spdx: The BSD-2-Clause-FreeBSD identifier is obsolete, drop -FreeBSD

The SPDX folks have obsoleted the BSD-2-Clause-FreeBSD identifier. Catch
up to that fact and revert to their recommended match of BSD-2-Clause.

sys: general adoption of SPDX licensing ID tags.

Mainly focus on files that use BSD 2-Clause license, however the tool I
was using misidentified many licenses so this was mostly a manual - error
prone - task.

The Software Package Data Exchange (SPDX) group provides a specification
to make it easier for automated tools to detect and summarize well known
opensource licenses. We are gradually adopting the specification, noting
that the tags are considered only advisory and do not, in any way,
superceed or replace the license texts.

No functional change intended.

# f809f280 10-Dec-2015 Warner Losh <imp@FreeBSD.org>

Create a USB_PNP_INFO and use it to export the existing PNP
tables. Some drivers needed some slight re-arrangement of declarations
to accommodate this. Change the USB pnp tables slightly to allow
better compatibility with the system by moving linux driver info from
start of each entry to the end. All other PNP tables in the system
have the per-device flags and such at the end of the elements rather
that at the beginning.

- Move all USB device ID arrays into so-called sections,
sorted according to the mode which they support:
host, device or dual mode
- Add generic tool to extract these data:
tools/bus_autoconf

# 6bfa9a2d 27-Sep-2008 Ed Schouten <ed@FreeBSD.org>

Replace all calls to minor() with dev2unit().

After I removed all the unit2minor()/minor2unit() calls from the kernel
yesterday, I realised calling minor() everywhere is quite confusing.
Character devices now only have the ability to store a unit number, not
a minor number. Remove the confusion by using dev2unit() everywhere.

This commit could also be considered as a bug fix. A lot of drivers call
minor(), while they should actually be calling dev2unit(). In -CURRENT
this isn't a problem, but it turns out we never had any problem reports
related to that issue in the past. I suspect not many people connect
more than 256 pieces of the same hardware.

I don't know from where the notion that device driver should or
even could call VOP_REVOKE() on vnodes associated with its dev_t's
has originated, but it stops right here.

If there are things people belive destroy_dev() needs to learn how to
do, please tell me about it, preferably with a reproducible test case.

Include <sys/uio.h> in bluetooth code rather than rely on <sys/vnode.h>
to do so.

The fact that some of the USB code needs to include <sys/vnode.h>
still disturbs me greatly, but I do not have time to chase that.
