ath9k: Update AR9462 initvals

HW peak detect calibration would fail for AR9300 chips and
we went for implementing the SW way of doing it instead of
HW doing the peak detect calibration.

ath9k: Fix ETSI compliance for AR9462 2.0

The minimum CCA power threshold values have to be adjusted
for existing cards to be in compliance with new regulations.
Newer cards will make use of the values obtained from EEPROM,
support for this was added earlier. To make sure that cards
that are already in use and don't have proper values in EEPROM,
do not violate regulations, use the initvals instead.

to reduce module size.

ath9k_hw: Fix wrong peak detector DC offset

An issue is reported in AR9462 & AR9565 that NF_cal_not_done is
not observed when HW peak detector calibration is disabled. At that
state, the HW is stuck at NF calibration which prevents tx output.
The root cause is wrong peak detector offset calibrated by HW. To
resolve this issue, peak detector calibration is done manually by SW
for AR9462 and AR9565.

ath9k: de-duplicate initvals

The initvals tool from https://github.com/mcgrof/qca-swiss-army-knife has
been modified to detect identical initval tables and replace them with
macros. This patch contains the generated changes.

On MIPS this reduces the binary size by 24 KB with no runtime changes.

ath9k_hw: Initvals update for AR9462

MSI is enabled by default for most of the 4th generation
chips. Add this for AR9462 - this fixes PowerSave operation,
the chip was not entering Network-Sleep mode earlier.
With proper powering down of the MAC now, power consumption
in associated state is reduced considerably.

ath9k_hw: Remove BTCOEX initvals

The MAX_TXPWR table for BTCOEX is not needed for AR9462.
Programming these values to the HW results in undesirable
behavior - for example, large number of delimiter/data
underruns are seen in AES-CCMP mode. Also, registers like
AR_PCU_MISC_MODE2 return 0xdeadbeef after the BTCOEX_MAX
power table is programmed to the HW, and frames being transmitted
end up being looped back to the RX engine, an example being beacon
frames in IBSS mode.

Remove this table for now - this fixes CCMP performance and general
IBSS usage.

The AR946/8x chips are 2x2 Dual band with BT support. In order
to avoid misleading with other chips and to be in sync with
marketing team's term, AR9480 is renamed as AR9462.
