usb: dwc2: add otg_rev and otg_caps information for gadget driver

Currently the dwc2 doesn't fill in the 'otg_caps' of usb_gadget structure.
When registering a gadget device (e.g. via configfs), the
usb_otg_descriptor_init() checks the 'otg_caps' and 'otg_rev'. It defaults
to HNP and SRP bmAttributes if unspecified. There may be a mismatch with
what's being set in dwc2 params structure. This result in the descriptors
to be miss-configured in this case.

So replace 'otg_cap' bit field by 'otg_caps' structure, so hnp, srp and
otg_rev' can be configured directly in the params.
It's then provided to the gadget struct. These parameters can be tuned
for each platform. In case it's not set, it will default to current
behavior.

Also add option to setup these from the device tree by calling
of_usb_update_otg_caps(). This provides support for standard properties
such as "otg-rev", "hnp-disable" and "srp-disable" (see usb-drd.yaml).

USB: ch9: add "USB_" prefix in front of TEST defines

For some reason, the TEST_ defines in the usb/ch9.h files did not have
the USB_ prefix on it, making it a bit confusing when reading the file,
as well as not the nicest thing to do in a uapi file.

So fix that up and add the USB_ prefix on to them, and fix up all
in-kernel usages. This included deleting the duplicate copy in the
net2272.h file.

usb: dwc2: debugfs: Don't touch RX FIFO during register dump

Dumping the registers via debugfs makes USB on Raspberry Pi completely
unusable. The read of register GRXSTSP ("Receive Status Read and Pop
Register") is responsible for this behaviour, because it pops the RX FIFO.
So avoid this by omitting the relevant register.

USB: dwc2: Remove redundant license text

Now that the SPDX tag is in all USB files, that identifies the license
in a specific and legally-defined manner. So the extra GPL text wording
can be removed as it is no longer needed at all.

This is done on a quest to remove the 700+ different ways that files in
the kernel describe the GPL license text. And there's unneeded stuff
like the address (sometimes incorrect) for the FSF which is never
needed.

No copyright headers or other non-license-description text was removed.

USB: add SPDX identifiers to all remaining files in drivers/usb/

It's good to have SPDX identifiers in all files to make it easier to
audit the kernel tree for correct licenses.

Update the drivers/usb/ and include/linux/usb* files with the correct
SPDX license identifier based on the license text in the file itself.
The SPDX identifier is a legally binding shorthand, which can be used
instead of the full boiler plate text.

This work is based on a script and data from Thomas Gleixner, Philippe
Ombredanne, and Kate Stewart.

This commmit is the result of running checkpatch --fix.

The results were verified for correctness. Some of the fixes result in
line over 80 char which we will fix manually later.

The following is a summary of what was done by checkpatch:
* Remove externs on function prototypes.
* Replace symbolic permissions with octal.
* Align code to open parens.
* Replace 'unsigned' with 'unsigned int'.
* Remove unneccessary blank lines.
* Add blank lines after declarations.
* Add spaces around operators.
* Remove unnecessary spaces after casts.
* Replace 'x == NULL' with '!x'.
* Replace kzalloc() with kcalloc().
* Concatenate multi-line strings.
* Use the BIT() macro.

usb: dwc2: Use platform endianness when accessing registers

This patch switches calls to readl/writel to their
dwc2_readl/dwc2_writel equivalents which preserve platform endianness.

This patch is necessary to access dwc2 registers correctly on big-endian
systems such as the mips based SoCs made by Lantiq. Then dwc2 can be
used to replace ifx-hcd driver for Lantiq platforms found e.g. in
OpenWrt.

The patch was autogenerated with the following commands:
$EDITOR core.h
sed -i "s/\<readl\>/dwc2_readl/g" *.c hcd.h hw.h
sed -i "s/\<writel\>/dwc2_writel/g" *.c hcd.h hw.h

Some files were then hand-edited to fix checkpatch.pl warnings about
too long lines.

usb: dwc2: remove dwc2_platform.ko

As dwc2 pci module is now exporting dwc2 platform device, include
platform.o in dwc2-y and remove USB_DWC2_PLATFORM configuration
option. Driver will be built as two modules, dwc2.ko and dwc2_pci.ko.
dwc2.ko is the new platform driver.

Remove all EXPORT_SYMBOL_GPL as they are not needed any more.

Prepare to add more debug code. Moreover, don't save dentry * for
each file in struct dwc2_hsotg as clean up is done with
debugfs_remove_recursive(). s3c_hsotg_delete_debug() is removed
