kernel/arm: hush memory type warnings

arch_vm_set_memory_type() seems to be x86-specific (cf. MTRR) and
on other architectures it's enough to respect memoryType argument
passed to Map() and Protect()

ARM: make sure we cleanup after the bootloader

The "2nd" assert that we always ran into was due to bootloader mappings
still being active after VM init. Turns out we missed a call in the
architecture specific code for cleaning this up.

Many thanks to Ingo for spending the time to figure this out!

ARM: only warn if unknown memory types are being mapped.

This is a workaround for missing writecombine & friends support. Needs
proper fixing, but too many other things to do atm to focus on that...

kernel arm: Quick style cleanup; No functional change

* First run through the kernel's private parts to use phys_{addr,size}_t
where appropriate.
* Typedef'ed page_num_t to phys_addr_t and used it in more places in
vm_page.{h,cpp}.

* Moved the VM headers into subdirectory vm/.
* Renamed vm_cache.h/vm_address_space.h to VMCache.h/VMAddressSpace.

[GSoC] [ARM] Patch by Johannes Wischert.
- stubbed out many arch kernel functions (borrowed from other archs),
- partially implement ELF relocations code, enough to load the kernel,
- move uart.c to kernel sources and use the same one for the loader,
- default implementation for gensyscalls,
- assembler code functions for interrupt masking (enable/disable/restore/query).
