mdoc: drop redundant .Pp and .LP calls

They have no effect when coming in pairs, or before .Bl/.Bd

More mdoc nitpicking to improve compatibility to mdocml

- .Nd in section NAME is not optional
- .Ed was missing
- "indent" is not a flag, but a literal argument for -offset
- stop switching font sizes for acronyms
- use .Brq instead of rolling our own

Revert the xterm terminal behavior to NOT clear the screen after
exiting a pager, vi, etc.

Add some example xterm*-clear entries to the termcap files to make
it easier for people to enable that behavior.

Document the examples in the man page to make them easier to find.

Expand *n't contractions.

Mechanically kill hard sentence breaks and double whitespaces.

Add or correct section number in .Xr

Handle the escaped colon \: in a capability.

We don't have rogue(1), and our rogue(6) doesn't use
the `ma' attribute. (The original BSD manual stated
that this attribute is used by vi version 2 only.)

mdoc(7) police:

Avoid using parenthesis enclosure macros (.Pq and .Po/.Pc) with plain text.
Not only this slows down the mdoc(7) processing significantly, but it also
has an undesired (in this case) effect of disabling hyphenation within the
entire enclosed block.

Remove whitespace at EOL.

mdoc(7) police: removed HISTORY info from the .Os call.

mdoc(7) police: Change -filled displays (which just happen
to be the same as -ragged in the current implementation) to
-ragged. With mdocNG, -filled displays produce the correct
output, formatted and justified to both margins.

mdoc(7) police: split punctuation characters + misc fixes.

mdoc(7) police: removed leading whitespaces that are not inside
Bd/Ed; these hardly degrade the quality of the produced output.

Prepare for mdoc(7)NG.

mdoc(7) police: use the new features of the Nm macro.

Remove single-space hard sentence breaks. These degrade the quality
of the typeset output, tend to make diffs harder to read and provide
bad examples for new-comers to mdoc.

Revert $FreeBSD$ to $Id$

Make the long-awaited change from $Id$ to $FreeBSD$

This will make a number of things easier in the future, as well as (finally!)
avoiding the Id-smashing problem which has plagued developers for so long.

Boy, I'm glad we're not using sup anymore. This update would have been
insane otherwise.

Sort cross references.

Correct a bunch of man page cross references and generally
try and silence "manck".

ncurses, rpc, and some of the gnu stuff are still a big mess, however.

This commit was generated by cvs2svn to compensate for changes in r1638,
which included commits to RCS files with non-trunk default branches.
