remove ioremap_nocache and devm_ioremap_nocache

ioremap has provided non-cached semantics by default since the Linux 2.6
days, so remove the additional ioremap_nocache interface.

bcma: keep a direct pointer to the struct device

Accessing struct device is pretty useful/common so having a direct
pointer:
1) Simplifies some code
2) Makes bcma_bus_get_host_dev() unneeded
3) Allows further improvements like using dev_* printing helpers

bcma: use standard bus scanning during early register

Starting with kernel 3.19-rc1 early registration of bcma on MIPS is done
a bit later, with memory allocator available. This allows us to simplify
code by using standard bus scanning method.

bcma: register bcma as device tree driver

This driver is used by the bcm53xx ARM SoC code. Now it is possible to
give the address of the chipcommon core in device tree and bcma will
search for all the other cores.

bcma: use separated function to initialize bus on SoC

This is required to split SoC bus init into two phases. The later one
(which includes scanning) should be called when kalloc is available.

bcma: move bus struct setup into early part of host specific code

This change is important for SoC host. In future we will want to know
chip ID (needed for early MIPS boot) before doing cores scanning.

bcma: make some functions static

The functions and structs are not used in an other file and the
prototypes are in no header file, just make them static so the compiler
is able to optimize them better.

bcma: add SOC bus

This patch adds support for using bcma on a Broadcom SoC as the system
bus. An SoC like the bcm4716 could register this bus and use it to
searches for the bcma cores and register the devices on this bus.

BCMA_HOSTTYPE_NONE was intended for SoCs at first but BCMA_HOSTTYPE_SOC
is a better name.
