mdoc: make pages render with mandoc

It's a bit more pedantic regarding .Bl list elements. This has an added
benefit of unbreaking the ipfw(8) manpage, where groff was silently
skipping one list element.

mdoc: drop redundant .Pp and .LP calls

They have no effect when coming in pairs, or before .Bl/.Bd

Cross reference ar(5) from elf(5).

Document extended program header numbering.

Document Elf{32,64}_Lword. Keep the list of ELF types sorted.

Describe the contents of the 'e_shnum' and 'e_shstrndx' fields when
using extended section numbering.

Update the date. Forgotten in previous commit.

Make our ELF64 type definitions match standards. In particular this
means:
o Remove Elf64_Quarter,
o Redefine Elf64_Half to be 16-bit,
o Redefine Elf64_Word to be 32-bit,
o Add Elf64_Xword and Elf64_Sxword for 64-bit entities,
o Use Elf_Size in MI code to abstract the difference between
Elf32_Word and Elf64_Word.
o Add Elf_Ssize as the signed counterpart of Elf_Size.

me whitespace at EOL in troff files with a thick blue block...
