KVM: x86: Unify pr_fmt to use module name for all KVM modules Define pr_fmt using KBUILD_MODNAME for all KVM x86 code so that printks use consistent formatting across common x86, Intel, and AMD code. In addition to providing consistent print formatting, using KBUILD_MODNAME, e.g. kvm_amd and kvm_intel, allows referencing SVM and VMX (and SEV and SGX and ...) as technologies without generating weird messages, and without causing naming conflicts with other kernel code, e.g. "SEV: ", "tdx: ", "sgx: " etc.. are all used by the kernel for non-KVM subsystems. Opportunistically move away from printk() for prints that need to be modified anyways, e.g. to drop a manual "kvm: " prefix. Opportunistically convert a few SGX WARNs that are similarly modified to WARN_ONCE; in the very unlikely event that the WARNs fire, odds are good that they would fire repeatedly and spam the kernel log without providing unique information in each print. Note, defining pr_fmt yields undesirable results for code that uses KVM's printk wrappers, e.g. vcpu_unimpl(). But, that's a pre-existing problem as SVM/kvm_amd already defines a pr_fmt, and thankfully use of KVM's wrappers is relatively limited in KVM x86 code. KVM: x86: PIT: Preserve state of speaker port data bit Currently the state of the speaker port (0x61) data bit (bit 1) is not saved in the exported state (kvm_pit_state2) and hence is lost when re-constructing guest state. This patch removes the 'speaker_data_port' field from kvm_kpit_state and instead tracks the state using a new KVM_PIT_FLAGS_SPEAKER_DATA_ON flag defined in the API. KVM: x86: Add wrappers for setting/clearing APICv inhibits Add set/clear wrappers for toggling APICv inhibits to make the call sites more readable, and opportunistically rename the inner helpers to align with the new wrappers and to make them more readable as well. Invert the flag from "activate" to "set"; activate is painfully ambiguous as it's not obvious if the inhibit is being activated, or if APICv is being activated, in which case the inhibit is being deactivated. For the functions that take @set, swap the order of the inhibit reason and @set so that the call sites are visually similar to those that bounce through the wrapper. KVM: x86: Migrate the PIT only if vcpu0 is migrated, not any BSP Make vcpu0 the arbitrary owner of the PIT, as was intended when PIT migration was added by commit 2f5997140f22 ("KVM: migrate PIT timer"). The PIT was unintentionally turned into being owned by the BSP by commit c5af89b68abb ("KVM: Introduce kvm_vcpu_is_bsp() function."), and was then unintentionally converted to a shared ownership model when kvm_vcpu_is_bsp() was modified to check the APIC base MSR instead of hardcoding vcpu0 as the BSP. Functionally, this just means the PIT's hrtimer is migrated less often. The real motivation is to remove the usage of kvm_vcpu_is_bsp(), so that more legacy/broken crud can be removed in a future patch. kvm: x86: Add memcg accounting to KVM allocations There are many KVM kernel memory allocations which are tied to the life of the VM process and should be charged to the VM process's cgroup. If the allocations aren't tied to the process, the OOM killer will not know that killing the process will free the associated kernel memory. Add __GFP_ACCOUNT flags to many of the allocations which are not yet being charged to the VM process's cgroup. Tested: Ran all kvm-unit-tests on a 64 bit Haswell machine, the patch introduced no new failures. Ran a kernel memory accounting test which creates a VM to touch memory and then checks that the kernel memory allocated for the process is within certain bounds. With this patch we account for much more of the vmalloc and slab memory allocated for the VM. There remain a few allocations which should be charged to the VM's cgroup but are not. In x86, they include: vcpu->arch.pio_data There allocations are unaccounted in this patch because they are mapped to userspace, and accounting them to a cgroup causes problems. KVM: x86: Handle the kthread worker using the new API Use the new API to create and destroy the "kvm-pit" kthread worker. The API hides some implementation details. In particular, kthread_create_worker() allocates and initializes struct kthread_worker. It runs the kthread the right way and stores task_struct into the worker structure. kthread_destroy_worker() flushes all pending works, stops the kthread and frees the structure. This patch does not change the existing behavior except for dynamically allocating struct kthread_worker and storing only the pointer of this structure. It is compile tested only because I did not find an easy way how to run the code. Well, it should be pretty safe given the nature of the change. kthread: kthread worker API cleanup A good practice is to prefix the names of functions by the name of the subsystem. The kthread worker API is a mix of classic kthreads and workqueues. Each worker has a dedicated kthread. It runs a generic function that process queued works. It is implemented as part of the kthread subsystem. This patch renames the existing kthread worker API to use the corresponding name from the workqueues API prefixed by kthread_: __init_kthread_worker() -> __kthread_init_worker() init_kthread_worker() -> kthread_init_worker() init_kthread_work() -> kthread_init_work() insert_kthread_work() -> kthread_insert_work() queue_kthread_work() -> kthread_queue_work() flush_kthread_work() -> kthread_flush_work() flush_kthread_worker() -> kthread_flush_worker() Note that the names of DEFINE_KTHREAD_WORK*() macros stay as they are. It is common that the "DEFINE_" prefix has precedence over the subsystem names. Note that INIT() macros and init() functions use different naming scheme. There is no good solution. There are several reasons for this solution: + "init" in the function names stands for the verb "initialize" aka "initialize worker". While "INIT" in the macro names stands for the noun "INITIALIZER" aka "worker initializer". + INIT() macros are used only in DEFINE() macros + init() functions are used close to the other kthread() functions. It looks much better if all the functions use the same scheme. + There will be also kthread_destroy_worker() that will be used close to kthread_cancel_work(). It is related to the init() function. Again it looks better if all functions use the same naming scheme. + there are several precedents for such init() function names, e.g. amd_iommu_init_device(), free_area_init_node(), jump_label_init_type(), regmap_init_mmio_clk(), + It is not an argument but it was inconsistent even before. KVM: i8254: remove unnecessary uses of PIT state lock - kvm_create_pit had to lock only because it exposed kvm->arch.vpit very early, but initialization doesn't use kvm->arch.vpit since the last patch, so we can drop locking. - kvm_free_pit is only run after there are no users of KVM and therefore is the sole actor. - Locking in kvm_vm_ioctl_reinject doesn't do anything, because reinject is only protected at that place. - kvm_pit_reset isn't used anywhere and its locking can be dropped if we hide it. Removing useless locking allows to see what actually is being protected by PIT state lock (values accessible from the guest). A mb() was added in kvm_pit_ack_irq, to pair with implicit barrier between pit_timer_fn and pit_do_work. The mb() prevents a race that could happen if pending == 0 and irq_ack == 0: kvm_pit_ack_irq: | pit_timer_fn: p = atomic_read(&ps->pending); | | atomic_inc(&ps->pending); | queue_work(pit_do_work); | pit_do_work: | atomic_xchg(&ps->irq_ack, 0); | return; atomic_set(&ps->irq_ack, 1); | if (p == 0) return; | where the interrupt would not be delivered in this tick of pit_timer_fn. PIT would have eventually delivered the interrupt, but we sacrifice perofmance to make sure that interrupts are not needlessly delayed. sfence isn't enough: atomic_dec_if_positive does atomic_read first and x86 can reorder loads before stores. lfence isn't enough: store can pass lfence, turning it into a nop. A compiler barrier would be more than enough as CPU needs to stall for unbelievably long to use fences. This patch doesn't do anything in kvm_pit_reset_reinject, because any order of resets can race, but the result differs by at most one interrupt, which is ok, because it's the same result as if the reset happened at a slightly different time. KVM: i8254: change PIT discard tick policy Discard policy uses ack_notifiers to prevent injection of PIT interrupts before EOI from the last one. This patch changes the policy to always try to deliver the interrupt, which makes a difference when its vector is in ISR. Old implementation would drop the interrupt, but proposed one injects to IRR, like real hardware would. The old policy breaks legacy NMI watchdogs, where PIT is used through virtual wire (LVT0): PIT never sends an interrupt before receiving EOI, thus a guest deadlock with disabled interrupts will stop NMIs. Note that NMI doesn't do EOI, so PIT also had to send a normal interrupt through IOAPIC. (KVM's PIT is deeply rotten and luckily not used much in modern systems.) Even though there is a chance of regressions, I think we can fix the LVT0 NMI bug without introducing a new tick policy. kvm: fix i8254 counter 0 wraparound The kvm i8254 emulation for counter 0 (but not for counters 1 and 2) has at least two bugs in mode 0: 1. The OUT bit, computed by pit_get_out(), is never set high. 2. The counter value, computed by pit_get_count(), wraps back around to the initial counter value, rather than wrapping back to 0xFFFF (which is the behavior described in the comment in __kpit_elapsed, the behavior implemented by qemu, and the behavior observed on AMD hardware). The bug stems from __kpit_elapsed computing the elapsed time mod the initial counter value (stored as nanoseconds in ps->period). This is both unnecessary (none of the callers of kpit_elapsed expect the value to be at most the initial counter value) and incorrect (it causes pit_get_count to appear to wrap around to the initial counter value rather than 0xFFFF). Removing this mod from __kpit_elapsed fixes both of the above bugs. KVM: x86: Run PIT work in own kthread We can't run PIT IRQ injection work in the interrupt context of the host timer. This would allow the user to influence the handler complexity by asking for a broadcast to a large number of VCPUs. Therefore, this work was pushed into workqueue context in 9d244caf2e. However, this prevents prioritizing the PIT injection over other task as workqueues share kernel threads. This replaces the workqueue with a kthread worker and gives that thread a name in the format "kvm-pit/<owner-process-pid>". KVM: Intelligent device lookup on I/O bus Currently the method of dealing with an IO operation on a bus (PIO/MMIO) is to call the read or write callback for each device registered on the bus until we find a device which handles it. Since the number of devices on a bus can be significant due to ioeventfds and coalesced MMIO zones, this leads to a lot of overhead on each IO operation. Instead of registering devices, we now register ranges which points to a device. Lookup is done using an efficient bsearch instead of a linear search. Performance test was conducted by comparing exit count per second with 200 ioeventfds created on one byte and the guest is trying to access a different byte continuously (triggering usermode exits). Before the patch the guest has achieved 259k exits per second, after the patch the guest does 274k exits per second. include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h percpu.h is included by sched.h and module.h and thus ends up being included when building most .c files. percpu.h includes slab.h which in turn includes gfp.h making everything defined by the two files universally available and complicating inclusion dependencies. percpu.h -> slab.h dependency is about to be removed. Prepare for this change by updating users of gfp and slab facilities include those headers directly instead of assuming availability. As this conversion needs to touch large number of source files, the following script is used as the basis of conversion. http://userweb.kernel.org/~tj/misc/slabh-sweep.py The script does the followings. * Scan files for gfp and slab usages and update includes such that only the necessary includes are there. ie. if only gfp is used, gfp.h, if slab is used, slab.h. * When the script inserts a new include, it looks at the include blocks and try to put the new include such that its order conforms to its surrounding. It's put in the include block which contains core kernel includes, in the same order that the rest are ordered - alphabetical, Christmas tree, rev-Xmas-tree or at the end if there doesn't seem to be any matching order. * If the script can't find a place to put a new include (mostly because the file doesn't have fitting include block), it prints out an error message indicating which .h file needs to be added to the file. The conversion was done in the following steps. 1. The initial automatic conversion of all .c files updated slightly over 4000 files, deleting around 700 includes and adding ~480 gfp.h and ~3000 slab.h inclusions. The script emitted errors for ~400 files. 2. Each error was manually checked. Some didn't need the inclusion, some needed manual addition while adding it to implementation .h or embedding .c file was more appropriate for others. This step added inclusions to around 150 files. 3. The script was run again and the output was compared to the edits from #2 to make sure no file was left behind. 4. Several build tests were done and a couple of problems were fixed. e.g. lib/decompress_*.c used malloc/free() wrappers around slab APIs requiring slab.h to be added manually. 5. The script was run on all .h files but without automatically editing them as sprinkling gfp.h and slab.h inclusions around .h files could easily lead to inclusion dependency hell. Most gfp.h inclusion directives were ignored as stuff from gfp.h was usually wildly available and often used in preprocessor macros. Each slab.h inclusion directive was examined and added manually as necessary. 6. percpu.h was updated not to include slab.h. 7. Build test were done on the following configurations and failures were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my distributed build env didn't work with gcov compiles) and a few more options had to be turned off depending on archs to make things build (like ipr on powerpc/64 which failed due to missing writeq). * x86 and x86_64 UP and SMP allmodconfig and a custom test config. * powerpc and powerpc64 SMP allmodconfig * sparc and sparc64 SMP allmodconfig * ia64 SMP allmodconfig * s390 SMP allmodconfig * alpha SMP allmodconfig * um on x86_64 SMP allmodconfig 8. percpu.h modifications were reverted so that it could be applied as a separate patch and serve as bisection point. Given the fact that I had only a couple of failures from tests on step 6, I'm fairly confident about the coverage of this conversion patch. If there is a breakage, it's likely to be something in one of the arch headers which should be easily discoverable easily on most builds of the specific arch. KVM: PIT support for HPET legacy mode When kvm is in hpet_legacy_mode, the hpet is providing the timer interrupt and the pit should not be. So in legacy mode, the pit timer is destroyed, but the *state* of the pit is maintained. So if kvm or the guest tries to modify the state of the pit, this modification is accepted, *except* that the timer isn't actually started. When we exit hpet_legacy_mode, the current state of the pit (which is up to date since we've been accepting modifications) is used to restart the pit timer. The saved_mode code in kvm_pit_load_count temporarily changes mode to 0xff in order to destroy the timer, but then restores the actual value, again maintaining "current" state of the pit for possible later reenablement. KVM: Allow PIT emulation without speaker port The in-kernel speaker emulation is only a dummy and also unneeded from the performance point of view. Rather, it takes user space support to generate sound output on the host, e.g. console beeps. To allow this, introduce KVM_CREATE_PIT2 which controls in-kernel speaker port emulation via a flag passed along the new IOCTL. KVM: x86: Enable NMI Watchdog via in-kernel PIT source LINT0 of the LAPIC can be used to route PIT events as NMI watchdog ticks into the guest. This patch aligns the in-kernel irqchip emulation with the user space irqchip with already supports this feature. The trick is to route PIT interrupts to all LAPIC's LVT0 lines. KVM: PIT: fix injection logic and count The PIT injection logic is problematic under the following cases: 1) If there is a higher priority vector to be delivered by the time kvm_pit_timer_intr_post is invoked ps->inject_pending won't be set. This opens the possibility for missing many PIT event injections (say if guest executes hlt at this point). 2) ps->inject_pending is racy with more than two vcpus. Since there's no locking around read/dec of pt->pending, two vcpu's can inject two interrupts for a single pt->pending count. Fix 1 by using an irq ack notifier: only reinject when the previous irq has been acked. Fix 2 with appropriate locking around manipulation of pending count and irq_ack by the injection / ack paths. KVM: close timer injection race window in __vcpu_run If a timer fires after kvm_inject_pending_timer_irqs() but before local_irq_disable() the code will enter guest mode and only inject such timer interrupt the next time an unrelated event causes an exit. It would be simpler if the timer->pending irq conversion could be done with IRQ's disabled, so that the above problem cannot happen. For now introduce a new vcpu requests bit to cancel guest entry.
