usb: ohci-exynos: Convert to platform remove callback returning void

The .remove() callback for a platform driver returns an int which makes
many driver authors wrongly assume it's possible to do error handling by
returning an error code. However the value returned is ignored (apart from
emitting a warning) and this typically results in resource leaks. To improve
here there is a quest to make the remove callback return void. In the first
step of this quest all drivers are converted to .remove_new() which already
returns void. Eventually after all drivers are converted, .remove_new() is
renamed to .remove().

Trivially convert this driver from always returning zero in the remove
callback to the void returning variant.

usb: host: ohci-exynos: Convert to devm_of_phy_optional_get()

Use the new devm_of_phy_optional_get() helper instead of open-coding the
same operation.

As devm_of_phy_optional_get() returns NULL if either the PHY cannot be
found, or if support for the PHY framework is not enabled, it is no
longer needed to check for -ENODEV or -ENOSYS.

This lets us drop several checks for IS_ERR(), as phy_power_{on,off}()
handle NULL parameters fine.

usb: reduce kernel log spam on driver registration

Drivers are typically supposed to be quiet unless they are actually
probed, but for some reason, USB host controllers seem to be exempt from
this rule, and happily broadcast their existence into the kernel log at
boot even if the hardware in question is nowhere to be found.

Let's fix that, and remove these pr_info() calls.

If the function platform_get_irq() failed, the negative value
returned will not be detected here. So fix error handling in
exynos_ohci_probe(). And when get irq failed, the function
platform_get_irq() logs an error message, so remove redundant
message here.

usb: exynos: Rename Samsung and Exynos to lowercase

Fix up inconsistent usage of upper and lowercase letters in "Samsung"
and "Exynos" names.

"SAMSUNG" and "EXYNOS" are not abbreviations but regular trademarked
names. Therefore they should be written with lowercase letters starting
with capital letter.

The lowercase "Exynos" name is promoted by its manufacturer Samsung
Electronics Co., Ltd., in advertisement materials and on website.

Although advertisement materials usually use uppercase "SAMSUNG", the
lowercase version is used in all legal aspects (e.g. on Wikipedia and in
privacy/legal statements on
https://www.samsung.com/semiconductor/privacy-global/).

usb: exynos: add support for getting PHYs from the standard dt array

Add the code for getting generic PHYs from standard device tree array
from the main controller device node. This is a first step in resolving
the conflict between Exynos EHCI/OHCI sub-nodes and generic USB device
bindings. Later the sub-nodes currently used for assigning PHYs to root
ports of the controller will be removed making a place for the generic
USB device bindings nodes.

usb: exynos: add workaround for the USB device bindings conflict

Commit 69bec7259853 ("USB: core: let USB device know device node") added
support for attaching devicetree node for USB devices. Those nodes are
children of their USB host controller. However Exynos EHCI and OHCI
driver bindings already define child-nodes for each physical root hub
port and assigns respective PHY controller and parameters to them. Those
bindings predates support for USB device tree nodes.

To mitigate the side-effects of the conflict between those bindings,
lets reset Exynos host controller of_node pointer before registering it
to USB subsystem. This fixes the issue raised by the commit 01fdf179f4b0
("usb: core: skip interfaces disabled in devicetree"), which incorrectly
disabled some devices on Exynos based boards.

USB: host: ohci: Remove redundant license text

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner. So the extra GPL text wording
can be removed as it is no longer needed at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.

USB: add SPDX identifiers to all remaining files in drivers/usb/

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

usb: ehci/ohci-exynos: Fix of_node_put() for child when getting PHYs

On enabling CONFIG_OF_SELFTEST which enables CONFIG_OF_DYNAMIC,
we found out that while getting PHYs for the controller we were
doing an extra of_node_put on the child node in our routines -
exynos_e/ohci_get_phy().
This child is however already put by of_get_next_available_child()
which does a of_node_put() on the "prev" node. So there's no point
in putting the same node again in our routine.

newer generic phys, lets first get the generic PHYs and fallback to
older USB-PHYs only when we fail to get the former.
This should fix the issue with ehci-exynos and ohci-exynos, wherein
in the absence of SAMSUNG_USB2PHY config symbol, we end up getting
the NOP_USB_XCEIV phy when the same is enabled. And thus the PHYs
are not configured properly.

Using devm_ioremap_resource() API should actually be preferred over
devm_ioremap(), since the former request the mem region first and then
gives back the ioremap'ed memory pointer.
devm_ioremap_resource() calls request_mem_region(), therby preventing
other drivers to make any overlapping call to the same region.

... where tegra_ehci_dma_mask is a global. I suspect this code originated
in commit 4a53f4e "USB: ehci-tegra: add probing through device tree" and
was simply copied everywhere else.

This works fine when the code is built-in, but can cause a crash when the
code is in a module. The first module load sets up the dma_mask pointer,
but if the module is removed and re-inserted, the value is now non-NULL,
and hence is not updated to point at the new location, and hence points
at a stale location within the previous module load address, which in
turn causes a crash if the pointer is de-referenced.

The simplest way of solving this seems to be to copy the code from
ehci-platform.c, which uses the coherent_dma_mask as the target for the
dma_mask pointer.

usb: exynos: do not include plat/usb-phy.h

The definitions have moved to include/linux/usb/samsung-usb-phy.h,
and plat/usb-phy.h is unavailable from drivers in a multiplatform
configuration.

Also fix up the plat/usb-phy.h header file to use the definitions
from the new header instead of providing a separate copy.

USB: ohci-exynos: initialize registers pointer earlier

In the former code, we have a race condition between the first interrupt
and the regs field initilization in the usb_hcd structure.
If the OHCI irq fires before hcd->regs is set, we are getting a null
pointer dereference in ohci_irq.

When calling usb_add_hcd(), it first executes the reset() callback,
then enables the ohci interrupt, and finally executes the start()
callback. So moving the ohci_init() call which actually initializes the
reg field from start() to reset() should remove the race.

Tested by enabling the external HSIC hub in the bootloader on an exynos5
machine and booting. With the former code, this triggers an early interrupt
about 50% of the boots and a subsequent kernel panic in ohci_irq when trying
to access the registers.

USB: ohci: merge ohci_finish_controller_resume with ohci_resume

Merge ohci_finish_controller_resume with ohci_resume as suggested by Alan
Stern. Since ohci_finish_controller_resume no longer exists, update the
various OHCI drivers to call ohci_resume() instead. Some drivers used to set
themselves the bit HCD_FLAG_HW_ACCESSIBLE, which is now handled by
ohci_resume().
