Revert r330897:
This was intended to be a non-functional change. It wasn't. The commit message was thus wrong. In addition it broke arm, and merged crypto related code.
Revert with prejudice.
This revert skips files touched in r316370 since that commit was since MFCed. This revert also skips files that require $FreeBSD$ property changes.
Thank you to those who helped me get out of this mess including but not limited to gonzo, kevans, rgrimes.

- Switch to using the common MII bitbang'ing code instead of duplicating it. - Based on lessons learnt with dc(4) (see r185750), add bus space barriers to the MII bitbang read and write functions as well as to instances of page switching. - Add missing locking to ed_ifmedia_{upd,sts}(). - Canonicalize some messages. - Based on actual functionality, ED_TC5299J_MII_DIROUT should be rather named ED_TC5299J_MII_DIRIN. - Remove unused headers. - Use DEVMETHOD_END. - Use NULL instead of 0 for pointers.

The r48589 promised to remove implicit inclusion of if_var.h soon. Prepare to this event, adding if_var.h to files that do need it. Also, include all includes that now are included due to implicit pollution via if_var.h

In the ISA case, we call ed_probe_WD80x3 before we call ed_probe_rtl80x9. In the pci case we call ed_probe_rtl80x9 first. In the PCI case we were using the correct nic_offset by accident because softc is initialized to zero. In the isa case we were using the wrong value by accident, since ed_probe_WD80x3 sets the offset value to 0x10. This lead to the identification routines failing. Fix this problem by always initalizing the nic_offset and asic_offset before making ed_{asic,nic}_{in,out}* calls.
