Add the MEM_EXTRACT_PADDR ioctl to /dev/mem. This allows privileged userspace processes to find information about the physical page backing a given mapping. sys/arm: further adoption of SPDX licensing ID tags. Mainly focus on files that use BSD 2-Clause license, however the tool I was using misidentified many licenses so this was mostly a manual - error prone - task. The Software Package Data Exchange (SPDX) group provides a specification to make it easier for automated tools to detect and summarize well known opensource licenses. We are gradually adopting the specification, noting that the tags are considered only advisory and do not, in any way, superceed or replace the license texts.

Add reader/writer lock around mem_range_attr_get() and mem_range_attr_set(). Compile sys/dev/mem/memutil.c for all supported platforms and remove now unnecessary dev_mem_md_init(). Consistently define mem_range_softc from mem.c for all platforms. Add missing #include guards for machine/memdev.h and sys/memrange.h. Clean up some nearby style(9) nits. MFC after: 1 month

Remove the unused major/minor numbers from iodev and memdev. Now that st_rdev is being automatically generated by the kernel, there is no need to define static major/minor numbers for the iodev and memdev. We still need the minor numbers for the memdev, however, to distinguish between /dev/mem and /dev/kmem. Approved by: philip (mentor)
