b43: fix a lower bounds test

The problem is that "channel" is an unsigned int, when it's less 5 the
value of "channel - 5" is not a negative number as one would expect but
is very high positive value instead.

This means that "start" becomes a very high positive value. The result
of that is that we never enter the "for (i = start; i <= end; i++) {"
loop. Instead of storing the result from b43legacy_radio_aci_detect()
it just uses zero.

b43: Replace HTTP links with HTTPS ones

Rationale:
Reduces attack surface on kernel devs opening the links for MITM
as HTTPS traffic is much harder to manipulate.

Deterministic algorithm:
For each file:
If not .svg:
For each line:
If doesn't contain `\bxmlns\b`:
For each link, `\bhttp://[^# \t\r\n]*(?:\w|/)`:
If neither `\bgnu\.org/license`, nor `\bmozilla\.org/MPL\b`:
If both the HTTP and HTTPS versions
return 200 OK and serve the same content:
Replace HTTP with HTTPS.

b43: fix unitialized reads of ret by initializing the array to zero

The u8 char array ret is not being initialized and elements outside
the range start to end contain just garbage values from the stack.
This results in a later scan of the array to read potentially
uninitialized values. Fix this by initializing the array to zero.
This seems to have been an issue since the very first commit.

b43: Remove unused phy_a code

gcc-6 reports the following error with -Werror=unused-const-variable.

drivers/net/wireless/broadcom/b43/phy_a.c:576:40: error:
'b43_phyops_a' defined but not used

Per Michael Büsch: "All a-phy code is usused", so remove it all,
and move the remaining Type-G initialization code into phy_g.c.
