sh: add support for folded p4d page tables

Implement primitives necessary for the 4th level folding, add walks of p4d
level where appropriate and remove usage of __ARCH_USE_5LEVEL_HACK.

sh: Enable shared page fault handler for _32/_64.

This moves the now generic _32 page fault handling code to a shared place
and adapts the _64 implementation to make use of it.
