ng_ubt(4): do not attach Intel Wireless 8260/8265 in bootloader mode.

Add helper function for synchronous execution of HCI commands at probe
stage and use this function to check firmware state of Intel Wireless
8260/8265 bluetooth devices found in many post 2016 year laptops.

Attempt to initialize FreeBSD bluetooth stack while such a device is in
bootloader mode locks the adapter hardly so it requires power on/off
cycle to restore.

This change blocks ng_ubt attachment unless operational firmware is
loaded thus preventing the lock up.

sys: general adoption of SPDX licensing ID tags.

Mainly focus on files that use BSD 2-Clause license, however the tool I
was using misidentified many licenses so this was mostly a manual - error
prone - task.

The Software Package Data Exchange (SPDX) group provides a specification
to make it easier for automated tools to detect and summarize well known
opensource licenses. We are gradually adopting the specification, noting
that the tags are considered only advisory and do not, in any way,
superceed or replace the license texts.

No functional change intended.

Get rid of device nodes interface. It is useless and confusing.
The original idea was to use it for firmware upgrading and similar
operations. In real life almost all Bluetooth USB devices do not
need firmware download. If device does require firmware download
then ugen(4) (or specialized driver like ubtbcmfw(8)) should be
used instead.

The second try a committing the bluetooth code
Has been seen to work on several cards and communicating with
several mobile phones to use them as modems etc.

We are still talking with 3com to try get them to allow us to include
the firmware for their pccard in the driver but the driver is here..
In the mean time
it can be downloaded from the 3com website and loaded using the utility
bt3cfw(8) (supplied) (instructions in the man page)
