arch/*/io.h: remove ioremap_uc in some architectures

ioremap_uc() is only meaningful on old x86-32 systems with the PAT
extension, and on ia64 with its slightly unconventional ioremap()
behavior. So remove the ioremap_uc() definition in architecutures
other than x86 and ia64. These architectures all have asm-generic/io.h
included and will have the default ioremap_uc() definition which
returns NULL.

This changes the existing behaviour, while no need to worry about
any breakage because in the only callsite of ioremap_uc(), code
has been adjusted to eliminate the impact. Please see
atyfb_setup_generic() of drivers/video/fbdev/aty/atyfb_base.c.

If any new invocation of ioremap_uc() need be added, please consider
using ioremap() intead or adding a ARCH specific version if necessary.

powerpc: mm: convert to GENERIC_IOREMAP

By taking GENERIC_IOREMAP method, the generic generic_ioremap_prot(),
generic_iounmap(), and their generic wrapper ioremap_prot(), ioremap()
and iounmap() are all visible and available to arch. Arch needs to
provide wrapper functions to override the generic versions if there's
arch specific handling in its ioremap_prot(), ioremap() or iounmap().
This change will simplify implementation by removing duplicated code
with generic_ioremap_prot() and generic_iounmap(), and has the equivalent
functioality as before.

Here, add wrapper functions ioremap_prot() and iounmap() for powerpc's
special operation when ioremap() and iounmap().

Patch series "mm: ioremap: Convert architectures to take GENERIC_IOREMAP
way", v8.

Motivation and implementation:
==============================
Currently, many architecutres have't taken the standard GENERIC_IOREMAP
way to implement ioremap_prot(), iounmap(), and ioremap_xx(), but make
these functions specifically under each arch's folder. Those cause many
duplicated code of ioremap() and iounmap().

In this patchset, firstly introduce generic_ioremap_prot() and
generic_iounmap() to extract the generic code for GENERIC_IOREMAP. By
taking GENERIC_IOREMAP method, the generic generic_ioremap_prot(),
generic_iounmap(), and their generic wrapper ioremap_prot(), ioremap() and
iounmap() are all visible and available to arch. Arch needs to provide
wrapper functions to override the generic version if there's arch specific
handling in its corresponding ioremap_prot(), ioremap() or iounmap().
With these changes, duplicated ioremap/iounmap() code uder ARCH-es are
removed, and the equivalent functioality is kept as before.

2) In the previous v1 to v3, it's basically further action after arm64
has converted to GENERIC_IOREMAP way in below patchset. It's done by
adding hook ioremap_allowed() and iounmap_allowed() in ARCH to add ARCH
specific handling the middle of ioremap_prot() and iounmap().

Later, during v3 reviewing, Christophe Leroy suggested to introduce
generic_ioremap_prot() and generic_iounmap() to generic codes, and ARCH
can provide wrapper function ioremap_prot(), ioremap() or iounmap() if
needed. Christophe made a RFC patchset as below to specially demonstrate
his idea. This is what v4 and now v5 is doing.

Testing:
========
In v8, I only applied this patchset onto the latest linus's tree to build
and run on arm64 and s390.


This patch (of 19):

Let's use '#define ioremap_xx' and "#ifdef ioremap_xx" instead.

To remove defined ARCH_HAS_IOREMAP_xx macros in <asm/io.h> of each ARCH,
the ARCH's own ioremap_wc|wt|np definition need be above "#include
<asm-generic/iomap.h>. Otherwise the redefinition error would be seen
during compiling. So the relevant adjustments are made to avoid compiling
error:

loongarch:
- doesn't include <asm-generic/iomap.h>, defining ARCH_HAS_IOREMAP_WC
is redundant, so simply remove it.

m68k:
- selected GENERIC_IOMAP, <asm-generic/iomap.h> has been added in
<asm-generic/io.h>, and <asm/kmap.h> is included above
<asm-generic/iomap.h>, so simply remove ARCH_HAS_IOREMAP_WT defining.

mips:
- move "#include <asm-generic/iomap.h>" below ioremap_wc definition
in <asm/io.h>

powerpc:
- remove "#include <asm-generic/iomap.h>" in <asm/io.h> because it's
duplicated with the one in <asm-generic/io.h>, let's rely on the
latter.

x86:
- selected GENERIC_IOMAP, remove #include <asm-generic/iomap.h> in
the middle of <asm/io.h>. Let's rely on <asm-generic/io.h>.

powerpc/64: Add support to build with prefixed instructions

Add an option to build kernel and module with prefixed instructions if
the CPU and toolchain support it.

This is not related to kernel support for userspace execution of
prefixed instructions.

Building with prefixed instructions breaks some extended inline asm
memory addressing, for example it will provide immediates that exceed
the range of simple load/store displacement. Whether this is a
toolchain or a kernel asm problem remains to be seen. For now, these
are replaced with simpler and less efficient direct register addressing
when compiling with prefixed.

asm/ppc_asm.h is not needed in any of the header it is included.

It is only needed by irq.c. Include it there and remove it from
other headers.

word-at-a-time.h only need ex_table.h, so include it instead.

arch/*/: remove CONFIG_VIRT_TO_BUS

All architecture-independent users of virt_to_bus() and bus_to_virt()
have been fixed to use the dma mapping interfaces or have been
removed now. This means the definitions on most architectures, and the
CONFIG_VIRT_TO_BUS symbol are now obsolete and can be removed.

The only exceptions to this are a few network and scsi drivers for m68k
Amiga and VME machines and ppc32 Macintosh. These drivers work correctly
with the old interfaces and are probably not worth changing.

On alpha and parisc, virt_to_bus() were still used in asm/floppy.h.
alpha can use isa_virt_to_bus() like x86 does, and parisc can just
open-code the virt_to_phys() here, as this is architecture specific
code.

I tried updating the bus-virt-phys-mapping.rst documentation, which
started as an email from Linus to explain some details of the Linux-2.0
driver interfaces. The bits about virt_to_bus() were declared obsolete
backin 2000, and the rest is not all that relevant any more, so in the
end I just decided to remove the file completely.

Building tinyconfig with gcc (Debian 11.2.0-16) and assembler (Debian
2.37.90.20220207) the following build error shows up:

Rework to add assembler directives [1] around the instruction. Going
through them one by one shows that the changes should be safe. Like
__get_user_atomic_128_aligned() is only called in p9_hmi_special_emu(),
which according to the name is specific to power9. And __raw_rm_read*()
are only called in things that are powernv or book3s_hv specific.

powerpc: Fix update form addressing in inline assembly

In several places, inline assembly uses the "%Un" modifier
to enable the use of instruction with update form addressing,
but the associated "<>" constraint is missing.

As mentioned in previous patch, this fails with gcc 4.9, so
"<>" can't be used directly.

Use UPD_CONSTR macro everywhere %Un modifier is used.

powerpc: inline iomap accessors

ioreadXX()/ioreadXXbe() accessors are equivalent to ppc
in_leXX()/in_be16() accessors but they are not inlined.

Since commit 0eb573682872 ("powerpc/kerenl: Enable EEH for IO
accessors"), the 'le' versions are equivalent to the ones
defined in asm-generic/io.h, allthough the ones there are inlined.

Include asm-generic/io.h to get them. Keep ppc versions of the
'be' ones as they are optimised, but make them inline in ppc io.h.

This reduces the size of ppc64e_defconfig build by 3 kbytes:

mm: don't include asm/pgtable.h if linux/mm.h is already included

Patch series "mm: consolidate definitions of page table accessors", v2.

The low level page table accessors (pXY_index(), pXY_offset()) are
duplicated across all architectures and sometimes more than once. For
instance, we have 31 definition of pgd_offset() for 25 supported
architectures.

Most of these definitions are actually identical and typically it boils
down to, e.g.

These patches introduce include/linux/pgtable.h that replaces
include/asm-generic/pgtable.h and add the definitions of the page table
accessors to the new header.

This patch (of 12):

The linux/mm.h header includes <asm/pgtable.h> to allow inlining of the
functions involving page table manipulations, e.g. pte_alloc() and
pmd_alloc(). So, there is no point to explicitly include <asm/pgtable.h>
in the files that include <linux/mm.h>.

The include statements in such cases are remove with a simple loop:

powerpc: add an ioremap_phb helper

Factor code shared between pci_64 and electra_cf into a ioremap_pbh helper
that follows the normal ioremap semantics, and returns a useful __iomem
pointer. Note that it opencodes __ioremap_at as we know from the callers
the slab is available. Switch pci_64 to also store the result as __iomem
pointer, and unmap the result using iounmap instead of force casting and
using vmalloc APIs.

powerpc/mm: refactor ioremap vm area setup.

PPC32 and PPC64 are doing the same once SLAB is available.
Create a do_ioremap() function that calls get_vm_area and
do the mapping.

For PPC64, we add the 4K PFN hack sanity check to __ioremap_caller()
in order to avoid using __ioremap_at(). Other checks in __ioremap_at()
are irrelevant for __ioremap_caller().

On PPC64, VM area is allocated in the range [ioremap_bot ; IOREMAP_END]
On PPC32, VM area is allocated in the range [VMALLOC_START ; VMALLOC_END]

Lets define IOREMAP_START is ioremap_bot for PPC64, and alias
IOREMAP_START/END to VMALLOC_START/END on PPC32

powerpc/mm: refactor ioremap_range() and use ioremap_page_range()

book3s64's ioremap_range() is almost same as fallback ioremap_range(),
except that it calls radix__ioremap_range() when radix is enabled.

radix__ioremap_range() is also very similar to the other ones, expect
that it calls ioremap_page_range when slab is available.

PPC32 __ioremap_caller() have a loop doing the same thing as
ioremap_range() so use it on PPC32 as well.

Lets keep only one version of ioremap_range() which calls
ioremap_page_range() on all platforms when slab is available.

At the same time, drop the nid parameter which is not used.

In order to provide non-atomic functions for io{read|write}64 that will

powerpc/io: Fix the IO workarounds code to work with Radix

Back in 2006 Ben added some workarounds for a misbehaviour in the
Spider IO bridge used on early Cell machines, see commit
014da7ff47b5 ("[POWERPC] Cell "Spider" MMIO workarounds"). Later these
were made to be generic, ie. not tied specifically to Spider.

The code stashes a token in the high bits (59-48) of virtual addresses
used for IO (eg. returned from ioremap()). This works fine when using
the Hash MMU, but when we're using the Radix MMU the bits used for the
token overlap with some of the bits of the virtual address.

This is because the maximum virtual address is larger with Radix, up
to c00fffffffffffff, and in fact we use that high part of the address
range for ioremap(), see RADIX_KERN_IO_START.

As it happens the bits that are used overlap with the bits that
differentiate an IO address vs a linear map address. If the resulting
address lies outside the linear mapping we will crash (see below), if
not we just corrupt memory.

This hasn't been a problem because CONFIG_PPC_IO_WORKAROUNDS which
enables this code is usually not enabled. It is only enabled when it's
selected by PPC_CELL_NATIVE which is only selected by
PPC_IBM_CELL_BLADE and that in turn depends on BIG_ENDIAN. So in order
to hit the bug you need to build a big endian kernel, with IBM Cell
Blade support enabled, as well as Radix MMU support, and then boot
that on Power9 using Radix MMU.

Still we can fix the bug, so let's do that. We simply use fewer bits
for the token, taking the union of the restrictions on the address
from both Hash and Radix, we end up with 8 bits we can use for the
token. The only user of the token is iowa_mem_find_bus() which only
supports 8 token values, so 8 bits is plenty for that.

powerpc/io: Add __raw_writeq_be() __raw_rm_writeq_be()

Add byte-swapping versions of __raw_writeq() and __raw_rm_writeq().

This allows us to avoid sparse warnings caused by passing __be64 to
__raw_writeq(), which takes unsigned long:

arch/powerpc/platforms/powernv/pci-ioda.c:1981:38:
warning: incorrect type in argument 1 (different base types)
expected unsigned long [unsigned] v
got restricted __be64 [usertype] <noident>

It's also generally preferable to use a byte-swapping accessor rather
than doing it by hand in the code, which is more bug prone.

powerpc: Drop include of linux/io.h from asm/io.h

Currently powerpc's asm/io.h includes linux/io.h, and linux/io.h
includes asm/io.h.

This can cause problems because depending on which is included first the
order of definitions between the two files will change.

The include of linux/io.h was added back in 2008 in commit b41e5fffe8b8
("[POWERPC] devres: Add devm_ioremap_prot()"). It's not entirely clear
it was needed then, but devm_ioremap_prot() has since been removed
entirely as unused, in dedd24a12fe6 ("powerpc: Remove unused
devm_ioremap_prot()").

So it seems to be unnecessary and can potentially cause problems, so
remove the include of linux/io.h from asm/io.h

powerpc: Consolidate variants of real-mode MMIOs

We have all sort of variants of MMIO accessors for the real mode
instructions. This creates a clean set of accessors based on
Linux normal naming conventions, replacing all occurrences of
the old ones in the tree.

I have purposefully removed the "out/in" variants in favor of
only including __raw variants. Any code using these is already
pretty much hand tuned to operate in a very specific environment.
I've fixed up the 2 users (only one of them actually needed
a barrier in the first place).

Add simple cache inhibited accessors for memory mapped I/O.
Unlike the accessors built from the DEF_MMIO_* macros, these
don't include any hardware memory barriers, callers need to
manage memory barriers on their own. These can only be called
in hypervisor real mode.

arch/*/io.h: Add ioremap_uc() to all architectures

This adds ioremap_uc() only for architectures that do not
include asm-generic.h/io.h as that already provides a default
definition for them for both cases where you have CONFIG_MMU
and you do not, and because of this, the number of architectures
this patch address is less than the architectures that the
ioremap_wt() patch addressed, "arch/*/io.h: Add ioremap_wt() to
all architectures").

In order to reduce the number of architectures we have to
modify by adding new architecture IO APIs we'll have to review
the architectures in this patch, see why they can't add
asm-generic.h/io.h or issues that would be created by doing
so and then spread a consistent inclusion of this header
towards the end of their own header. For instance arch/metag
includes the asm-generic/io.h *before* the ioremap*()
definitions, this should be the other way around but only
once we have guard wrappers for the non-MMU case also for
asm-generic/io.h.

powerpc: Enable /dev/port when isa_io_special is set

isa_io_special is set when the platform provides a "special"
implementation of inX/outX via some FW interface for example.

Such a platform doesn't need an ISA bridge on PCI, and so /dev/port
should be made available even if one isn't present.

This makes the LPC bus IOs accessible via /dev/port on PowerNV Power8

powerpc: Support endian agnostic MMIO

This patch maps the MMIO functions for 32bit PowerPC to their
appropriate instructions depending on CPU endianness.

The macros used to create the corresponding inline functions are also
renamed by this patch. Previously they had BE or LE in their names which
was misleading - they had nothing to do with endianness, but actually
created different instruction forms so their new names reflect the
instruction form they are creating (D-Form and X-Form).

Little endian 64bit PowerPC is not supported, so the lack of mappings
(and corresponding breakage) for that case is intentional to bring the
attention of anyone doing a 64bit little endian port. 64bit big endian
is unaffected.

powerpc: Disable /dev/port interface on systems without an ISA bridge

Some power systems do not have legacy ISA devices. So, /dev/port is not
a valid interface on these systems. User level tools such as kbdrate is
trying to access the device using this interface which is causing the
system crash.

This patch will fix this issue by not creating this interface on these
powerpc systems.

POWERPC: Allow 32-bit hashed pgtable code to support 36-bit physical

This rearranges a bit of code, and adds support for
36-bit physical addressing for configs that use a
hashed page table. The 36b physical support is not
enabled by default on any config - it must be
explicitly enabled via the config system.

This patch *only* expands the page table code to accomodate
large physical addresses on 32-bit systems and enables the
PHYS_64BIT config option for 86xx. It does *not*
allow you to boot a board with more than about 3.5GB of
RAM - for that, SWIOTLB support is also required (and
coming soon).
