scsi: qla2xxx: Remove in_interrupt() from qla82xx-specific code qla82xx_idc_lock() spins on a certain hardware state until it's updated. At the end of each spin, if in_interrupt() is true, it does 20 loops of cpu_relax(). Otherwise, it yields the CPU. While in_interrupt() is ill-defined and does not provide what the name suggests, it is not needed here: qla82xx_idc_lock() is always called from process context. Below is an analysis of its callers, in order of appearance: - qla_nx.c: qla82xx_device_bootstrap(), only called by qla82xx_device_state_handler(), has multiple msleep()s. - qla_nx.c: qla82xx_need_qsnt_handler(), has one second msleep() - qla_nx.c: qla82xx_wait_for_state_change(), one second msleep() - qla_nx.c: qla82xx_need_reset_handler(), can sleep up to 10 seconds - qla_nx.c: qla82xx_device_state_handler(), has multiple msleep()s - qla_nx.c: qla82xx_abort_isp(), if it's a qla82xx controller, calls qla82xx_device_state_handler(), which sleeps. It's also bound to isp_operations ->abort_isp() hook, where all the callers are in process context. - qla_nx.c: qla82xx_beacon_on(), bound to isp_operations ->beacon_on() hook. That hook is only called once, in a mutex locked context, from qla2x00_beacon_store(). - qla_nx.c: qla82xx_beacon_off(), bound to isp_operations ->beacon_off() hook. Like ->beacon_on(), it's only called once, in a mutex locked context, from qla2x00_beacon_store(). - qla_nx.c: qla82xx_fw_dump(), calls qla2x00_wait_for_chip_reset(), which has msleep() in a loop. It is bound to isp_operations ->fw_dump() hook. That hook *is* called from atomic context at qla_isr.c by multiple interrupt handlers. Nonetheless, it's other controllers interrupt handlers, and not the qla82xx. - qla82xx_msix_default() and qla82xx_msix_rsp_q() call qla24xx_process_response_queue() which doesn't implement the firmware dumping. - qla_attr.c: qla2x00_sysfs_write_fw_dump(), and qla2x00_sysfs_write_reset(), process-context sysfs ->write() hooks. - qla_os.c: qla2x00_probe_one(). PCI ->probe(), process context. - qla_os.c: qla2x00_clear_drv_active(), called solely from qla2x00_remove_one(), which is PCI ->remove() hook, process context. - qla_os.c: qla2x00_do_dpc(), kthread function, process context. Remove the in_interrupt() check. Change qla82xx_idc_lock() specification to a purely process-context function. Mark it with "Context: task, might sleep". Change qla82xx_idc_lock() implementation to sleep 100ms, instead of a schedule(), for each spin. This is more deterministic, and it matches the other qla models idc_lock() functions. scsi: qla2xxx: Remove defer flag to indicate immeadiate port loss During cable pull test case, if the port is disconnected for time larger than devloss timeout, driver does not mark path offline. In such case, instead of notifying SCSI-ML of loop down, driver goes into endless loop of device relogin because defer flag is set. With newer handling of device relogin in driver discovery, defer flag is now redundant. This patch removes defer flag and cleans up code handling port lost indication to SCSI-ML. treewide: kmalloc() -> kmalloc_array() The kmalloc() function has a 2-factor argument form, kmalloc_array(). This patch replaces cases of: kmalloc(a * b, gfp) with: kmalloc_array(a * b, gfp) as well as handling cases of: kmalloc(a * b * c, gfp) with: kmalloc(array3_size(a, b, c), gfp) as it's slightly less ugly than: kmalloc_array(array_size(a, b), c, gfp) This does, however, attempt to ignore constant size factors like: kmalloc(4 * 1024, gfp) though any constants defined via macros get caught up in the conversion. Any factors with a sizeof() of "unsigned char", "char", and "u8" were dropped, since they're redundant. The tools/ directory was manually excluded, since it has its own implementation of kmalloc(). [SCSI] qla2xxx: Disable adapter when we encounter a PCI disconnect. If we become disconnected from the PCI bus/PCIe fabric, there can be long delays in register reads which can cause erroneous decisions to be made and cause a soft lockup if a lock is held too long. As a preventative measure, check for a disconnection (register reads that return -1) and then disable the board if we find ourselves in this condition. For now, check in our interrupt handlers and the per adapter one second timer. Under certain conditions, this can lead to multiple mailbox command completions being signaled, which, in turn, leads to a false mailbox timeout error for the subsequently issued mailbox command. The issue occurs frequently but intermittenly with the Qlogic 8GFC mezz card during card initialization, resulting in card initialization failure. Setting ql2xextended_error_logging module parameter to any of the above value, will enable the debug for that particular level. Do LOGICAL OR of the value to enable more than one level. [SCSI] qla2xxx: Handle outstanding mbx cmds on hung f/w scenarios. Outstanding mailbox commands, have no way to recover on f/w hung, and we timeout on waiting for mbx response. This in turn affects the recovery process as follows: - We might already be in dpc while waiting for mbx to complete, so recovery for that pci function will never get invoked. Reset Timeout (10 sec) is far less than mbx timeout (30 sec). - Other mbx cmds will get stuck due to serial mbx access. Solution is to identify fw-hung scenario and handle outstanding mbx commands to have an early-exit instead of waiting for response. Other mbx commands waiting for access will also do an early-exit if fw-hung is still applicable.
