powerpc/book3s64/mm: enable transparent pud hugepage This is enabled only with radix translation and 1G hugepage size. This will be used with devdax device memory with a namespace alignment of 1G. Anon transparent hugepage is not supported even though we do have helpers checking pud_trans_huge(). We should never find that return true. The only expected pte bit combination is _PAGE_PTE | _PAGE_DEVMAP. Some of the helpers are never expected to get called on hash translation and hence is marked to call BUG() in such a case. mmu_notifiers: call invalidate_range() when invalidating TLBs The invalidate_range() is going to become an architecture specific mmu notifier used to keep the TLB of secondary MMUs such as an IOMMU in sync with the CPU page tables. Currently it is called from separate code paths to the main CPU TLB invalidations. This can lead to a secondary TLB not getting invalidated when required and makes it hard to reason about when exactly the secondary TLB is invalidated. To fix this move the notifier call to the architecture specific TLB maintenance functions for architectures that have secondary MMUs requiring explicit software invalidations. This fixes a SMMU bug on ARM64. On ARM64 PTE permission upgrades require a TLB invalidation. This invalidation is done by the architecture specific ptep_set_access_flags() which calls flush_tlb_page() if required. However this doesn't call the notifier resulting in infinite faults being generated by devices using the SMMU if it has previously cached a read-only PTE in it's TLB. Moving the invalidations into the TLB invalidation functions ensures all invalidations happen at the same time as the CPU invalidation. The architecture specific flush_tlb_all() routines do not call the notifier as none of the IOMMUs require this. mm: add PTE pointer parameter to flush_tlb_fix_spurious_fault() s390 can do more fine-grained handling of spurious TLB protection faults, when there also is the PTE pointer available. Therefore, pass on the PTE pointer to flush_tlb_fix_spurious_fault() as an additional parameter. This will add no functional change to other architectures, but those with private flush_tlb_fix_spurious_fault() implementations need to be made aware of the new parameter. powerpc/64s: Fix __pte_needs_flush() false positive warning Userspace PROT_NONE ptes set _PAGE_PRIVILEGED, triggering a false positive debug assertion that __pte_flags_need_flush() is not called on a kernel mapping. Detect when it is a userspace PROT_NONE page by checking the required bits of PAGE_NONE are set, and none of the RWX bits are set. pte_protnone() is insufficient here because it always returns 0 when CONFIG_NUMA_BALANCING=n. powerpc/64s: Reconnect tlb_flush() to hash__tlb_flush() Commit baf1ed24b27d ("powerpc/mm: Remove empty hash__ functions") removed some empty hash MMU flushing routines, but got a bit overeager and also removed the call to hash__tlb_flush() from tlb_flush(). In regular use this doesn't lead to any noticable breakage, which is a little concerning. Presumably there are flushes happening via other paths such as arch_leave_lazy_mmu_mode(), and/or a bit of luck. Fix it by reinstating the call to hash__tlb_flush(). powerpc/tlb: Add local flush for page given mm_struct and psize Adds a local TLB flush operation that works given an mm_struct, VA to flush, and page size representation. Most implementations mirror the surrounding code. The book3s/32/tlbflush.h implementation is left as a BUILD_BUG because it is more complicated and not required for anything as yet. This removes the need to create a vm_area_struct, which the temporary patching mm work does not need. powerpc/mm: Remove flush_all_mm, local_flush_all_mm These functions were introduced for "cxl: Enable global TLBIs for cxl contexts" [1], which ended up using them for Radix only. They were never implemented on Hash (and creating an implementation appears to be difficult), so nothing can actually rely on them. They behave differently to the existing surrounding functions too, in that they actually need to do something on Hash. The other functions are primarily for use in generic code that expects their definitions, but Hash updates the TLB during PTE updates. After replacing the only usage with the Radix specific version, there are no more users of these functions, and given they are not implemented anyway it is safe to delete them. powerpc/mm: Remove empty hash__ functions The empty hash__* functions are unnecessary. The empty definitions were introduced when 64-bit Hash support was added, as the functions were still used in generic code. These empty definitions were prefixed with hash__ when Radix support was added, and new wrappers with the original names were added that selected the Radix or Hash version based on radix_enabled(). But the hash__ prefixed functions were not part of a public interface, so there is no need to include them for compatibility with anything. Generic code will use the non-prefixed wrappers, and Hash specific code will know that there is no point in calling them (or even worse, call them and expect them to do something). powerpc/64s: add pte_needs_flush and huge_pmd_needs_flush Allow PTE changes to avoid flushing the TLB when access permissions are being relaxed, the dirty bit is being set, and the accessed bit is being changed. Relaxing access permissions and setting dirty and accessed bits do not require a flush because the MMU will re-load the PTE and notice the updates (it may also cause a spurious fault). Clearing the accessed bit does not require a flush because of the imprecise PTE accessed bit accounting that is already performed, as documented in ptep_clear_flush_young(). This reduces TLB flushing for some mprotect(2) calls. powerpc/64s: Remove spurious fault flushing for NMMU Commit 6d8278c414cb2 ("powerpc/64s/radix: do not flush TLB on spurious fault") removed the TLB flush for spurious faults, except when a coprocessor (nest MMU) maps the address space. This is not needed because the NMMU workaround in the PTE permission upgrade paths prevents PTEs existing with less restrictive access permissions than their corresponding TLB entries have. Remove it and replace with a comment. powerpc/64s/radix: introduce options to disable use of the tlbie instruction Introduce two options to control the use of the tlbie instruction. A boot time option which completely disables the kernel using the instruction, this is currently incompatible with HASH MMU, KVM, and coherent accelerators. And a debugfs option can be switched at runtime and avoids using tlbie for invalidating CPU TLBs for normal process and kernel address mappings. Coherent accelerators are still managed with tlbie, as will KVM partition scope translations. Cross-CPU TLB flushing is implemented with IPIs and tlbiel. This is a basic implementation which does not attempt to make any optimisation beyond the tlbie implementation. This is useful for performance testing among other things. For example in certain situations on large systems, using IPIs may be faster than tlbie as they can be directed rather than broadcast. Later we may also take advantage of the IPIs to do more interesting things such as trim the mm cpumask more aggressively. powerpc/64s/radix: do not flush TLB on spurious fault In the case of a spurious fault (which can happen due to a race with another thread that changes the page table), the default Linux mm code calls flush_tlb_page for that address. This is not required because the pte will be re-fetched. Hash does not wire this up to a hardware TLB flush for this reason. This patch avoids the flush for radix. >From Power ISA v3.0B, p.1090: Setting a Reference or Change Bit or Upgrading Access Authority (PTE Subject to Atomic Hardware Updates) If the only change being made to a valid PTE that is subject to atomic hardware updates is to set the Refer- ence or Change bit to 1 or to add access authorities, a simpler sequence suffices because the translation hardware will refetch the PTE if an access is attempted for which the only problems were reference and/or change bits needing to be set or insufficient access authority. The nest MMU on POWER9 does not re-fetch the PTE after such an access attempt before faulting, so address spaces with a coprocessor attached will continue to flush in these cases. This reduces tlbies for a kernel compile workload from 0.95M to 0.90M. fork --fork --exec benchmark improved 0.5% (12300->12400). It should never be called, due to checks in callers so it calls BUG(). We should probably clean it up further but this will suffice for now. powerpc/64s: Improve local TLB flush for boot and MCE on POWER9 There are several cases outside the normal address space management where a CPU's entire local TLB is to be flushed: 1. Booting the kernel, in case something has left stale entries in the TLB (e.g., kexec). 2. Machine check, to clean corrupted TLB entries. One other place where the TLB is flushed, is waking from deep idle states. The flush is a side-effect of calling ->cpu_restore with the intention of re-setting various SPRs. The flush itself is unnecessary because in the first case, the TLB should not acquire new corrupted TLB entries as part of sleep/wake (though they may be lost). This type of TLB flush is coded inflexibly, several times for each CPU type, and they have a number of problems with ISA v3.0B: - The current radix mode of the MMU is not taken into account, it is always done as a hash flushn For IS=2 (LPID-matching flush from host) and IS=3 with HV=0 (guest kernel flush), tlbie(l) is undefined if the R field does not match the current radix mode. - ISA v3.0B hash must flush the partition and process table caches as well. - ISA v3.0B radix must flush partition and process scoped translations, partition and process table caches, and also the page walk cache. So consolidate the flushing code and implement it in C and inline asm under the mm/ directory with the rest of the flush code. Add ISA v3.0B cases for radix and hash, and use the radix flush in radix environment. Provide a way for IS=2 (LPID flush) to specify the radix mode of the partition. Have KVM pass in the radix mode of the guest. Take out the flushes from early cputable/dt_cpu_ftrs detection hooks, and move it later in the boot process after, the MMU registers are set up and before relocation is first turned on. The TLB flush is no longer called when restoring from deep idle states. This was not be done as a separate step because booting secondaries uses the same cpu_restore as idle restore, which needs the TLB flush. License cleanup: add SPDX GPL-2.0 license identifier to files with no license Many source files in the tree are missing licensing information, which makes it harder for compliance tools to determine the correct license. By default all files without license information are under the default license of the kernel, which is GPL version 2. Update the files which contain no license information with the 'GPL-2.0' SPDX license identifier. The SPDX identifier is a legally binding shorthand, which can be used instead of the full boiler plate text. This patch is based on work done by Thomas Gleixner and Kate Stewart and Philippe Ombredanne. How this work was done: Patches were generated and checked against linux-4.14-rc6 for a subset of the use cases: - file had no licensing information it it. - file was a */uapi/* one with no licensing information in it, - file was a */uapi/* one with existing licensing information, Further patches will be generated in subsequent months to fix up cases where non-standard license headers were used, and references to license had to be inferred by heuristics based on keywords. The analysis to determine which SPDX License Identifier to be applied to a file was done in a spreadsheet of side by side results from of the output of two independent scanners (ScanCode & Windriver) producing SPDX tag:value files created by Philippe Ombredanne. Philippe prepared the base worksheet, and did an initial spot review of a few 1000 files. The 4.13 kernel was the starting point of the analysis with 60,537 files assessed. Kate Stewart did a file by file comparison of the scanner results in the spreadsheet to determine which SPDX license identifier(s) to be applied to the file. She confirmed any determination that was not immediately clear with lawyers working with the Linux Foundation. Criteria used to select files for SPDX license identifier tagging was: - Files considered eligible had to be source code files. - Make and config files were included as candidates if they contained >5 lines of source - File already had some variant of a license header in it (even if <5 lines). All documentation files were explicitly excluded. The following heuristics were used to determine which SPDX license identifiers to apply. - when both scanners couldn't find any license traces, file was considered to have no license information in it, and the top level COPYING file license applied. For non */uapi/* files that summary was: SPDX license identifier # files ---------------------------------------------------|------- GPL-2.0 11139 and resulted in the first patch in this series. If that file was a */uapi/* path one, it was "GPL-2.0 WITH Linux-syscall-note" otherwise it was "GPL-2.0". Results of that was: SPDX license identifier # files ---------------------------------------------------|------- GPL-2.0 WITH Linux-syscall-note 930 and resulted in the second patch in this series. - if a file had some form of licensing information in it, and was one of the */uapi/* ones, it was denoted with the Linux-syscall-note if any GPL family license was found in the file or had no licensing in it (per prior point). Results summary: SPDX license identifier # files ---------------------------------------------------|------ GPL-2.0 WITH Linux-syscall-note 270 GPL-2.0+ WITH Linux-syscall-note 169 ((GPL-2.0 WITH Linux-syscall-note) OR BSD-2-Clause) 21 ((GPL-2.0 WITH Linux-syscall-note) OR BSD-3-Clause) 17 LGPL-2.1+ WITH Linux-syscall-note 15 GPL-1.0+ WITH Linux-syscall-note 14 ((GPL-2.0+ WITH Linux-syscall-note) OR BSD-3-Clause) 5 LGPL-2.0+ WITH Linux-syscall-note 4 LGPL-2.1 WITH Linux-syscall-note 3 ((GPL-2.0 WITH Linux-syscall-note) OR MIT) 3 ((GPL-2.0 WITH Linux-syscall-note) AND MIT) 1 and that resulted in the third patch in this series. - when the two scanners agreed on the detected license(s), that became the concluded license(s). - when there was disagreement between the two scanners (one detected a license but the other didn't, or they both detected different licenses) a manual inspection of the file occurred. - In most cases a manual inspection of the information in the file resulted in a clear resolution of the license that should apply (and which scanner probably needed to revisit its heuristics). - When it was not immediately clear, the license identifier was confirmed with lawyers working with the Linux Foundation. - If there was any question as to the appropriate license identifier, the file was flagged for further research and to be revisited later in time. In total, over 70 hours of logged manual review was done on the spreadsheet to determine the SPDX license identifiers to apply to the source files by Kate, Philippe, Thomas and, in some cases, confirmation by lawyers working with the Linux Foundation. Kate also obtained a third independent scan of the 4.13 code base from FOSSology, and compared selected files where the other two scanners disagreed against that SPDX file, to see if there was new insights. The Windriver scanner is based on an older version of FOSSology in part, so they are related. Thomas did random spot checks in about 500 files from the spreadsheets for the uapi headers and agreed with SPDX license identifier in the files he inspected. For the non-uapi files Thomas did random spot checks in about 15000 files. In initial set of patches against 4.14-rc6, 3 files were found to have copy/paste license identifier errors, and have been fixed to reflect the correct identifier. Additionally Philippe spent 10 hours this week doing a detailed manual inspection and review of the 12,461 patched files from the initial patch version early this week with: - a full scancode scan run, collecting the matched texts, detected license ids and scores - reviewing anything where there was a license detected (about 500+ files) to ensure that the applied SPDX license was correct - reviewing anything where there was no detection but the patch license was not GPL-2.0 WITH Linux-syscall-note to ensure that the applied SPDX license was correct This produced a worksheet with 20 files needing minor correction. This worksheet was then exported into 3 different .csv files for the different types of files to be modified. These .csv files were then reviewed by Greg. Thomas wrote a script to parse the csv files and add the proper SPDX tag to the file, in the format that the file expected. This script was further refined by Greg based on the output to detect more types of files automatically and to distinguish between header and source .c files (which need different comment types.) Finally Greg ran the script using the .csv files to generate the patches. powerpc/mm: remove flush_tlb_page_nohash This should be same as flush_tlb_page except for hash32. For hash32 I guess the existing code is wrong, because we don't seem to be flushing tlb for Hash != 0 case at all. powerpc/mm/radix: Flush page walk cache when freeing page table Even though a tlb_flush() does a flush with invalidate all cache, we can end up doing an RCU page table free before calling tlb_flush(). That means we can have page walk cache entries even after we free the page table pages. This can result in us doing wrong page table walk. Avoid this by doing pwc flush on every page table free. We can't batch the pwc flush, because the rcu call back function where we free the page table pages doesn't have information of the mmu gather. Thus we have to do a pwc on every page table page freed. Note: I also removed the dummy tlb_flush_pgtable call functions for hash 32. powerpc/mm/radix: Add tlbflush routines Core kernel doesn't track the page size of the VA range that we are invalidating. Hence we end up flushing TLB for the entire mm here. Later patches will improve this. We also don't flush page walk cache separetly instead use RIC=2 when flushing TLB, because we do a MMU gather flush after freeing page table.
