mmc: omap: restore original power up/down steps

Commit e519f0bb64ef ("ARM/mmc: Convert old mmci-omap to GPIO descriptors")
moved Nokia N810 MMC power up/down from the board file into the MMC driver.

The change removed some delays, and ordering without a valid reason.
Restore power up/down to match the original code. This matters only on N810
where the 2nd GPIO is in use. Other boards will see an additional delay but
that should be a lesser concern than omitting delays altogether.

mmc: omap: fix broken slot switch lookup

The lookup is done before host->dev is initialized. It will always just
fail silently, and the MMC behaviour is totally unpredictable as the switch
is left in an undefined state. Fix that.

mmc: omap: Convert to platform remove callback returning void

The .remove() callback for a platform driver returns an int which makes
many driver authors wrongly assume it's possible to do error handling by
returning an error code. However the value returned is (mostly) ignored
and this typically results in resource leaks. To improve here there is a
quest to make the remove callback return void. In the first step of this
quest all drivers are converted to .remove_new() which already returns
void.

Trivially convert this driver from always returning zero in the remove
callback to the void returning variant.

mmc: omap: fix deferred probing

The driver overrides the error codes returned by platform_get_irq() to
-ENXIO, so if it returns -EPROBE_DEFER, the driver will fail the probe
permanently instead of the deferred probing. Switch to propagating the
error codes upstream.

ARM/mmc: Convert old mmci-omap to GPIO descriptors

A recent change to the OMAP driver making it use a dynamic GPIO
base created problems with some old OMAP1 board files, among
them Nokia 770, SX1 and also the OMAP2 Nokia n8x0.

Fix up all instances of GPIOs being used for the MMC driver
by pushing the handling of power, slot selection and MMC
"cover" into the driver as optional GPIOs.

This is maybe not the most perfect solution as the MMC
framework have some central handlers for some of the
stuff, but it at least makes the situtation better and
solves the immediate issue.

mmc: omap: Make it CCF clk API compatible

The driver, OMAP specific, now omits clk_prepare/unprepare() steps, not
supported by OMAP custom implementation of clock API. However, non-CCF
stubs of those functions exist for use on such platforms until converted
to CCF.

Update the driver to be compatible with CCF implementation of clock API.

mmc: host: Drop redundant MMC_CAP_ERASE

The MMC_CAP_ERASE bit is no longer used by the mmc core as erase, discard
and trim operations are now always supported. Therefore, drop the bit and

mmc: omap: fix the maximum timeout setting

When running OMAP1 kernel on QEMU, MMC access is annoyingly noisy:

MMC: CTO of 0xff and 0xfe cannot be used!
MMC: CTO of 0xff and 0xfe cannot be used!

Emulator warnings appear to be valid. The TI document SPRU680 [1]
("OMAP5910 Dual-Core Processor MultiMedia Card/Secure Data Memory Card
(MMC/SD) Reference Guide") page 36 states that the maximum timeout is 253
cycles and "0xff and 0xfe cannot be used".

Fix by using 0xfd as the maximum timeout.

Tested using QEMU 2.5 (Siemens SX1 machine, OMAP310), and also checked on
real hardware using Palm TE (OMAP310), Nokia 770 (OMAP1710) and Nokia N810
(OMAP2420) that MMC works as before.

MMC: OMAP: fix broken MMC on OMAP15XX/OMAP5910/OMAP310

Since v2.6.22 or so there has been reports [1] about OMAP MMC being
broken on OMAP15XX based hardware (OMAP5910 and OMAP310). The breakage
seems to have been caused by commit 46a6730e3ff9 ("mmc-omap: Fix
omap to use MMC_POWER_ON") that changed clock enabling to be done
on MMC_POWER_ON. This can happen multiple times in a row, and on 15XX
the hardware doesn't seem to like it and the MMC just stops responding.
Fix by memorizing the power mode and do the init only when necessary.

mmc: omap: Don't use mmc_card_present() when validating for inserted card

The mmc_card_present() function helps the mmc core to track an internal
state of the card device. More importantly, it's not intended to be used by
mmc host drivers to check for an inserted card. Therefore, let's stop using
it and instead rely on checking for a valid pointer to a struct mmc_card,
as it should be good enough.

With the new dma_request_chan() the client driver does not need to look for
the DMA resource and it does not need to pass filter_fn anymore.
By switching to the new API the driver can now support deferred probing

Reason for this is that omap_notify_cover_event which calls
tasklet_hi_schedule gets called before struct cover_tasklet is initialized.

Call to omap_notify_cover_event on Nokia N810 happens from menelaus.c PMIC
driver via board-n8x0.c during execution of mmc_add_host in case of open
miniSD cover.

Fix this by moving cover_timer and cover_tasklet initialization before
mmc_add_host call in mmc_omap_new_slot.

mmc: omap: Fix I2C dependency and make driver usable with device tree

Some features can be configured by the companion I2C chips,
which may not be available at the probe time. Fix the issue
by returning -EPROBE_DEFER when the MMC controller slots
are not configured.

While at it, let's also add minimal device tree support so
omap24xx platforms can use this driver without legacy mode
since we claim to support device tree for mach-omap2 based
systems.

Although adding the minimal device tree support is not strictly
a fix, it does remove one of the last blockers for dropping a
bunch of legacy platform data for mach-omap2.

This turned out to be due to memory corruption caused by long-broken
PIO code in drivers/mmc/host/omap.c. (Previously, this driver had
been using DMA; but the above commit caused the MMC driver to fall
back to PIO mode with an unmodified Kconfig.)

The PIO code, added with the rest of the driver in commit
730c9b7e6630f786fcec026fb11d2e6f2c90fdcb ("[MMC] Add OMAP MMC host
driver"), confused bytes with 16-bit words. This bug caused memory
located after the PIO transfer buffer to be corrupted with transfers
larger than 32 bytes. The driver also did not increment the buffer
pointer after the transfer occurred. This bug resulted in data
corruption during any transfer larger than 64 bytes.

Commit fa550189 (mmc: core: Prevent eMMC VCC supply to be cut from late init)
slightly affected timings for how things are done for the omap MMC driver

MMC driver that was happening after mmc_add_host() was being called.

Fix this by initializing reg_shift before mmc_add_host() is called.

* host/omap.c: Create, use and flush mmc_omap_wq instead of the
system_wq.

* Flush host->mmc_carddetect_work directly on removal instead of using
flush_scheduled_work().

omap: remove BUG_ON for disabled interrupts

Remove a BUG_ON for when interrupts are disabled during an MMC request.

During boot, interrupts can be disabled when a request is made, causing
this bug to be triggered. In reality, there's no reason this should halt
the kernel, as the driver has proved reliable in spite of disabled
interrupts, and additionally, there's nothing in this code that would
require interrupts to be enabled.

The only setup I've managed to make it trigger on is on the HTC Herald
during bootup when the driver is built into the kernel (mostly because
that's all I have). I believe it's related to the fact that on bootup I
get many timeout errors on "CMD5" while initializing the card. Each CMD5
timeout triggers that bug (I changed it to a WARN_ON to get it to boot in)
due to the fact that part of the timeout code involves sending the request
again. With interrupts turned off, that BUG would be triggered.

mmc: remove the "state" argument to mmc_suspend_host()

Even though many mmc host drivers pass a pm_message_t argument to
mmc_suspend_host() that argument isn't used the by MMC core. As host
drivers are converted to dev_pm_ops they'll have to construct
pm_message_t's (as they won't be passed by the PM subsystem any more) just
to appease the mmc suspend interface.

We might as well just delete the unused paramter.

mmc-omap: add support for 16-bit and 32-bit registers

The omap850 and omap730 use 16-bit registers instead of 32-bit, requiring
a modification of the register addresses in the mmc-omap driver. To
resolve this, a bit shift is performed on base register addresses, either
by 1 or 2 bits depending on the CPU in use. This yields the correct
registers for each CPU.

include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.

2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).

* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

mmc/omap: make mmci-omap using platform_driver_probe

A pointer to mmc_omap_probe which lives in .init.text is passed to the
core via platform_driver_register and so the kernel might oops if probe
is called after the init code is discarded.

As requested by David Brownell platform_driver_probe is used instead of
moving the probe function to .devinit.text. This saves some memory, but
might have the downside that a device being registered after the call to
mmc_omap_init but before the init sections are discarded will not be
bound anymore to the driver.

disable_irq() should wait for all running handlers to complete
before returning. As such, if it's used to disable an interrupt
from that interrupt's handler it will deadlock. This replaces

have this problem.

Relax requirements on host controllers and only require that they do not
report a transfer count than is larger than the actual one (i.e. a lower
value is okay). This is how many other parts of the kernel behaves so
upper layers should already be prepared to handle that scenario. This
gives us a performance boost on MMC cards.

This same issue should not affect other drivers as they don't seem
to use dev_name with dev_dbg.

The limit was a fixed 100k limit in the busy loop, which is not
accurate. It would better to have time limit for the worst case
which occurs when sending 80 cycles at 400 kHz and takes about
200 microseconds, so limit the max time spend in the busy loop
for some 250 microseconds.

The cover waitqueue is occasionally scheduled twice from timer
and the interrupt and oops follows. It would have been possible

MMC: OMAP: Abort stuck commands

When a card is removed while it is being accessed, a command can get stuck so
that no timeout or end of command interrupt ever occurs. The command getting
stuck is almost always CDM12, but also the other commands can get stuck. Catch
a stuck command with a timer and try sending the initialization stream until
the controller starts running again and responds with the end of command
status.

from Linux-OMAP tree, the BYTEBLOCK capability was removed by Pierre Ossman.
MMC_CAP_BYTEBLOCK is not defined causing the compile error:

mmc: fix platform driver hotplug/coldplug

Since 43cc71eed1250755986da4c0f9898f9a635cb3bf, the platform modalias is
prefixed with "platform:". Add MODULE_ALIAS() to the hotpluggable MMC host
platform drivers, to re-enable auto loading.

Also, add missing owner declarations in driver init.
