scsi: csiostor: Replace deprecated strncpy() with strscpy()

strncpy() is deprecated for use on NUL-terminated destination strings [1]
and as such we should prefer more robust and less ambiguous string
interfaces.

'hw' is kzalloc'd just before this string assignment:
| hw = kzalloc(sizeof(struct csio_hw), GFP_KERNEL);

... which means any NUL-padding is redundant.

Since CSIO_DRV_VERSION is a small string literal (smaller than
sizeof(dest)):

... there is functionally no change in this swap from strncpy() to
strscpy(). Nonetheless, let's make the change for robustness' sake -- as
it will ensure that drv_version is _always_ NUL-terminated.

scsi: csiostor: Add module softdep on cxgb4

Both cxgb4 and csiostor drivers run on their own independent Physical
Function. But when cxgb4 and csiostor are both being loaded in parallel via
modprobe, there is a race when firmware upgrade is attempted by both the
drivers.

When the cxgb4 driver initiates the firmware upgrade, it halts the firmware
and the chip until upgrade is complete. When the csiostor driver is coming
up in parallel, the firmware mailbox communication fails with timeouts and
the csiostor driver probe fails.

Add a module soft dependency on cxgb4 driver to ensure loading csiostor
triggers cxgb4 to load first when available to avoid the firmware upgrade
race.

scsi: csiostor: remove unneeded DRIVER_LICENSE #define

There is no need to #define the license of the driver, just put it in
the MODULE_LICENSE() line directly as a text string.

This allows tools that check that the module license matches the source
code license to work properly, as there is no need to unwind the
unneeded dereference, especially when the string is defined in a .h file
far away from the .c file it is used in.

csiostor:Remove T4 FCoE Support.

We found a subtle issue with FCoE on T4 very late in the game
and decided not to productize FCoE on T4 and therefore there
are no customers that will be impacted by this change. Hence
T4 FCoE support is removed. FCoE supported only on T5 cards.

cxgb4: Cleanup macros so they follow the same style and look consistent

Various patches have ended up changing the style of the symbolic macros/register
to different style.

As a result, the current kernel.org files are a mix of different macro styles.
Since this macro/register defines is used by different drivers a
few patch series have ended up adding duplicate macro/register define entries
with different styles. This makes these register define/macro files a complete
mess and we want to make them clean and consistent. This patch cleans up a part
of it.

PCI: Remove DEFINE_PCI_DEVICE_TABLE macro use

We should prefer `struct pci_device_id` over `DEFINE_PCI_DEVICE_TABLE` to
meet kernel coding style guidelines. This issue was reported by checkpatch.

SCSI: remove unnecessary pci_set_drvdata()

Since commit 0998d0631001288a5974afc0b2a5f568bcdecb4d
(device-core: Ensure drvdata = NULL when no driver is bound),
the driver core clears the driver data to NULL after device_release
or on probe failure. Thus, it is not needed to manually clear the
device driver data to NULL.

Drivers: scsi: remove __dev* attributes.

CONFIG_HOTPLUG is going away as an option. As a result, the __dev*
markings need to be removed.

This change removes the use of __devinit, __devexit_p, __devinitdata,
__devinitconst, and __devexit from these drivers.

Based on patches originally written by Bill Pemberton, but redone by me
in order to handle some of the coding style issues better, by hand.
