added some more headers from pcmcia-cs
fixed the build for non haiku platforms

added pcmcia headers and command line tools from pcmcia-cs-3.2.8
tested ok on Dano
note that _IO* macros were redefined for the R5 target, the ones in Haiku aren't compatible with R5
