selinux: stop passing selinux_state pointers and their offspring

Linus observed that the pervasive passing of selinux_state pointers
introduced by me in commit aa8e712cee93 ("selinux: wrap global selinux
state") adds overhead and complexity without providing any
benefit. The original idea was to pave the way for SELinux namespaces
but those have not yet been implemented and there isn't currently
a concrete plan to do so. Remove the passing of the selinux_state
pointers, reverting to direct use of the single global selinux_state,
and likewise remove passing of child pointers like the selinux_avc.
The selinux_policy pointer remains as it is needed for atomic switching
of policies.

selinux: clean up selinux_enabled/disabled/enforcing_boot

Rename selinux_enabled to selinux_enabled_boot to make it clear that
it only reflects whether SELinux was enabled at boot. Replace the
references to it in the MAC_STATUS audit log in sel_write_enforce()
with hardcoded "1" values because this code is only reachable if SELinux
is enabled and does not change its value, and update the corresponding
MAC_STATUS audit log in sel_write_disable(). Stop clearing
selinux_enabled in selinux_disable() since it is not used outside of
initialization code that runs before selinux_disable() can be reached.
Mark both selinux_enabled_boot and selinux_enforcing_boot as __initdata
since they are only used in initialization code.

Wrap the disabled field in the struct selinux_state with
CONFIG_SECURITY_SELINUX_DISABLE since it is only used for
runtime disable.

selinux: always return a secid from the network caches if we find one

Previously if we couldn't find an entry in the cache and we failed to
allocate memory for a new cache entry we would fail the network object
label lookup; this is obviously not ideal. This patch fixes this so
that we return the object label even if we can't cache the object at
this point in time due to memory pressure.

selinux: wrap global selinux state

Define a selinux state structure (struct selinux_state) for
global SELinux state and pass it explicitly to all security server
functions. The public portion of the structure contains state
that is used throughout the SELinux code, such as the enforcing mode.
The structure also contains a pointer to a selinux_ss structure whose
definition is private to the security server and contains security
server specific state such as the policy database and SID table.

This change should have no effect on SELinux behavior or APIs
(userspace or LSM). It merely wraps SELinux state and passes it
explicitly as needed.

selinux: make the netif cache namespace aware

While SELinux largely ignores namespaces, for good reason, there are
some places where it needs to at least be aware of namespaces in order
to function correctly. Network namespaces are one example. Basic
awareness of network namespaces are necessary in order to match a
network interface's index number to an actual network device.

This patch corrects a problem with network interfaces added to a
non-init namespace, and can be reproduced with the following commands:

selinux: reduce the number of calls to synchronize_net() when flushing caches

When flushing the AVC, such as during a policy load, the various
network caches are also flushed, with each making a call to
synchronize_net() which has shown to be expensive in some cases.
This patch consolidates the network cache flushes into a single AVC
callback which only calls synchronize_net() once for each AVC cache
flush.

include cleanup: Update gfp.h and slab.h includes to prepare for breaking implicit slab.h inclusion from percpu.h

percpu.h is included by sched.h and module.h and thus ends up being
included when building most .c files. percpu.h includes slab.h which
in turn includes gfp.h making everything defined by the two files
universally available and complicating inclusion dependencies.

percpu.h -> slab.h dependency is about to be removed. Prepare for
this change by updating users of gfp and slab facilities include those
headers directly instead of assuming availability. As this conversion
needs to touch large number of source files, the following script is
used as the basis of conversion.

The script does the followings.

* Scan files for gfp and slab usages and update includes such that
only the necessary includes are there. ie. if only gfp is used,
gfp.h, if slab is used, slab.h.

* When the script inserts a new include, it looks at the include
blocks and try to put the new include such that its order conforms
to its surrounding. It's put in the include block which contains
core kernel includes, in the same order that the rest are ordered -
alphabetical, Christmas tree, rev-Xmas-tree or at the end if there
doesn't seem to be any matching order.

* If the script can't find a place to put a new include (mostly
because the file doesn't have fitting include block), it prints out
an error message indicating which .h file needs to be added to the
file.

The conversion was done in the following steps.

1. The initial automatic conversion of all .c files updated slightly
over 4000 files, deleting around 700 includes and adding ~480 gfp.h
and ~3000 slab.h inclusions. The script emitted errors for ~400
files.

2. Each error was manually checked. Some didn't need the inclusion,
some needed manual addition while adding it to implementation .h or
embedding .c file was more appropriate for others. This step added
inclusions to around 150 files.

3. The script was run again and the output was compared to the edits
from #2 to make sure no file was left behind.

4. Several build tests were done and a couple of problems were fixed.
e.g. lib/decompress_*.c used malloc/free() wrappers around slab
APIs requiring slab.h to be added manually.

5. The script was run on all .h files but without automatically
editing them as sprinkling gfp.h and slab.h inclusions around .h
files could easily lead to inclusion dependency hell. Most gfp.h
inclusion directives were ignored as stuff from gfp.h was usually
wildly available and often used in preprocessor macros. Each
slab.h inclusion directive was examined and added manually as
necessary.

6. percpu.h was updated not to include slab.h.

7. Build test were done on the following configurations and failures
were fixed. CONFIG_GCOV_KERNEL was turned off for all tests (as my
distributed build env didn't work with gcov compiles) and a few
more options had to be turned off depending on archs to make things
build (like ipr on powerpc/64 which failed due to missing writeq).

* x86 and x86_64 UP and SMP allmodconfig and a custom test config.
* powerpc and powerpc64 SMP allmodconfig
* sparc and sparc64 SMP allmodconfig
* ia64 SMP allmodconfig
* s390 SMP allmodconfig
* alpha SMP allmodconfig
* um on x86_64 SMP allmodconfig

8. percpu.h modifications were reverted so that it could be applied as
a separate patch and serve as bisection point.

Given the fact that I had only a couple of failures from tests on step
6, I'm fairly confident about the coverage of this conversion patch.
If there is a breakage, it's likely to be something in one of the arch
headers which should be easily discoverable easily on most builds of
the specific arch.

SELinux fixups needed for preemptable RCU from -rt

The attached patch needs to move from -rt to mainline given preemptable RCU.
This patch fixes SELinux code that implicitly assumes that disabling
preemption prevents an RCU grace period from completing, an assumption that
is valid for Classic RCU, but not necessarily for preemptable RCU. Explicit
rcu_read_lock() calls are thus added.

Currently network traffic can be sliently dropped due to non-avc errors which
can lead to much confusion when trying to debug the problem. This patch adds
warning messages so that when these events occur there is a user visible
notification.

SELinux: Convert the netif code to use ifindex values

The current SELinux netif code requires the caller have a valid net_device
struct pointer to lookup network interface information. However, we don't
always have a valid net_device pointer so convert the netif code to use
the ifindex values we always have as part of the sk_buff. This patch also
removes the default message SID from the network interface record, it is
not being used and therefore is "dead code".

[NET]: Make device event notification network namespace safe

Every user of the network device notifiers is either a protocol
stack or a pseudo device. If a protocol stack that does not have
support for multiple network namespaces receives an event for a
device that is not in the initial network namespace it quite possibly
can get confused and do the wrong thing.

To avoid problems until all of the protocol stacks are converted
this patch modifies all netdev event handlers to ignore events on
devices that are not in the initial network namespace.

As the rest of the code is made network namespace aware these
checks can be removed.

Linux-2.6.12-rc2

Initial git repository build. I'm not bothering with the full history,
even though we have it. We can create a separate "historical" git
archive of that later if we want to, and in the meantime it's about
3.2GB when imported into git - space that would just make the early
git days unnecessarily complicated, when we don't have a lot of good
infrastructure for it.

Let it rip!
