phy: constify of_phandle_args in xlate

The xlate callbacks are supposed to translate of_phandle_args to proper
provider without modifying the of_phandle_args. Make the argument
pointer to const for code safety and readability.

phy: Explicitly include correct DT includes

The DT of_device.h and of_platform.h date back to the separate
of_platform_bus_type before it as merged into the regular platform bus.
As part of that merge prepping Arm DT support 13 years ago, they
"temporarily" include each other. They also include platform_device.h
and of.h. As a result, there's a pretty much random mix of those include
files used throughout the tree. In order to detangle these headers and
replace the implicit includes with struct declarations, users need to
explicitly include the correct includes.

phy: xgene: make array serdes_reg static const, makes object smaller

Don't populate the array serdes_reg on the stack but instead make it
static const. Makes the object code smaller by 228 bytes.

phy: core: Let node ptr of PHY point to PHY and not of PHY provider

In case of multi-phy PHY providers, each PHY should be modeled as a sub
node of the PHY provider. Then each PHY will have a different node pointer
(node pointer of sub node) than that of PHY provider. Added this provision
in the PHY core.
Also fixed all drivers to use the updated API.

This patch adds support for the APM X-Gene SoC 15Gbps Multi-purpose PHY.
This is the physical layer interface for the corresponding host
controller. Currently, only external clock and Gen3 SATA mode
are supported.
