Replace a number of conflations of mp_ncpus and mp_maxid with either
mp_maxid or CPU_FOREACH() as appropriate. This fixes a number of places in
the kernel that assumed CPU IDs are dense in [0, mp_ncpus) and would try,
for example, to run tasks on CPUs that did not exist or to allocate too
few buffers on systems with sparse CPU IDs in which there are holes in the
range and mp_maxid > mp_ncpus. Such circumstances generally occur on
systems with SMT, but on which SMT is disabled. This patch restores system
operation at least on POWER8 systems configured in this way.

There are a number of other places in the kernel with potential problems
in these situations, but where sparse CPU IDs are not currently known
to occur, mostly in the ARM machine-dependent code. These will be fixed
in a follow-up commit after the stable/11 branch.

Add an API for easily creating userspace threads in kernelspace.

This change refactors the existing create_thread() function to be more
generic. It replaces almost all of its arguments by a callback that can
be used to extract the thread ID and copy it out to the right place, but
also to perform additional initialization steps, such as setting the
trapframe. This also makes the difference between thr_new() and
thr_create() more clear in my opinion.

This function is going to be used by the CloudABI compatibility layer.

It looks like the OpenSolaris compatibility framework already provides a
function called thread_create(). Rename this function to
do_thread_create() and use a macro to deal with the namespacing
conflict. A similar approach is already used for thread_exit().

Finally... Import the latest open-source ZFS version - (SPA) 28.

Few new things available from now on:

- Data deduplication.
- Triple parity RAIDZ (RAIDZ3).
- zfs diff.
- zpool split.
- Snapshot holds.
- zpool import -F. Allows to rewind corrupted pool to earlier
transaction group.
- Possibility to import pool in read-only mode.

Update ZFS from version 6 to 13 and bring some FreeBSD-specific changes.

This bring huge amount of changes, I'll enumerate only user-visible changes:

- Delegated Administration

Allows regular users to perform ZFS operations, like file system
creation, snapshot creation, etc.

- L2ARC

Level 2 cache for ZFS - allows to use additional disks for cache.
Huge performance improvements mostly for random read of mostly
static content.

- slog

Allow to use additional disks for ZFS Intent Log to speed up
operations like fsync(2).

- vfs.zfs.super_owner

Allows regular users to perform privileged operations on files stored
on ZFS file systems owned by him. Very careful with this one.

- chflags(2)

Not all the flags are supported. This still needs work.

- ZFSBoot

Support to boot off of ZFS pool. Not finished, AFAIK.

- Snapshot properties

- New failure modes

Before if write requested failed, system paniced. Now one
can select from one of three failure modes:
- panic - panic on write error
- wait - wait for disk to reappear
- continue - serve read requests if possible, block write requests

- Refquota, refreservation properties

Just quota and reservation properties, but don't count space consumed
by children file systems, clones and snapshots.

- Sparse volumes

ZVOLs that don't reserve space in the pool.

- External attributes

Compatible with extattr(2).

- NFSv4-ACLs

Not sure about the status, might not be complete yet.

Rename the kthread_xxx (e.g. kthread_create()) calls
to kproc_xxx as they actually make whole processes.
Thos makes way for us to add REAL kthread_create() and friends
that actually make theads. it turns out that most of these
calls actually end up being moved back to the thread version
when it's added. but we need to make this cosmetic change first.

I'd LOVE to do this rename in 7.0 so that we can eventually MFC the
new kthread_xxx() calls.

Some ZFS threads needs stack larger than the default 8kB, so use 16kB of
alternate stack if the default is smaller than 16kB.
