[audio][intel-hda][qemu] Update QEMU HDA codec drv proto

Update QEMU HDA codecs to support reporting unique IDs as well as
manufacturer/product strings.

Test: Manual; Tested in QEMU using new audio app

[dev][intel-hda] Fix min & max sample-rate in QEMU

Previously, QEMU did not set max_frames_per_second. Because of typo, it
set min_frames_per_second twice. Fixing this takes us one step closer
to (re)enabling QEMU's record-to-wavfile support underneath the system
mixer. However, because the system mixer can now save its output(s)
directly to file(s), we may never need to complete the QEMU work.

This CL also includes a trivial fix for an assert in the realtek codec.
