i40e: Move memory allocation structures to i40e_alloc.h

Structures i40e_dma_mem & i40e_virt_mem are defined i40e_osdep.h while
memory allocation functions that use them are declared in i40e_alloc.h
Move them there.

i40e: Simplify memory allocation functions

Enum i40e_memory_type enum is unused in i40e_allocate_dma_mem() thus
can be safely removed. Useless macros in i40e_alloc.h can be removed
as well.

i40e: use int for i40e_status

To prepare for removal of i40e_status, change the variables
from i40e_status to int. This eases the transition when values
are changed to return standard int error codes over enum i40e_status.

As such changes often also change variable orders, a cleanup
is also applied here to make variables conform to RCT and
some lines are also reformatted where applicable.

net: intel: Cleanup the copyright/license headers

After many years of having a ~30 line copyright and license header to our
source files, we are finally able to reduce that to one line with the
advent of the SPDX identifier.

Also caught a few files missing the SPDX license identifier, so fixed
them up.

i40e: Fix GPL header

is already part of the larger kernel.
