docs: block/bfq: describe per-device weight

The functionality of setting per-device weight for BFQ was added
in v5.4 (commit 795fe54c2a828099), but the documentation was never
updated.

While at it, improve formatting a bit.

block/bfq: update comments and default value in docs for fifo_expire

Correct the comments since bfq_fifo_expire[0] is for async request,
while bfq_fifo_expire[1] is for sync request.
Also update docs, according the source code, the default
fifo_expire_async is 250ms, and fifo_expire_sync is 125ms.

The max value of blkio.bfq.weight is 1000, rather than 10000.
And 'weights' have been remove from /sys/block/XXX/queue/iosched.

docs: block: convert to ReST

Rename the block documentation files to ReST, add an
index for them and adjust in order to produce a nice html
output via the Sphinx build system.

At its new index.rst, let's add a :orphan: while this is not linked to
the main index.rst file, in order to avoid build warnings.
