bin: Automated cleanup of cdefs and other formatting

Apply the following automated changes to try to eliminate
no-longer-needed sys/cdefs.h includes as well as now-empty
blank lines in a row.

ed(1): Prevent possible overflows during allocation.

Make sure the parameters used for malloc(3) can hold size_t sizes.
This should help ed(1) handle bigger data in the future.

bin/ed: use correct type in multiplication
The result is line_t** so the multiplication should be size *
sizeof(line_t*)

Fix warnings found by -Wmising-variable-declarations.

This self-written compiler warning, which is hopefully going to be
committed into LLVM sources soon, warns about potentially missing
`static' keywords, similar to -Wmissing-prototypes.

- bin/pax: Move external declaration of chdname and s_mask into extern.h.
- bin/setfacl: Move setfacl.c-specific stuff out of setfacl.h.
- sbin/mount_fusefs: Remove char *progname; use getprogname().
- others: add `static' where possible.

A new jail(8) with a configuration file, to replace the work currently done
by /etc/rc.d/jail.

Create the altix project branch. The altix project will add support
for the SGI Altix 350 to FreeBSD/ia64. The hardware used for porting
is a two-module system, consisting of a base compute module and a
CPU expansion module. SGI's NUMAFlex architecture can be an excellent
platform to test CPU affinity and NUMA-aware features in FreeBSD.

- Import the HEAD csup code which is the basis for the cvsmode work.

Consistently use __FBSDID

o Old-style K&R declarations have been converted to new C89 style
o register has been removed
o prototype for main() has been removed (gcc3 makes it an error)
o int main(int argc, char *argv[]) is the preferred main definition.

sccsid -> rcsid and fix some minor nits.

Revert $FreeBSD$ to $Id$

Make the long-awaited change from $Id$ to $FreeBSD$

This will make a number of things easier in the future, as well as (finally!)
avoiding the Id-smashing problem which has plagued developers for so long.

Boy, I'm glad we're not using sup anymore. This update would have been
insane otherwise.

You will find enclosed some changes to make gcc -Wall more happy in
/usr/src/bin. Note that some patches are still needed in that directory.

I (Joerg) finished most of Philippe's cleanup. /bin/sh will still
need *allot* of work, however.
