iwlwifi: remove contact information

The list address is going away, and the postal address isn't
useful, remove all the contact information.

iwlwifi: remove flags argument for nic_access

Since we no longer save interrupts, we no longer need the flags
argument here, remove it throughout.

iwlwifi: dvm: remove useless condition

If we're in the else branch of checking "tt->state == IWL_TI_CT_KILL"

iwlwifi: remove all occurrences of the FSF address paragraph

The Free Software Foundation address is superfluous and causes
checkpatch to issue a warning when present. Remove all paragraphs
with FSF's address to prevent that.

# 86cb30ec 17-Oct-2017 Kees Cook <keescook@chromium.org>

treewide: setup_timer() -> timer_setup() (2 field)

This converts all remaining setup_timer() calls that use a nested field
to reach a struct timer_list. Coccinelle does not have an easy way to
match multiple fields, so a new script is needed to change the matches of
"&_E->_timer" into "&_E->_field1._timer" in all the rules.
