this. xen, balloon: Fix CPU hotplug callback registration Subsystems that want to register CPU hotplug callbacks, as well as perform initialization for the CPUs that are already online, often do it as shown below: get_online_cpus(); for_each_online_cpu(cpu) init_cpu(cpu); register_cpu_notifier(&foobar_cpu_notifier); put_online_cpus(); This is wrong, since it is prone to ABBA deadlocks involving the cpu_add_remove_lock and the cpu_hotplug.lock (when running concurrently with CPU hotplug operations). The xen balloon driver doesn't take get/put_online_cpus() around this code, but that is also buggy, since it can miss CPU hotplug events in between the initialization and callback registration: for_each_online_cpu(cpu) init_cpu(cpu); ^ | Race window; Can miss CPU hotplug events here. v register_cpu_notifier(&foobar_cpu_notifier); Interestingly, the balloon code in xen can simply be reorganized as shown below, to have a race-free method to register hotplug callbacks, without even taking get/put_online_cpus(). This is because the initialization performed for already online CPUs is exactly the same as that performed for CPUs that come online later. Moreover, the code has checks in place to avoid double initialization. register_cpu_notifier(&foobar_cpu_notifier); get_online_cpus(); for_each_online_cpu(cpu) init_cpu(cpu); put_online_cpus(); A hotplug operation that occurs between registering the notifier and calling get_online_cpus(), won't disrupt anything, because the code takes care to perform the memory allocations only once. So reorganize the balloon code in xen this way to fix the issues with CPU hotplug callback registration. Fix that by avoiding any p2m modifications in both increase_reservation and decrease_reservation for auto_translated guests. xen/balloon: account for pages released during memory setup In xen_memory_setup() pages that occur in gaps in the memory map are released back to Xen. This reduces the domain's current page count in the hypervisor. The Xen balloon driver does not correctly decrease its initial current_pages count to reflect this. If 'delta' pages are released and the target is adjusted the resulting reservation is always 'delta' less than the requested target. This affects dom0 if the initial allocation of pages overlaps the PCI memory region but won't affect most domU guests that have been setup with pseudo-physical memory maps that don't have gaps. Fix this by accouting for the released pages when starting the balloon driver. If the domain's targets are managed by xapi, the domain may eventually run out of memory and die because xapi currently gets its target calculations wrong and whenever it is restarted it always reduces the target by 'delta'. xen: Mark all initial reserved pages for the balloon as INVALID_P2M_ENTRY. With this patch, we diligently set regions that will be used by the balloon driver to be INVALID_P2M_ENTRY and under the ownership of the balloon driver. We are OK using the __set_phys_to_machine as we do not expect to be allocating any P2M middle or entries pages. The set_phys_to_machine has the side-effect of potentially allocating new pages and we do not want that at this stage. We can do this because xen_build_mfn_list_list will have already allocated all such pages up to xen_max_p2m_pfn. We also move the check for auto translated physmap down the stack so it is present in __set_phys_to_machine. include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h percpu.h is included by sched.h and module.h and thus ends up being included when building most .c files. percpu.h includes slab.h which in turn includes gfp.h making everything defined by the two files universally available and complicating inclusion dependencies. percpu.h -> slab.h dependency is about to be removed. Prepare for this change by updating users of gfp and slab facilities include those headers directly instead of assuming availability. As this conversion needs to touch large number of source files, the following script is used as the basis of conversion. http://userweb.kernel.org/~tj/misc/slabh-sweep.py The script does the followings. * Scan files for gfp and slab usages and update includes such that only the necessary includes are there. ie. if only gfp is used, gfp.h, if slab is used, slab.h. * When the script inserts a new include, it looks at the include blocks and try to put the new include such that its order conforms to its surrounding. It's put in the include block which contains core kernel includes, in the same order that the rest are ordered - alphabetical, Christmas tree, rev-Xmas-tree or at the end if there doesn't seem to be any matching order. * If the script can't find a place to put a new include (mostly because the file doesn't have fitting include block), it prints out an error message indicating which .h file needs to be added to the file. The conversion was done in the following steps. 1. The initial automatic conversion of all .c files updated slightly over 4000 files, deleting around 700 includes and adding ~480 gfp.h and ~3000 slab.h inclusions. The script emitted errors for ~400 files. 2. Each error was manually checked. Some didn't need the inclusion, some needed manual addition while adding it to implementation .h or embedding .c file was more appropriate for others. This step added inclusions to around 150 files. 3. The script was run again and the output was compared to the edits from #2 to make sure no file was left behind. 4. Several build tests were done and a couple of problems were fixed. e.g. lib/decompress_*.c used malloc/free() wrappers around slab APIs requiring slab.h to be added manually. 5. The script was run on all .h files but without automatically editing them as sprinkling gfp.h and slab.h inclusions around .h files could easily lead to inclusion dependency hell. Most gfp.h inclusion directives were ignored as stuff from gfp.h was usually wildly available and often used in preprocessor macros. Each slab.h inclusion directive was examined and added manually as necessary. 6. percpu.h was updated not to include slab.h. 7. Build test were done on the following configurations and failures were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my distributed build env didn't work with gcov compiles) and a few more options had to be turned off depending on archs to make things build (like ipr on powerpc/64 which failed due to missing writeq). * x86 and x86_64 UP and SMP allmodconfig and a custom test config. * powerpc and powerpc64 SMP allmodconfig * sparc and sparc64 SMP allmodconfig * ia64 SMP allmodconfig * s390 SMP allmodconfig * alpha SMP allmodconfig * um on x86_64 SMP allmodconfig 8. percpu.h modifications were reverted so that it could be applied as a separate patch and serve as bisection point. Given the fact that I had only a couple of failures from tests on step 6, I'm fairly confident about the coverage of this conversion patch. If there is a breakage, it's likely to be something in one of the arch headers which should be easily discoverable easily on most builds of the specific arch. xen: try harder to balloon up under memory pressure. Currently if the balloon driver is unable to increase the guest's reservation it assumes the failure was due to reaching its full allocation, gives up on the ballooning operation and records the limit it reached as the "hard limit". The driver will not try again until the target is set again (even to the same value). However it is possible that ballooning has in fact failed due to memory pressure in the host and therefore it is desirable to keep attempting to reach the target in case memory becomes available. The most likely scenario is that some guests are ballooning down while others are ballooning up and therefore there is temporary memory pressure while things stabilise. You would not expect a well behaved toolstack to ask a domain to balloon to more than its allocation nor would you expect it to deliberately over-commit memory by setting balloon targets which exceed the total host memory. This patch drops the concept of a hard limit and causes the balloon driver to retry increasing the reservation on a timer in the same manner as when decreasing the reservation. Also if we partially succeed in increasing the reservation (i.e. receive less pages than we asked for) then we may as well keep those pages rather than returning them to Xen.
