Removed superflous NULL pointer checks.
The media kit will always call BMediaNode
and BMediaAddOn functions with valid pointers.

Renamed from fileproducer to reader, both classes and directories
Don't bother looking at fileproducer since there are no versions there
beyond the initial commit.
