scsi: pm8001: Remove PM8001_USE_MSIX The pm8001 driver does not compile if PM8001_USE_MSIX is not defined in pm8001_sas.h because various fields and functions conditionally defined are used unconditionally without a "#ifdef PM8001_USE_MSIX" protection. This macro is rather useless anyway and not convenient as diabling MSI-X use requires recompiling the driver. Remove this macro and replace it with the bool module parameter "use_msix" which defaults to true. The use of MSI-X interrupts for an adapter is gated by this module parameter for adapters that actually support MSI-X. The "use_msix" boolean field is added to struct pm8001_hba_info and all code defined depending on PM8001_USE_MSIX is modified to rely on pm8001_hba_info->use_msix instead. scsi: pm8001: Setup IRQs on resume The function pm8001_pci_resume() only calls pm8001_request_irq() without calling pm8001_setup_irq(). This causes the IRQ allocation to fail, which leads all drives being removed from the system. Fix this issue by integrating the code for pm8001_setup_irq() directly inside pm8001_request_irq() so that MSI-X setup is performed both during normal initialization and resume operations. This changed the execution order of allocation. pm8001_tag_init() used to be called after the pm8001_ha->tags allocation and now it is called before the allocation. Before: pm8001_pci_probe() `--> pm8001_pci_alloc() `--> pm8001_alloc() `--> pm8001_ha->tags = kzalloc(...) `--> pm8001_tag_init(pm8001_ha); // OK: tags are allocated After: pm8001_pci_probe() `--> pm8001_pci_alloc() | `--> pm8001_alloc() | `--> pm8001_tag_init(pm8001_ha); // NOK: tags are not allocated | `--> pm8001_init_ccb_tag() `--> pm8001_ha->tags = kzalloc(...) // today it is bitmap_zalloc() Since pm8001_ha->tags_num is zero when pm8001_tag_init() is called it does nothing. Tags memory is allocated with bitmap_zalloc() so there is no need to manually clear each bit with pm8001_tag_free(). scsi: pm8001: Expose hardware queues for pm80xx In commit 05c6c029a44d ("scsi: pm80xx: Increase number of supported queues"), support for 80xx chip was improved by enabling multiple HW queues. In this, like other SCSI MQ HBA drivers at the time, the HW queues were not exposed to upper layer, and instead the driver managed the queues internally. However, this management duplicates blk-mq code. In addition, the HW queue management is sub-optimal for a system where the number of CPUs exceeds the HW queues - this is because queues are selected in a round-robin fashion, when it would be better to make adjacent CPUs submit on the same queue. And finally, the affinity of the completion queue interrupts is not set to mirror the cpu<->HQ queue mapping, which is suboptimal. As such, for when MSIX is supported, expose HW queues to upper layer. We always use queue index #0 for "internal" commands, i.e. anything which does not come from the block layer, so omit this from the affinity spreading. scsi: pm8001: Set up tags before using them The current code is buggy in that the tags are set up after they are needed in pm80xx_chip_init() -> pm80xx_set_sas_protocol_timer_config(). The tag depth is earlier read in pm80xx_chip_init() -> read_main_config_table(). Add a post init callback to do the pm80xx work which needs to be done after reading the tags. I don't see a better way to do this. scsi: pm8001: Fix tag values handling The function pm8001_tag_alloc() determines free tags using the function find_first_zero_bit() which can return 0 when the first bit of the bitmap being inspected is 0. As such, tag 0 is a valid tag value that should not be dismissed as invalid. Fix the functions pm8001_work_fn(), mpi_sata_completion(), pm8001_mpi_task_abort_resp() and pm8001_open_reject_retry() to not dismiss 0 tags as invalid. scsi: pm80xx: Fix incorrect port value when registering a device During phyup event, the firmware provides the phy_id and port_id and driver is supposed to use these during device handle registration. Previously the driver was using the port id value from libsas during device handle registration. Since id can be different from the one assigned by firmware, this can lead to wrong device registration and drives not showing up. Use firmware assigned port id during device registration. scsi: pm80xx: Fix drives missing during rmmod/insmod loop When driver is loaded after rmmod some drives are not showing up during discovery. SATA drives are directly attached to the controller connected phys. During device discovery, the IDENTIFY command (qc timeout (cmd 0xec)) is timing out during revalidation. This will trigger abort from host side and controller successfully aborts the command and returns success. Post this successful abort response ATA library decides to mark the disk as NODEV. To overcome this, inside pm8001_scan_start() after phy_start() call, add get start response and wait for few milliseconds to trigger next phy start. This millisecond delay will give sufficient time for the controller state machine to accept next phy start. scsi: pm80xx: Check main config table address The driver initializes main configuration, general status, inbound queue and outbound queue table addresses based on a value read from MSGU_SCRATCH_PAD_0 register. We should validate these addresses before dereferencing them. Adds two validations: 1. Check if main configuration table offset lies within the pcibar mapped 2. Check if first dword of main configuration table reads "PMCS" There are two calls to init_pci_device_addresses() done during pm8001_pci_probe() in this sequence: 1. First inside chip_soft_rst, where if init_pci_device_addresses fails we will go ahead assuming MPI state is not ready and reset the device as long as bootloader is okay. This gives chance to second call of init_pci_device_addresses to set up the addresses after reset. 2. The second call is via pm80xx_chip_init, after soft reset is done and firmware is checked to be ready. Once that is done we are safe to go ahead and initialize default table values and use them. But in general, drivers using legacy PCI PM framework for .suspend()/.resume() have to manage many PCI PM-related tasks themselves which can be done by PCI Core itself. This brings extra load on the driver and it directly calls PCI helper functions to handle them. Switch to the new generic framework by updating function signatures and define a "struct dev_pm_ops" variable to bind PM callbacks. scsi: pm80xx: Make running_req atomic Incorrect value of the running_req was causing the driver unload to be stuck during the SAS lldd_dev_gone notification handling. During SATA I/O completion, for some error status values, the driver schedules the event handler and running_req is decremented from that. However, there are some other error status values (like IO_DS_IN_RECOVERY, IO_XFER_ERR_LAST_PIO_DATAIN_CRC_ERR) where the I/O has already been completed by fw/driver so running_req is not decremented. Also during NCQ error handling, driver itself will initiate READ_LOG_EXT and ABORT_ALL. When libsas/libata initiate READ_LOG_EXT (0x2F), driver increments running_req. This will be completed by the driver in pm80xx_chip_sata_req(), but running_req was not decremented. A global spin lock is used to control the shared access. This can create a lock contention and it is real bottleneck in the I/O path. To avoid this, the number of supported Inbound and Outbound queues is increased to 64, and the number of queues used is decided based on number of CPU cores online and number of MSI-X vectors allocated. The non-fatal error is an error condition or abnormal behavior detected by the host, or detected and reported by the controller to the host.The non-fatal error does not stop the controller firmware and enables it to still respond to host requests. A typical example of a non-fatal error is an I/O timeout or an unusual error notification from the controller. This assertion shows up in two ways, the controller doesn't show up as ready and an interrupt is waiting as soon as the handler is registered. scsi: pm80xx: Increase request sg length Increasing the per-request size maximum (max_sectors_kb) runs into the per-device DMA scatter gather list limit (max_segments) for users of the io vector system calls (eg, readv and writev). This is because the kernel combines io vectors into DMA segments when possible, but it doesn't work for our user because the vectors in the buffer cache get scrambled. This change bumps the advertised max scatter gather length to 528 to cover 2M w/ x86's 4k pages and some extra for the user checksum. It trims the size of some of the tables we don't care about and exposes all of the command slots upstream to the SCSI layer. Also reduced the PM8001_MAX_CCB to 256 as pm8001 driver has memory limit depend on machine capability. If we increase the sg length, we need to trade-off it by decreasing PM8001_MAX_CCB. Change the implementation to check for in_compat_syscall() in order to do both cases correctly, and change all callers to use it as both native and compat callback pointers, including the indirect callers through sas_ioctl and ata_scsi_ioctl. pm80xx: remove the SCSI host before detaching from SAS transport Previously, when this module was unloaded via 'rmmod' with at least one drive attached, the SCSI error handler thread would become stuck in an infinite recovery loop and lockup the system, necessitating a reboot. Once the SAS layer is detached, the driver will fail any subsequent commands since the target devices are removed. However, removing the SCSI host generates a SYNCHRONIZE CACHE (10) command, which was failed and left the error handler no method of recovery. This patch simply removes the SCSI host first so that no more commands can come down, prior to cleaning up the SAS layer. Note that the stack is built up with the SCSI host first, and then the SAS layer. Perhaps it should be reversed for symmetry, so that commands cannot be sent to the pm80xx driver prior to attaching the SAS layer? What was really strange about this bug was that it was introduced at commit cff549e4860f ("[SCSI]: proper state checking and module refcount handling in scsi_device_get"). This commit appears to tinker with how the reference counting is performed for SCSI device objects. My theory is that prior to this commit, the refcount for a device object was blindly incremented at some point during the teardown process which coincidentially made the device stick around during the procedure, which also coincidentially made any commands sent to the driver not fail (since the device was technically still "there"). After this commit was applied, my theory is the refcount for the device object is not being incremented at a specific point anymore, which makes the device go away, and thus made the pm80xx driver fail any subsequent commands. pm80xx: avoid a panic if MSI(X) interrupts are disabled If MSI(X) interrupts are disabled via the kernel command line (pci=nomsi), the pm8001 driver will kernel panic because it does not detect that MSI interrupts are disabled and will soldier on and attempt to configure MSI interrupts anyways. This leads to a kernel panic, most likely because a required data structure is not available down the line. Using the pci_msi_enabled() function in order to detect if MSI interrupts are enabled before configuring them resolves this issue and avoids a kernel panic when the module is loaded. Additionally, the irq_vector structure must be initialized when legacy interrupts are being used otherwise legacy interrupts will simply not function and result in another panic. pm80xx: configure PHY settings based on subsystem vendor ID Previuosly, all PMC Sierra 80xx controllers are assumed to be a motherboard controller, except if the subsystem vendor ID was equal to PCI_VENDOR_ID_ADAPTEC. The driver then attempts to load PHY settings from NVRAM. While this may be correct behavior for most controllers, it does not work with Adaptec and ATTO controllers since they do not store PHY settings in NVRAM and choose to use either custom PHY settings or chip defaults. Loading random values from NVRAM may cause the controllers to malfunction in this edge case. libsas: remove task_collector mode The task_collector mode (or "latency_injector", (C) Dan Willians) is an optional I/O path in libsas that queues up scsi commands instead of directly sending it to the hardware. It generall increases latencies to in the optiomal case slightly reduce mmio traffic to the hardware. Only the obsolete aic94xx driver and the mvsas driver allowed to use it without recompiling the kernel, and most drivers didn't support it at all. Remove the giant blob of code to allow better optimizations for scsi-mq in the future. After increasing the CAN_QUEUE to 510 in pm8001 we discovered some performance degredation from time to time. We needed to increase the MPI queue to compensate and ensure we never hit that limit. We also needed to double the margin to support event and administrivial commands that take from the pool resulting in an occasional largely unproductive command completion with soft error to the caller when the command pool is overloaded temporarily. [SCSI] libsas: async ata scanning libsas ata error handling is already async but this does not help the scan case. Move initial link recovery out from under host->scan_mutex, and delay synchronization with eh until after all port probe/recovery work has been queued. Device ordering is maintained with scan order by still calling sas_rphy_add() in order of domain discovery. Since we now scan the domain list when invoking libata-eh we need to be careful to check for fully initialized ata ports. [SCSI] pm8001: simplify workqueue usage pm8001 manages its own list of pending works and cancel them on device free. It is unnecessarily complex and has a race condition - the works are canceled but not synced, so the work could still be running during and after the data structures are freed. This patch simplifies workqueue usage. * A driver specific workqueue pm8001_wq is created to serve these work items. * To avoid confusion, the "queue" suffixes are dropped from work items and functions. * Delayed queueing was never used. pm8001_work now uses work_struct instead. * The driver no longer keeps track of pending works. All pm8001_works are queued to pm8001_wq and the workqueue is flushed as necessary. flush_scheduled_work() usage is removed during conversion. include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h percpu.h is included by sched.h and module.h and thus ends up being included when building most .c files. percpu.h includes slab.h which in turn includes gfp.h making everything defined by the two files universally available and complicating inclusion dependencies. percpu.h -> slab.h dependency is about to be removed. Prepare for this change by updating users of gfp and slab facilities include those headers directly instead of assuming availability. As this conversion needs to touch large number of source files, the following script is used as the basis of conversion. http://userweb.kernel.org/~tj/misc/slabh-sweep.py The script does the followings. * Scan files for gfp and slab usages and update includes such that only the necessary includes are there. ie. if only gfp is used, gfp.h, if slab is used, slab.h. * When the script inserts a new include, it looks at the include blocks and try to put the new include such that its order conforms to its surrounding. It's put in the include block which contains core kernel includes, in the same order that the rest are ordered - alphabetical, Christmas tree, rev-Xmas-tree or at the end if there doesn't seem to be any matching order. * If the script can't find a place to put a new include (mostly because the file doesn't have fitting include block), it prints out an error message indicating which .h file needs to be added to the file. The conversion was done in the following steps. 1. The initial automatic conversion of all .c files updated slightly over 4000 files, deleting around 700 includes and adding ~480 gfp.h and ~3000 slab.h inclusions. The script emitted errors for ~400 files. 2. Each error was manually checked. Some didn't need the inclusion, some needed manual addition while adding it to implementation .h or embedding .c file was more appropriate for others. This step added inclusions to around 150 files. 3. The script was run again and the output was compared to the edits from #2 to make sure no file was left behind. 4. Several build tests were done and a couple of problems were fixed. e.g. lib/decompress_*.c used malloc/free() wrappers around slab APIs requiring slab.h to be added manually. 5. The script was run on all .h files but without automatically editing them as sprinkling gfp.h and slab.h inclusions around .h files could easily lead to inclusion dependency hell. Most gfp.h inclusion directives were ignored as stuff from gfp.h was usually wildly available and often used in preprocessor macros. Each slab.h inclusion directive was examined and added manually as necessary. 6. percpu.h was updated not to include slab.h. 7. Build test were done on the following configurations and failures were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my distributed build env didn't work with gcov compiles) and a few more options had to be turned off depending on archs to make things build (like ipr on powerpc/64 which failed due to missing writeq). * x86 and x86_64 UP and SMP allmodconfig and a custom test config. * powerpc and powerpc64 SMP allmodconfig * sparc and sparc64 SMP allmodconfig * ia64 SMP allmodconfig * s390 SMP allmodconfig * alpha SMP allmodconfig * um on x86_64 SMP allmodconfig 8. percpu.h modifications were reverted so that it could be applied as a separate patch and serve as bisection point. Given the fact that I had only a couple of failures from tests on step 6, I'm fairly confident about the coverage of this conversion patch. If there is a breakage, it's likely to be something in one of the arch headers which should be easily discoverable easily on most builds of the specific arch.
