IB/IPoIB: Fix queue count inconsistency for PKEY child interfaces

There are 2 ways to create IPoIB PKEY child interfaces:
1) Writing a PKEY to /sys/class/net/<ib parent interface>/create_child.
2) Using netlink with iproute.

While with sysfs the child interface has the same number of tx and
rx queues as the parent, with netlink there will always be 1 tx
and 1 rx queue for the child interface. That's because the
get_num_tx/rx_queues() netlink ops are missing and the default value
of 1 is taken for the number of queues (in rtnl_create_link()).

This change adds the get_num_tx/rx_queues() ops which allows for
interfaces with multiple queues to be created over netlink. This
constant only represents the max number of tx and rx queues on that
net device.

RDMA/ipoib: Fix warning caused by destroying non-initial netns

After the commit 5ce2dced8e95 ("RDMA/ipoib: Set rtnl_link_ops for ipoib
interfaces"), if the IPoIB device is moved to non-initial netns,
destroying that netns lets the device vanish instead of moving it back to
the initial netns, This is happening because default_device_exit() skips
the interfaces due to having rtnl_link_ops set.

Steps to reporoduce:

ip netns delete foo

To avoid the above warning and later on the kernel panic that could happen
on shutdown due to a NULL pointer dereference, make sure to set the
netns_refund flag that was introduced by commit 3a5ca857079e ("can: dev:
Move device back to init netns on owning netns delete") to properly
restore the IPoIB interfaces to the initial netns.

RDMA/ipoib: Set rtnl_link_ops for ipoib interfaces

Report the "ipoib pkey", "mode" and "umcast" netlink attributes for every
IPoiB interface type, not just children created with 'ip link add'.

After setting the rtnl_link_ops for the parent interface, implement the
dellink() callback to block users from trying to remove it.

RDMA/netdev: Fix netlink support in IPoIB

IPoIB netlink support was broken by the below commit since integrating
the rdma_netdev support relies on an allocation flow for netdevs that
was controlled by the ipoib driver while netdev's rtnl_newlink
implementation assumes that the netdev will be allocated by netlink.
Such situation leads to crash in __ipoib_device_add, once trying to
reuse netlink device.

This patch fixes the kernel oops for both mlx4 and mlx5
devices triggered by the following command:

IB/ipoib: Consolidate checking of the proposed child interface

Move all the checking for pkey and other validity to the __ipoib_vlan_add
function. This removes the last difference from the control flow
of the __ipoib_vlan_add to make the overall design simpler to
understand.

IB/ipoib: Maintain the child_intfs list from ndo_init/uninit

This fixes a bug in the netlink path where the vlan_rwsem was not
held around __ipoib_vlan_add causing the child_intfs to be manipulated
unsafely.

In the process this greatly simplifies the vlan_rwsem write side locking
to only cover a single non-sleeping statement.

This also further increases the safety of the removal ordering by holding
the netdev of the parent while the child is active to ensure most bugs
become either an oops on a NULL priv or a deadlock on the netdev refcount.

IB/ipoib: Move init code to ndo_init

Now that we have a proper ndo_uninit, move code that naturally pairs
with the ndo_uninit into ndo_init. This allows the netdev core to natually
handle ordering.

This fixes the situation where register_netdev can fail before calling
ndo_init, in which case it wouldn't call ndo_uninit either.

Also move a bunch of duplicated init code that is shared between child
and parent for clarity. Now the child and parent register functions look
very similar.

IB/IPoIB: Support acceleration options callbacks

IPoIB driver now uses the new set of callback functions.

If the hardware provider supports the new ipoib_options implementation,
the driver uses the callbacks in its data path flows, otherwise it uses the
driver default implementation for all data flows in its code.

The default implementation wasn't change and it is exactly as it was before
introduction of acceleration support.

infiniband: make sure the src net is infiniband when create new link

When we create a new infiniband link with uninfiniband device, e.g. `ip link
add link em1 type ipoib pkey 0x8001`. We will get a NULL pointer dereference
cause other dev like Ethernet don't have struct ib_device.

Fix this bug by make sure the src net is infiniband when create new link.

IPoIB: Fix deadlock between dev_change_flags() and __ipoib_dev_flush()

When ipoib interface is going down it takes all of its children with
it, under mutex.

For each child, dev_change_flags() is called. That function calls
ipoib_stop() via the ndo, and causes flush of the workqueue.
Sometimes in the workqueue an __ipoib_dev_flush work() is waiting and
when invoked tries to get the same mutex, which leads to a deadlock,
as seen below.

The solution is to switch to rw-sem instead of mutex.

IPoIB: Make sure child devices use valid/proper pkeys

Make sure that the IB invalid pkey (0x0000 or 0x8000) isn't used for
child devices.

Also, make sure to always set the full membership bit for the pkey of
devices created by rtnl link ops.

IB/ipoib: Add more rtnl_link_ops callbacks

Add the rtnl_link_ops changelink and fill_info callbacks, through
which the admin can now set/get the driver mode, etc policies.
Maintain the proprietary sysfs entries only for legacy childs.

For child devices, set dev->iflink to point to the parent
device ifindex, such that user space tools can now correctly
show the uplink relation as done for vlan, macvlan, etc

IB/ipoib: Add rtnl_link_ops support

Add rtnl_link_ops to IPoIB, with the first usage being child device
create/delete through them. Childs devices are now either legacy ones,
created/deleted through the ipoib sysfs entries, or RTNL ones.

Adding support for RTNL childs involved refactoring of ipoib_vlan_add
which is now used by both the sysfs and the link_ops code.

Also, added ndo_uninit entry to support calling unregister_netdevice_queue
from the rtnl dellink entry. This required removal of calls to
ipoib_dev_cleanup from the driver in flows which use unregister_netdevice,
since the networking core will invoke ipoib_uninit which does exactly that.
