USB: storage: replace HTTP links with HTTPS ones

Rationale:
Reduces attack surface on kernel devs opening the links for MITM
as HTTPS traffic is much harder to manipulate.

Deterministic algorithm:
For each file:
If not .svg:
For each line:
If doesn't contain `\bxmlns\b`:
For each link, `\bhttp://[^# \t\r\n]*(?:\w|/)`:
If neither `\bgnu\.org/license`, nor `\bmozilla\.org/MPL\b`:
If both the HTTP and HTTPS versions
return 200 OK and serve the same content:
Replace HTTP with HTTPS.

usb-storage: export symbols in USB_STORAGE namespace

Modules using these symbols are required to explicitly import the
namespace. This patch was generated with the following steps and serves
as a reference to use the symbol namespace feature:

1) Define DEFAULT_SYMBOL_NAMESPACE in the corresponding Makefile
2) make (see warnings during modpost about missing imports)
3) make nsdeps

Instead of a DEFAULT_SYMBOL_NAMESPACE definition, the EXPORT_SYMBOL_NS
variants can be used to explicitly specify the namespace. The advantage
of the method used here is that newly added symbols are automatically
exported and existing ones are exported without touching their
respective EXPORT_SYMBOL macro expansion.

usb: storage: Replace mdelay with msleep in init_freecom

init_freecom() is never called in atomic context.

init_freecom() is set as ".initFunction" through UNUSUAL_DEV().
And ->initFunction() is only called by usb_stor_acquire_resources(),
which is only called by usb_stor_probe2().
usb_stor_probe2() is called by *_probe() functions (like alauda_probe())
for each USB driver.
*_probe() functions are set ".probe" in struct usb_driver.
These functions are not called in atomic context.

Despite never getting called from atomic context, init_freecom()
calls mdelay() to busily wait.
This is not necessary and can be replaced with msleep() to
avoid busy waiting.

This is found by a static analysis tool named DCNS written by myself.
And I also manually check it.

USB: storage: Remove redundant license text

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner. So the extra GPL text wording
can be removed as it is no longer needed at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.

USB: add SPDX identifiers to all remaining files in drivers/usb/

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

usb: storage: fix module reference for scsi host

While accessing a unusual usb storage (ums-alauda, ums-cypress, ...),
the module reference count is not incremented. Because these drivers
allocate scsi hosts with usb_stor_host_template defined in usb-storage
module. So these drivers always can be unloaded.

This fixes it by preparing scsi host template which is initialized
at module_init() for each ums-* driver. In order to minimize the
difference in ums-* drivers, introduce module_usb_stor_driver() helper
macro which is same as module_usb_driver() except that it also
initializes scsi host template.

usb: remove libusual

The "Low Performance USB Block driver" has been removed which a user of
libusual. Now we have only the usb-storage driver as the only driver in
tree. This makes libusual needless.
This patch removes libusal, fixes up all users. The usual-table is now
linked into usb-storage.
usb_usual.h remains in public include directory because some staging
users seem to need it.

This converts the drivers in drivers/usb/* to use the
module_usb_driver() macro which makes the code smaller and a bit
simpler.

Added bonus is that it removes some unneeded kernel log messages about

Linux-2.6.12-rc2

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
