Revert r330897:
This was intended to be a non-functional change. It wasn't. The commit message was thus wrong. In addition it broke arm, and merged crypto related code.
Revert with prejudice.
This revert skips files touched in r316370 since that commit was since MFCed. This revert also skips files that require $FreeBSD$ property changes.
Thank you to those who helped me get out of this mess including but not limited to gonzo, kevans, rgrimes.

Copy head@r302406 to stable/11 as part of the 11.0-RELEASE cycle. Prune svn:mergeinfo from the new branch, as nothing has been merged here.
Additional commits post-branch will follow.

Update d_mmap() to accept vm_ooffset_t and vm_memattr_t.
This replaces d_mmap() with the d_mmap2() implementation and also changes the type of offset to vm_ooffset_t.
Purge d_mmap2().
All driver modules will need to be rebuilt since D_VERSION is also bumped.

The present defaults for the open and close for device drivers which provide no methods does not make any sense, and is not used by any driver.
It is a pretty hard to come up with even a theoretical concept of a device driver which would always fail open and close with ENODEV.
Change the defaults to be nullopen() and nullclose() which simply does nothing.
Remove explicit initializations to these from the drivers which already used them.

Boost sample rate to 1.25 MSPS since that allows us to use a 5Mhz (/4) or 10Mhz (/8) external clock.
Make the interrupt both MPSAFE and FAST, at 610 interrupts a second, and a max time to service of 5 msec, we brake for nobody.
