spi: bitbang: Follow renaming of SPI "master" to "controller"

In commit 8caab75fd2c2 ("spi: Generalize SPI "master" to "controller"")
some functions and struct members were renamed. To not break all drivers
compatibility macros were provided.

To be able to remove these compatibility macros push the renaming into
the SPI bitbang controller drivers.

spi: Replace all spi->chip_select and spi->cs_gpiod references with function call

Supporting multi-cs in spi drivers would require the chip_select & cs_gpiod
members of struct spi_device to be an array. But changing the type of these
members to array would break the spi driver functionality. To make the
transition smoother introduced four new APIs to get/set the
spi->chip_select & spi->cs_gpiod and replaced all spi->chip_select and
spi->cs_gpiod references with get or set API calls.
While adding multi-cs support in further patches the chip_select & cs_gpiod
members of the spi_device structure would be converted to arrays & the
"idx" parameter of the APIs would be used as array index i.e.,
spi->chip_select[idx] & spi->cs_gpiod[idx] respectively.

spi: au1550: Convert to platform remove callback returning void

The .remove() callback for a platform driver returns an int which makes
many driver authors wrongly assume it's possible to do error handling by
returning an error code. However the value returned is (mostly) ignored
and this typically results in resource leaks. To improve here there is a
quest to make the remove callback return void. In the first step of this
quest all drivers are converted to .remove_new() which already returns
void.

Trivially convert this driver from always returning zero in the remove
callback to the void returning variant.

spi: au1550: Simplify au1550_spi_setupxfer()

SPI core validates both bits_per_word and speed_hz transfer parameters and
defaults to spi->bits_per_word and spi->max_speed_hz in case these per
transfer parameters are not set. This can simplify a little the
au1550_spi_setupxfer() as there is need to check only for valid
"struct spi_transfer" pointer.

spi: spi-au1550: replace platform_driver_probe to support deferred probing

Subsystems like pinctrl and gpio rightfully make use of deferred probing at
core level. Now, deferred drivers won't be retried if they don't have a .probe
function specified in the driver struct. Fix this driver to have that, so the
devices it supports won't get lost in a deferred probe.

Many drivers that use bitbang library have a leak on probe error paths.
This is because once a spi_master_get() call succeeds, we need an additional
spi_master_put() call to free the memory.

Fix this issue by moving the code taking a reference to master to
spi_bitbang_start(), so spi_bitbang_start() will take a reference to master on
success. With this change, the caller is responsible for calling
spi_bitbang_stop() to decrement the reference and spi_master_put() as
counterpart of spi_alloc_master() to prevent a memory leak.

So now we have below patten for drivers using bitbang library:

probe:
spi_alloc_master -> Init reference count to 1
spi_bitbang_start -> Increment reference count
remove:
spi_bitbang_stop -> Decrement reference count
spi_master_put -> Decrement reference count (reference count reaches 0)

Fixup all users accordingly.

spi: remove unnecessary platform_set_drvdata()

The driver core clears the driver data to NULL after device_release
or on probe failure, since commit 0998d0631001288a5974afc0b2a5f568bcdecb4d
(device-core: Ensure drvdata = NULL when no driver is bound).
Thus, it is not needed to manually clear the device driver data to NULL.

spi: Remove erroneous __init, __exit and __exit_p() references in drivers

Some of the spi driver module remove hooks were annotated with __exit
and referenced with __exit_p(). Presumably these were supposed to be
__devinit, __devexit and __devexit_p() since __init/__exit for a
probe/remove hook has never been correct. They also got missed during
the big __devinit/__devexit purge since they didn't match the pattern.
Remove then now to be rid of it.

spi: reorganize drivers

Sort the SPI makefile and enforce the naming convention spi_*.c for
spi drivers.

This change also rolls the contents of atmel_spi.h into the .c file
since there is only one user of that particular include file.

v2: - Use 'spi-' prefix instead of 'spi_' to match what seems to be
be the predominant pattern for subsystem prefixes.
- Clean up filenames in Kconfig and header comment blocks
