powerpc: Untangle fixmap.h and pgtable.h and mmu.h

fixmap.h need pgtable.h for [un]map_kernel_page()

pgtable.h need fixmap.h for FIXADDR_TOP.

Untangle the two files by moving FIXADDR_TOP into pgtable.h

Also move VIRT_IMMR_BASE to fixmap.h to avoid fixmap.h in mmu.h

powerpc/8xx: Remove immr_map() and immr_unmap()

Since commit fb533d0c5a97 ("[POWERPC] 8xx: Infrastructure code cleanup.")
immr_map() is just returning mpc8xxx_immr pointer and immr_unmap()
do nothing.

We already have parts of code that use mpc8xxx_immr directly so get rid
of immr_map() and immr_unmap() by using mpc8xxx_immr directly. And avoid
going through local pointers that hide the pointed structure.

powerpc/8xx: Use a larger CPM1 command check mask

The CPM1 command mask is defined for use with the standard
CPM1 command register as described in the user's manual:
0 |1 3|4 7|8 11|12 14| 15|
RST| - |OPCODE|CH_NUM| - |FLG|

In the QMC extension the CPM1 command register is redefined
(QMC supplement user's manuel) with the following mapping:
0 |1 3|4 7|8 13|14| 15|
RST|QMC OPCODE| 1110|CHANNEL_NUMBER| -|FLG|

Extend the check command mask in order to support both the
standard CH_NUM field and the QMC extension CHANNEL_NUMBER
field.

gpiolib: split of_mm_gpio_chip out of linux/of_gpio.h

This is a rarely used feature that has nothing to do with the
client-side of_gpio.h.

Split it out with a separate header file and Kconfig option
so it can be removed on its own timeline aside from removing
the of_gpio consumer interfaces.

powerpc/8xx: Move CPM interrupt controller into a dedicated file

CPM interrupt controller is quite standalone. Move it into a
dedicated file. It will help for next step which will change
it to a platform driver.

This is pure code move, checkpatch report is ignored at this point,
except one parenthesis alignment which would remain at the end of
the series. All other points fly away with following patches.

Don't inherit headers "by chances" from asm/prom.h, asm/mpc52xx.h,
asm/pci.h etc...

Include the needed headers, and remove asm/prom.h when it was
needed exclusively for pulling necessary headers.

mm: don't include asm/pgtable.h if linux/mm.h is already included

Patch series "mm: consolidate definitions of page table accessors", v2.

The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once. For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.

Most of these definitions are actually identical and typically it boils
down to, e.g.

}

These definitions can be shared among 90% of the arches provided
XYZ_SHIFT, PTRS_PER_XYZ and xyz_page_vaddr() are defined.

For architectures that really need a custom version there is always
possibility to override the generic version with the usual ifdefs magic.

These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.

This patch (of 12):

The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g. pte_alloc() and
pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.

The include statements in such cases are remove with a simple loop:

powerpc: Replace setup_irq() by request_irq()

request_irq() is preferred over setup_irq(). Invocations of setup_irq()
occur after memory allocators are ready.

Per tglx[1], setup_irq() existed in olden days when allocators were not
ready by the time early interrupts were initialized.

Hence replace setup_irq() by request_irq().

powerpc/8xx: use the fixmapped IMMR in cpm_reset()

Since commit f86ef74ed919 ("powerpc/8xx: Fix vaddr for IMMR early
remap"), the IMMR area has been mapped at startup with fixmap.

Use that fixmap directly instead of calling ioremap(), this
avoids calling ioremap() early before the slab is available.
